The City has had a long standing partnership with Four Mounds Foundation. This work
session allows Four Mounds Foundation to share information on their past, current, and
future work and initiatives many of which relate to the City Council's equity goals.
The City owns Four Mounds Park and entered into a lease agreement with Four
Mounds Foundation in 1988 with a term of 49 years. In the agreement Four Mounds
Foundation is responsible for all ground maintenance and improvements to the grounds
in addition to all buildings maintenance and improvements. Any proposed material
improvements or modifications are submitted to the City for approval.
This has been a very fruitful and successful partnership for both the City and Four
Mounds Foundation. Their planning and care for this historic property has led to many
restorations and improvement projects that are funded by the Foundation. Four
Mounds Park is available to all residents as visitors to enjoy.

Greetings from Four Mounds,
Summer is here and Four Mounds is buzzing with activity! In addition to our vocational programming,
Adventure Day Camp has started, welcoming 25 campers each week. We love the hustle and bustle
and all the learning that takes place each day.
With the Four Mounds site activated by many programs, our team is intentional about gathering
every morning for what we call "group up." This is a time to check in with each other, identify daily
goals, address any potential setbacks and set a positive tone for the day. Here, we also take the
opportunity to acknowledgejobs well done, celebrate birthdays, employment opportunities, and

Goals Snapshot-Transitional Employment Pathway
Goal work is an essential component of all that we do. Students look forward to the time set aside each week for
them to work on their goals.They work in small groups or one-to-one with mentor staff.Though we are always

For more than a decade,Jacob's grandmother has been his caretaker and the one constant in his life.
� Tragically,his grandmother lost her battle with cancer last December.Jacob had to make grown-up
decisions quickly.One change Jacob had to navigate was finding housing. Because he is on our
Transitional Employment Pathway, he got the support to navigate these changes while at work. He has
�s f� ' made lasting friendships and taken up fishing as a hobby in his free time.

Arica has struggled with coming to work with a positive attitude.She has overcome this by showing up

HEART Partnership Job Site Updates
Four Mounds Proper
Spring and summer are always busy times at Four Mounds. Students have been working hard
in the woodshop, caring for the chickens, milling wood from fallen trees on site, repairing
furniture and woodworking, and doing groundwork and landscaping. Some of the projects this
quarter include plumbing repair and weatherization of water lines at the cabin, repair of
Adirondack chairs, the design and construction of a picnic table that will outlive us all,
construction of corn hole yard game boards using the CNC router, painting, framing and
building an interior wall, building a desk for Jacob's new apartment, and building a table for
our meeting room in our new office space complete with the Four Mounds logo.

Students working at 514 Angella Street are learning a lot in a short time. This past quarter they
finished indoor framing, replaced all windows, insulated, and are currently working on siding
and soffit while the weather cooperates. They also helped our team with drywall on the Garfield
site for a couple of days. This group of students is ready to join the workforce! These students
also earned their Flagger and Heavy Equipment Operator Certification through Northeast lowa
Community College this quarter.

This is the current project for our youth program, HEART Building Futures. We celebrated
3 graduates in May and hired two students from this program for the summer. Students on this
project learned how to read a blueprint, finished all interior framing, learned all processes and
pros and cons of spray foam insulation and drywall, took an active role in working with our
electrician to wire the entire home, and prepped the exterior for siding and soffit this summer.

Home Modifications
Students on this pathway are completing home modifications for local, income-qualifying seniors to
age safely in place. We recently completed our 20th project on this pathway. Homeowners complete
a satisfaction survey when our work is complete. To date, we have 100% positive feedback in the
areas of professionalism, quality of work, and increased safety for homeowners. Tub cutouts, as in
the picture below, are a popular and cost-effective home modification. This is not easy work and
requires some agility and ability to work in tight spaces. Students are now completing this work
independently with guidance and direction from theirjob coach.

Looking Ahead
The HEART Partnership programs have grown and thrived over the past four years,
requiring Four Mounds to explore opportunities to create a designated home base for
educational and workshop programming for our many students. We have received
generous funding support from the Dubuque County Board of Supervisors to embark upon
the development of a new versatile facility at Four Mounds and we look forward to
engaging the community in this process. The HEART program model is effective, high-
impact, and replicable and we know this isjust the beginning.

The HEART Partnership advances key City Goals and Priorities while also addressing
the national and local housing and workforce crises. We are doing this as well as meeting the individual
needs of our students to help them overcome barriers to employment.
Robust Local Economy: Diverse Businesses and Jobs with Economic Prosperity
The HEART Partnership is building workforce development and economic vibrancy through restoration
projects that revitalize and invest in our urban core. Our housing projects are positively impacting
downtown economic prosperity and creatingjobs and opportunities for adults who are unemployed,
underemployed, and who have been incarcerated, through holistic education and hands-on training in
historic restoration trades and more.
Vibrant Community: Healthy&Safe
Our team holistically and individually addresses participants' needs from food and transportation, to
brain health and life skills mentoring. Projects completed in Dubuque's downtown invest in dilapidated
buildings and neglected neighborhoods. These projects build neighborhood and community pride which
translates into further investment and safe neighborhoods. Further, we have completed more than 20
home modification projects in Dubuque making homes safer for local seniors to safely age in place.
Livable Neighborhoods& Housing: Great Place to Live
Students in the HEART Partnership programs are transforming distressed properties and buildings into
homes typically sold to first-time homebuyers. Because of the addition of our adult programs, our teams
are completing two gut rehabilitation projects simultaneously in partnership with the City as well as
projects on-site and 2 or more home modifications projects per month.
Sustainable Environment: Preserving and Enhancing Natural Resources
As the last remaining intact gentleman's farm in lowa, and with 100 acres of rare natural resources, Four
Mounds is dedicated to preserving and enhancing the natural resources at the site. Students in all of our
programs learn the art of window restoration and students on our Transitional Employment Pathway
recently restored three picnic tables for Eagle Point Park. These tables are thought to have been
originally constructed in 1937. In addition to the gut rehabilitation project students in our adult program
are completing, they recently built raised beds for the Dubuque Food Pantry using materials reclaimed
from the barn torn down at our farm property.

By working to recruit and attract community members who are frequently underserved, the HEART
Partnership is a critical partner in building equity in Dubuque. We strive to expand partnerships in our
effort to enhance programs.

LEASE AGREEMEHT
� THIS LEASE , made and executed between the CITY OF DUBUQUE , IOWA , a
municipal corporation ( Lessor ) , and the FOUR MOUNDS FOUNDATION , an Iowa
! nonprofit corporation , with its principal office in Dubuque , Iowa ( Lessee ) .

WHEREAS , Lessor is the owner � f legal title to a tract of land known as
Four Mounds Park ( the Park ) , legally described on Exhibit " A " attached
hereto and incorporated herein by this reference ; and
� WHEREAS , Lessor and Lessee desire to enter into an agreement whereby
Lessor leases to Lessee the Park upon the terms and conditions hereinafter
stated .
NOW THEREFORE , in consideration of the mutual covenants and agreements
of Lessor and Lessee , each to the other , it is agreed as follows :
ARTICLE I
, Term of Lease
Lessor hereby leases to Lessee the Park for a term of forty - nine ( 49 )
years , begi nni ng on the day of , 1988 , and endi ng on
the day of , 20 , un ess extended by additional
written agreement between the parties , or under the terms of this article .
Commencing on the day of , 2037 , this lease may
be extended by Lessee for up to three 3 additional successive periods of
fifteen ( 15 ) years each , unless terminated at the end of the original lease
or any extension thereof , upon resolution and notice by the Lessor as provided
herein . In order for the Lessee to extend this lease for an additional
, period of fifteen ( 15 } years , the Lessee shall notify the Lessor that Lessee
wishes to extend the lease for such an additional � eriod ; such notice shall
be given in writing , by delivery to the Dubuque City Clerk , not later than
two ( 2 ) years before the comnencement of the desired extension period . Upon
the receipt of such a notice to extend , the Lessor may nonetheless avoid
such an extension by completing all of the following acts not later than one
( 1 ) year before the commencement of such extension period :
( a ) Lesso ^ must give Lessee at least thirty ( 30 ) days written notice of
Lessor ' s intent to consider termination of the lease , which notice
must make specific reference to this provision and indicate the
time and place for public hearing on the matter ;

maintenance , roof repair , pest control , sewer system and water system . ,
Any improvements made by the Lessee shall remain on the leased premises
upon termination of this 7ease and shall become the property of the lessor .
Any proposed materia � improvements or modifications upon the leased premises
shall be submitted , in advance , for app � oval by the the Lessor by the Pa �k
and Recreation Director , which approval shall not be unreasonably withheid , '
- and which proposed improvement or modification shall be considered in the
context of the declaration of purppse stated in Article IV above . '

If it becames neeessary , for reasons of � conomic neeessity or for
effective overall development of the Park , the � �ssee may request the right
f rom the Lessor to rer�ove any of the residential or support buildings , and
permission sha11 not be unreasonably withheid by Lessar ,
ARTICLE VI
Management and Control of the Park
It as expressly agreed and � nderstoad between the parties that sole
management authority and control of the Park , for and within the purposes
stated in Article IV above shall be in the Lessee , and Lessor shall have no
authority or power to manage or control , nor to approve , disapprove , direct
or restrict action by the Lessee except as provided herein .
ARTICI. E VII
Insurance
Lessee shall insure the buildings and contents with fire and extended
coverage insurance , including wind damage , vandalism and theft , payable to
the � essor and Lessee as their interests may appear , or , in the alternative ,
at the request of Lessee , Lessor shall secure such insu � ance , and Lessee
shall pay Lessor , in advance , for the additional p �emium cost allocated to
such i � surance of leased buildings . In the event of a partial destruction
or damage of any of the buildings on the leased premises , in the amount of
ten thousand dollars ( $ 10 , 0 (! 0 ) or less , the proceeds of insurance shatl be
used at Lessee ' S option for the repai � of such damage . With respect to su� h
proceeds of insurance which are not used ta repair the damage , or with
respect to proceeds of insuran �e for loss or damage in exCess of ten
thousand dollars ( � 1C� , 000 ) , Lessor and Lessee a� gree ta negotiate , in good
faith regarding the use to be made of suCh proteeds for the benefit of Four
Mounds Park and for puDlic park purposes as defined in Artiele IV above .
Lessee agrees ta procure and maintain enforced during the term of the
lease a polity of general publie liability insurance insur9ng the liability
of Lessee and Lessor from any tlaims arising out of the ieasing of the Park ,
or any activity or operations conducted therein . Said liabitity policy '
shall be written with minimum limits of not less than five- hundred thousand
dotlars ( $ 500 , OOQ ) for e �ch OCGurrence and not les5 thdn one mililon dotlars
( $ 1 , 000 , 000 } in the aggregate , and not less than fifty thousand ( $ 50 , 000 )
for property damage . Lessor shall be furn9shed a true copy of Sd1d
liability policy written with a company authorized to do business in the '
State of Iowa and said policy shall contain a provision that said policy '
shall not be cancelled or materially altered without first having given ten '
( 10 ) days prior written notiGe to �essor . Prior to every tenth anniversary '
of the Co�nencement of this lease , Lessor and Lessee shall review the
minTmum limits of liability stated in this paragraph to determine whether or
not such minimum limits are sufficient adequately to protect the interests '
i'R of Le'ssor and Lessee ; in the absence af written memorandum of agreement '
regarding minimum limits to be maintained , Lessor and Lessee agree that on

ev� ry tenth anniversary , the minimum limit5 to be maintained during the
suc �eeding ten ( 10 ) year period shall be adjusted up or down in proportion
to the net change in the Consumer Price Index , " Urban , All - Items " , sinee the
last previous tenth anniversary , provided , further , tha� in no event shall
the minimum limits to be maintained be reduced betow the original limits
fixed by this paragraph . If , during the te �n of thi5 lease , publication of
the Consumer Price tndex is terminated , the �djustment eontemplated by this
paragraph shall be calculated through use of the federally - published
statistical data o � index most closely dpproximdting in fUnction the
Consumer Priee Index , " UrDan , All - Items " , as it exists at the time of the
commeneement of this lease .
Lessor agrees to continue its liabiiity coverage until May 1 � 1988 , and
to name the Lessee as an additionaT insured on its policy .
ARTICI� VIII
Indemnity
Lessee agrees to save , defend and indemnify and hold harm1e55 Lessar ,
� ts officers , agents and employees against any and aI1 toss , damage , claims ,
causes of action , costs and expenses which Lessor may hereafter suffer ,
incur , be put to , pay or lay out by reason of performance by the Lessee
and / or its agents and employees of the conditions of this lease in the
operation and development of the Park .
ARTICLE IX
Assignability/ Subleases
Lessee is authorized to enter into subleases with parties for custody ,
management or ope �ation of the Park or areas thereof , subject to Article IV
above , provided :
That in � o event shall st, ch a sublease extend beyo �d the term
of this lease and any extens9ons hereof , unless the Lessor
consents , in writing , to such sublease ;
That any sublease shall be submitted before the commencement
, of the subledse ; for approval by the LesSor by the Park and
Recreation pirector , which approval shall not be unreasonably
withheld or delayed ;
That in the event of termination of this lease for any �eason
dur� ng the te nn of a sublease which has been approved by the
Lessor , the Lessor ' s recovery of the property shall be subject
to the unexpired te rni of the sublease , plus any options for
renewal or ex � ension thereof , and Lessor agrees to execute an
acknowledgement or subordination agreement to this effett if
��` required by subtenant .

Lessee may , in its sole discretion , pledge , hypothecate or assign rents
and profits under any sublease to secure payment of any indebtedness which
Lessee may incur . Such pledge , hypothecation or assi9nment sha11 , likewise ,
survive the tennination of this lease , for any reason , and Lessor ' s recovery
of the property , follawing termination , shall be subject to such pledge ,
hypothecation or assignment of rents and profits .
ARTICLE X
Taxes and LiCenses
Lessee agrees to pay all taxes , licenses , imposts , special assessme �ts
or other governmental charges , levied or assessed against the ieased
property .
ARTICLE XI
Default
If Lessee shall default on a payment of rental or other lease payment ,
or shall default in a performance or shail make default in the performance
of any of the other covenants herein contained , and such defaults shall
Continue for a period of thirty ( 30 ) days after written notice thereof ,
Specifying the default compiained of , mailed to Lessee by United States
Certified mail , return receipt requeSteQ , then it shali be lawful for
Lessor , a�t its election , to declare the term of this lease ended and to
reenter upon the Park , with or without process of law , and to expel and
remove Lessee and , subject to Ar � icle IX , any person or persons upon the
same , using such force as may be necessary under the circumstdnce5 , but
without breach of the pea � e ; and if at any time , by reason of such default
of the Lessee and the contlnuante thereof for a period of thirty ( 30 ) days
after written notice has been given , said term shall be so ended , Lessee
' hereby covenants and agrees to surrender and deliver up the Park peaceably
to the Lessor , subject to Article IX , above .
If cure of any default spetffied under the foregoing paragraph cannot
be physically completed within such thirty ( 30 ) day period , or if Lessee is
prevented fram cure of SUGh default by acts of God , the elements , the public
enemy , st � ikes , riots , insurrection , government regulation , City ordinance ,
labor ,, material or transportation shortages , or other cause beyond Lessee ' s
reasonable cont � ol , then the running of such thirty ( 30 ) day period shall be
suspended if cure has bee � commenced within the thirty ( 30 ) ddy period and
for so long as such cure is pursued with reasonable diligence thereafter .
If default shall be made by either party in the performance of or the
canpliance with any of the terms , covenants or conditions of this lease , and
such default shall continue for thirty ( 30 ) days afte � written notice
thereof from one party to the othe � , the person aggrieved may , but need not ,
� perform such term , covenant or condition , or make good such default and any
amount advanced shall be repaid forthwith on demand , together with interest
at the rate of nine percent ( 9Y } per annum from date of advance .

In the event the City elec� s to terminate this lease as a conseqvence
of default by Lessee , recovery of the leased property sha11 be the City ' s
remedy , and no othe � right or remedy of the City sha11 survive termination
of the lease .
ARTICLE XII
Effective Date of this Lease
