Horsfall, WPC Plant Manager SUBJECT: Final Acceptance of the Catfish Creek Lift Station Pump Replacement Project NTRODUCTION The purpose of this memorandum is to recommend final acceptance of the Water Pollution Control Plant's Catfish Creek Lift Station Pump Replacement Project at a final cost of $344,445.96. DISCUSSION The Catfish Lift Station Pump Replacement project included the following: 1. Replacement of the pumps with new more efficient submersible 85 HP pumps. 2. Replacement of the motor control center and liquid rheostat pump controls with variable frequency drives. 3. Installation of a communitor (grinder); and 4. Replacement of the hydraulic activated check valves with cushioned check valves. The Catfish Pump Replacement Project was awarded to Portzen Construction of Dubuque, Iowa Substantial completion took place December 24, 2000. The original contract price with Portzen Construction was $336,680.00. A total of 2 change orders were issued to Portzen Construction and the result was an increase of $7765.96, bringing the total cost of the project to $344,445.96. Presently, all construction work and punch list items have been completed. Portzen Construction has submitted an application for final payment of $17,222.30. We have not yet received signed final lien waivers and sales tax forms from Portzen Construction. Bob Baumhover of Portzen Construction has assured me that they would be delivered within thirty days after acceptance of the project by the Dubuque City Council. REQUESTED ACTION All construction work for the Catfish Creek Pump Replacement Project has been completed and 1 am requesting your approval and recommendation to the City Council that they adopt the two attached resolutions, one approving the project and one approving the final cost of $344,445.96 and authorizing final payment of $17,222.30 in accordance with State of Iowa statutes and receipt of sales tax forms and signed final lien waivers. RESOLUTION NO. 216-01 ACCEPTING IMPROVEMENT Whereas, the contract for the Catfish Creek Lift Station Pump Replacement Project has been completed and the City's consultant, I.I.W. Engineers and Surveyors, P.C. of Dubuque, Iowa has recommended acceptance of the project. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF DUBUQUE, IOWA: Section 1. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF DUBUQUE, IOWA: Section 1. That the cost of said improvement is hereby determined to be $344,445.96. Section 2. That the final payment of $17,222.30 be paid to Portzen Construction Inc. in accordance with State statues from funds in the Water Pollution Control Department FY 2001 ClP Budget. IA 52003 Re: City of Dubuque - Catfish Creek Pump Replacement Project 1999 Substantial Completion IIW Project No. 94097-03 Dear Paul: Final inspection of Catfish Creek Pump Replacement Project 1999 has been completed. By way of this letter we hereby certify that Portzen Construction, Inc. of Dubuque, Iowa, has completed construction of Catfish Creek Pump Replacement Project 1999 in substantial compliance with the drawings and specifications therefore except for the following items: Pump controls need to be properly set so that pumps alternate lead and lag properly Software program for the PLC must be furnished to the wastewater plant staff Clicking noise of the No. 1 Flygt puml~ needs to be exan~med by Electric Pump Heating element in the sluice gate operator should be nm at 120 volt ~he starter bucket for the ventilation fans needs to be installed A telephone wire patch in the pump control panels needs to be finished The new bubbler stainless steel lines that are to replace plastic hose need to be completed - The hatch frame on the ground floor level in dry well needs to be bolted in place - Rebar in pump discharge piping location needs to be trimmed back to concrete ['ace - Plant staff will complete the dry well sump pump installation and wiring and the connection of the grinder drive shaft and motor The final project construction cost including Change Order No. 2 is $344,445.96. Change Order No. 2 was previously approved by the City of Dubuque. Enclosed is Contract Payment No. 7 in the amount of $22,496.00 for city approval and payment. We recommend that the City of Dubuque approve and accept the Catfish Creek Pump Replacement Project 1999 work. Retainage in the amount of 517,222.30 should be held for 30 days and paid ar that time provided the above items have been completed and provided the sales tax forms and original signed final lien waivers are submitted by the contractor and all subcontractors.
