employee's name.) My 2012 Subaru Impreza was parked at the end of my driveway on Burns
St. The City of Dubuque snow plow hit the front driver's side of my car.
The police report indicated the driver of the snow plow was Darren Steil.
8. What were weather conditions like? _Cold, wet, icy roads, possibly raining

10. Did police investigate? Yes, Officer Justin Dura, Badge 6, Case #2023-001315, time of
incident 1:13a.m. on 2/23/23
11. Was anyone injured? No
12. Was any damage done to property? (If so, describe property and the extent of damages.
Attach estimates of damages or describe basis for ascertaining extent of damage.)
There is damage to my front driver's side hood, bumper, fender light, side mirror (broken). My
front driver's side tire popped and is on the rim. There is also damage underneath the hood. There
is most likely damage to my driver's side tire/wheel assembly and possibly mechanical damage
underthe hood.

No
15. What amount do you claim from the City of Dubuque? Repair costs or total loss coverage
depending on what is determined. Rental Reimbursement (Amount Unknown at this time),

16. Why do you claim the City of Dubuque is responsible? The city's snow plow hit my parked
car.
