All services shall be performed in accordance with the Master Professional Services Agreement currently in force. Any attachments or exhibits referenced in this Agreement are made part of this Agreement. Payment for these services will be on a lump sum basis. Approval: MSA shall commence work on this project in accordance with your written authorization. This authorization is acknowledged by signature of the authorized representatives of the parties to this Agreement. PROJECT MANAGEMENT ($1,000) Project Manager, Sarah Fosbinder, will serve as the single pointof contact for the City on this project. Sarah will provide open lines of communication to track progress on major deliverables and notify of any time -sensitive information that might be needed from the City. TOPOGRAPHIC SURVEY AND BASE MAPPING ($2,000) MSA will request any and all underground utility documentation from the City to consider in the site investigation. Utility locates will be requested or the project site ahead of the survey crew. Photos will be taken ahead of construction to generally documentpre-construction conditions. Topographic survey will be conductedfor the project corridor from right-of-way to right-of-way, and a base map will be created for use in design. EASEMENT EXHIBITS ($1,600) MSA will provide (3) temporary construction easement exhibits for approximately 410' of new water main installation. Existing permanent utility easements are in place and will be utilized as best possible. (1) permanent easement exhibit will be created for water main crossing the driveway at 15485 Southwood Court. The preparation of Easement Agreements are excluded from MSA's scope of services. PRELIMINARY DESIGN PHASE ($3,000) In this phase of the design, MSA will draft preliminary plans for the project to reflect the City's comments from the scoping meeting and review of City -provided data. A preliminary opinion of probable cost will be developed based on the project components as approved by the City staff. MSA will prepare plans and draft specifications including the special provisions and project bid form for review and comment. MSA will deliver 60% documents to the City for review and comment. MSA will attend 1 meeting with City staff to present 60% design documents. FINAL DESIGN AND PERMITTING ($3,000) In the final design phase, MSA will complete drawings and specification to be bid ready. The bid -ready set of plans and project manual (specifications) will be provided to the City staff forfinal review and comment prior to bidding. MSA will develop construction documents for the water main improvements. MSA will provide the construction contract for the project that will include the City's "front end" bidding documents, including the City of Dubuque Standard Specification (SUDAS), General Conditions, Supplemental Specifications, the City standard for the contractor's insurance requirements, etc. Contract documents include drawings and specifications that will be prepared in accordance with the City Standard Specification and Engineering Guidelines. MSA will complete a documented QA/QC on all final deliverables and prepare DNR construction permits for the project. Alternate bid items will be in place for potential connection to new 6-inch water main by adjacent properties at 15485 and 15460 Southwood Court. Dubuque Water Department shall provide specifications and dimensioned drawings of City -provided water meter, backflow preventor, and pressure reducing valve for use in project plans. CONSTRUCTION COST ESTIMATE ($500) MSA will develop itemized preliminary and final construction estimates based on the documents developed during design. PROJECT BIDDING ($1,000) MSA will provide bid documents including drawings and specifications to the City for advertising and posting on the City website. MSA will contact local contractors to generate Contractor interest in the project. MSA will administer bidding efforts including answering bidder questions and developing addenda if formal clarification is required. MSA will recommend Contractor awardto City staff. CONSTRUCTION ADMINISTRATION ($2,500) Construction administration efforts include review of shopdrawings, change orders, and payment application requests. A formal preconstruction meeting is excluded. CONSTRUCTION AS -BUILT DRAWINGS ($1,000) Detailed as -built drawings including collection of GIS data points for all new water system and sanitary sewer features will be provided to the City to aid in record keeping. As -built drawings in AutoCAD 2020 format as well as two printed 11x17 copies will be provided to the City of Dubuque. CONSTRUCTION MAKING ��i,i00) MSA will provide one-time construction staking for the proposed water main alignment upon 3-days notice from the Contractor. FULL TIME ON -SITE CONSTRUCTION OBSERVATION SERVICES ($9,000) MSA's On -site Engineer will be available to oversee Contractor means and methods, document quantities installed, and take daily field notes and photos made available to the Owner. The On -site Engineer will also collect GIS data points as construction progresses for integration with the City's GIS mapping system. This fee was estimated based on active underground construction progressing for 10 working days with the On -site Engineer having 80 hours of time onsite. The contract amount may be adjusted upon Owner's request based on the Contractor's project schedule or the Owner's desired level of time commitment onsite. SCHEDULE MSA will endeavor to meet the Dubuque Water Department's quick schedule with the goal of having final bid documents prepared within (8) weeks following receipt of the signed task order. Please understand that DNR construction permit review timeline is out of MSA's control.
