Dear Friends,
Twelve years ago, a team of local leaders were challenged to develop a solution that
would fill the healthcare gap in our community. Utilizing the health center model,
Crescentwasformed and began providing high-quality, affordable, medical and
dental care to those who otherwise had nowhere to turn.Today,we are fulfilling
our responsibility by not only serving the people of Dubuque but providing care to
patients in the tri-states.
Since opening our doors Crescent has become one of the nation's top health
centers exceeding nationally established benchmarks for quality care year after
year. In 2018 Crescentwas awarded the Health Center Quality Leader award, given
bythe Health Resources and ServicesAdministration (HRSA). As a Health Center
Quality Leader, Crescent achieved the best overall dinical performance among all
health centers, placing in the top 30% ofthe adjusted quartile rankingsfor Clinical
Quality Measures. While Crescent has always been about quality, affordability, and
access,this award from H RSA demonstrates that ou r providers and staff continue
to strive for the best patient outcomes while remaining affordable and accessible
to all, all while being committed to our mission.
Community health centers continue to be economic drivers, having a $35.5 million
annual economic impact in lowa alone. We are also cost-effective Medicaid
providers, serving 13.6% of all Medicaid beneficiaries in lowa, but representing
onlyl2l% ofthe state of lowa'stotal Medicaid expenditures. In our local service
area Crescent has a 23% petention rate ofthe uninsured population and a 34%
penetration rate ofthetotal Medicaid population.
In 2018, Crescent served over 6,300 patients with this number expected to grow
to 8,000 by2021. With growth comes logistical challenges and opportunities.To
address our space and service needs, Crescent will be moving to a new facility,
expanding our space, and staying true to our community. Construction iswell
underway and we expect to move in the fal l of 2019 to our new faci lity at 1690 Elm
Street. We lookforward to the exciting opportunities this expansion will bring to
our community!

TREATING CHRONIC CONDITIONS
My name is Tammy and I have been a Crescent patient since 2011. I was originally
referred here by a friend of mine and have been receiving medical services ever since.
In 2012 I had a chronic cough condition and was finally diagnosed with the help of my
nurse practitioner,Jenny Zalaznik,who monitored my symptoms and made a referral to
the pulmonologistwho diagnosed me with right middle lobe syndromewhich affects
the right lung and bronchial airway. Working with Jennyshetookthetimeto listen
to my health concems to find the cause and make the necessary referrals. She is truly
focused on patientcare, ratherthan making mefeel like a number.
If I didn't have Crescent, I would probably be making frequenttrips to receive medical
care in lowa City.Additionally,for the last six months I have become part ofthe patient
and family advisory council so I can advocatewhatthe patients hears and see, and offer
suggestions to improve patient/provider relationships.

and medical care as a substantial wayto advance access for low-income
populations, ensure that care is affordable, and provide data to help decision-
makers develop and implementforward-thinking policies. AtCrescent,we

healthcare costs that average $560 higher than those with just a chronic

Crescent has taken valuable initial steps to integrate behavioral healthcare + ���„��
into our primary care services. In 2018, 966 patients received behavioral health o'$
treatment at Crescentthrough our partnership with Hillcrest Family Services. ' ��'�'
Additionally,196 patients received substance use disorder treatment omsite �'�� �
through our partnership with the Substance Abuse Services Center. Crescent
has plans to expand these services in 2019 as we move into the new dinic.
Crescentwilladdtreatment aaliststothecurrentprogram, allowingus
