This written: report constitutes your claim against the. City of Dubuque, Iowa. You should

The. Claim: must be filed with the City Cleric. at City Hali, 50 W. 1.3th St., Dubuque, IA 52001. It

Extensive water was found to have leaked into our floor pits within our foundry. The water was due
to tha city water. rnain Breaks that occurred. along Chavenelle Rd.

No personal injury occured . We caught the issue before it could have lead to this.

damage.) The large amount of water caused $66,746.63 worth of expense to fix and correct.

Loss of revenue due to foundry production being halted.
