You should complete this form in full and attach any additional information that supports your claim. The Claim must be filed with the City Clerk at City Hall, 50 W. 131h St., Dubuque, IA 52001. It will then be referred by the City Council to the appropriate department for investigation. Once that investigation is completed, a report and recommendation will be submitted to the City Council. You will be provided with a copy of that report and recommendation. THE FINAL DECISION ON ALL CLAIMS IS MADE BY THE CITY COUNCIL. Location of Incident (Be specific): City parking ramp directly south of the Grand Theatre. 7. DESCRIBE ACCIDENT OR OCCURRENCE THAT CAUSED INJURY OR DAMAGE. (Give full details upon which you base your claim. If a City employee was involved, give the employee's name.) I attended the play at the Grand Threatre. I had to park on the 2nd level of the City parking ramp. When I went to leave we could not see the sign for down only the sign for going up was lit. As I turned to the left trying to find the exit, my passenger in the front seat saw the sign for down that was burnt out as I past it. I made a 3 point turn, DacKing up into the next aisle way that wasn't occupied & went back to where my passenger saw the sign hitting the high curb that protrudes out and is not marked with edge paint or a pylon making it visible. 8. Attach estimates of damages or describe basis for ascertaining extent of damage.) Front lower bumper cover and fender liner were damaged. See Kroeger Body Shop estimate attached. 13. What other damages do you claim, if any? None 14. Have you been compensated for any part or all of your claim by any Insurance company? The city is negligent because lighting is poor, the light for down was OUT & couldn't be seen & a high curb (at least 10 inches high) protrudes out without any paint marking the edge or a pylon making the edge visible. 17.
