Steger, Housing and Community Development Director SUBJECT: Fiscal Year 2024 Purchase of Services Grant Amendment & Reallocated Agreement DATE: September 7, 2023 Introduction This memorandum transmits the Fiscal Year 2024 Purchase of Services Grant Amendment for St. Mark Youth Enrichment and an Agreement for St. John's Almost Home as proposed by the Community Development Advisory Commission (CDAC). Purchase of Services Grantees partner with the City of Dubuque to provide support for human service programs that further the City's goals, objectives, and priorities. Background There was $17,080 unexpended funds in FY23 Purchase of Services Grant program. These unexpended funds become carryover dollars. Therefore, the FY24 budget for the Purchase of Services Grant competition increased from $100,000 in general funds to $117,080. The City received ten applications for FY24 Purchase of Services Grant program. A subgroup of CDAC members and two city staff (8 in total) reviewed and scored applications and awarded initial grantees. With the increase in funding, CDAC met on Wednesday, August 16, 2023, and approved funding for the unallocated $17,080. Discussion St. Mark Youth Enrichment applied and requested $25,000 but was only awarded $18,000 because, at the time, only $100,000 was set aside for the Purchase of Service Applications. However, with the additional $17,080, CDAC approved an increase to their award by $7,000 to award St. Mark Youth Enrichment their full request of$25,000. St. John's Almost Home had the next highest applicant score based off the 8 reviewers. St. John's Almost Home requested $25,000 for onsite case management services via a social worker who can facilitate referrals for medical, brain health, addictions, and basic needs. CDAC approved funding St. John's Almost Home with the remaining $10,080. Recommendation I respectfully request the City Council approve the attached Purchase of Services Grant Amendment with St. Mark Youth Enrichment and Purchase of Services Agreement with St. John's Almost Home and authorize the Mayor to execute the agreements on behalf of the City. Prepared by: Mary Bridget Corken-Deutsch, Community Development Specialist RESOLUTION NO. 299-23 RESOLUTION AUTHORIZING EXECUTION OF FY24 PURCHASE OF SERVICES GRANT AGREEMENTS Whereas, the following non-profit organizations prepared and submitted requests to provide support for human service programs in the City of Dubuque: • St. Mark Youth Enrichment Amendment#1 • St. John's Almost Home ; and Whereas, the City Council approved the Fiscal Year 2024 budget that included the Purchase of Services allocation; and Whereas, the submissions have been approved by the Community Development Advisory Commission on August 16, 2023; and Whereas, all services provided will be to a minimum of 51% low/moderate income residents. City Responsibilities City designafes the Community Development Specialist, to act as its representative with respect to the work to be performed under this Agreement, and such person shail have authority to transmit instructions, receive information, interpret and define City's policies and provide decisions in a timely manner pertinent to the work covered by this Agreement until Agency has been advised in writing by City that such authority has been revoked. City wiil provide opportunities to demonstrate this support by inviting the President of the Board of the Agency and any volunteers working on services covered by this agreement to participate in the foilowing professionai development opportunities and supporting activities: a) The opportunity to be active and engaged participants in Inciusive Dubuque peer learning opportunities and strategic efForts to advance Diversity, Equity, and Inciusion; b) Tne opportunity to attend intercultural and equity workshops offered by the City, including trainings that focus an developing an intercultural team with a strategic 1 plan for advancing equity and inciusion within the Agency and through the Agency's work; c) Assistance in creating quarterly status reports that demonstrate the ways in which the Agency's efforts are advancing equity and inclusion; and d) Access to the City's equity toolkit and SMART Goals Worksheet. 3. Time of PerFormance The term of this Agreement shall be from September 5, 2023 through June 30, 2024. 4. Compensation Gity shall pay Agency $10,080 as compensation for the services provided by Agency, which payment shail constitute fuil and complete compensation for such services. The compensation shall be paid by Gity to Agency upon receipt by City of a requisition for such sum from Agency. 5. Audit and Evaluation Inasmuch as the funds to be provided to Agency by City are public funds, the Agency agrees that it will maintain books of account in accordance with Generally Accepted Accounting Standards together with necessary documentation to support aIl expenditures. The records of Agency, inciuding payroll accounts and other records of disbursements and receipts, and other records relating to the performance of the activities herein specified, shail be made availabie for inspection by City or any Certified Public Accountant acting on its behalf, in orderto 'ascertain performance of Agency under this Agreement. Agency agrees to retain such records for not less than three (3} years. 6. Reportinct and Monitorinq Agency shall compiy with the City of Dubuque's Reporting and Monitoring requirements by using the following link to submit quarterly online reports detailing Agency activities related to the use of City funds provided to Agency. The online quarterly reports shali be due on October 15�h, January 15i'', April 15th, and July 15t''. Such reports shall inciude a perFormance report on the program(s) supported by City funds and a financial report on the use of City funds. If Agency has an annual audit performed, Agency shall provide City a copy of the annuai audit report within 60 days of audit completion. The Audit shall be submitted as either an electronic or paper document to the Community Development Specialist. 7. Reimbursement Schedule 2 �he Agency may requisition a reimbursement of expenses for project costs with each quarterly report due on October 15t", January 15t'', Aprii 151", and July 15t". Such reimbursement requests must include proof of payment of the requested expense. Request for salary reimbursement must be within 30 days from end of pay period. Other requests dated within 60 days from date of service/purchase will be accepted. Final request must be submitted by June 15. 2024. 8. Discrimination In carrying out the services under this Agreement, Agency shail not discriminate against any employee or applicant for empioyment because of race, creed, color, sex, age, national origin, religion, sexuai orientation, gender identity, or disability. Agency shail take affirmative action to ensure that appiicants for employment are employed and that employees are treated during employment without regard to race, creed, color, sex, age, national origin, religion, sexuai orientation, gender identity, or disability. Such action shall include, but not be limited to, the following: employment, upgrading, demotion or transfer; recruitment or recruitment advertising; layoff or termination; rates of pay or other forms of compensation; and selection for training, including apprenticeship. Agency assures that ail qualified appiicants will receive consideration for empioyment without regard to race, creed, color, sex, age, national origin, religion, sexual orientation, gender identity or disability. Agency agrees and will undertake whatever affirmative measures are necessary so that no person shall be excluded from participation in, be denied the benefits of, nor be subjected to discrimination under any program or activities funded in whole or in part with any of the funds made available to Agency under the terms of this Agreement because of race, color, creed, religion, sex, national origin, age, sexual orientation, or disability. 9. Termination of Aareement if, through any cause, Agency shall faii to fulfiil in a timely and proper manner its obligations under this Agreement, or if the Agency shall violate any of the covenants, agreements or stipulations of this Agreement, City shali thereupon have the right to terminate this Agreement by giving written notice to Agency of such and specifying the effective date thereof not less than thirty days before the effective date of such termination. Final request for reimbursement must be submitted by June 15. 2024. if Agency shall fail to cure its defauif to the satisfaction of City in City's sole discretion within such seven-day period, this Agreement shail be deemed terminated at the end of such seven-day period without further action of either party. Upon termination of this Agreement, Agency shali return to City ail unexpended funds and/or any funds utilized for purposes other than stated in the Agreement. 10. Assignabilitv Agency shall not assign any interest in this Agreement without the prior written approval of City. 11. Acknowledgement. 3 Agency wiil acknowledge City's financial support in appropriate signage, program materials, publicity, advertising activities, websites, and other online media pertaining to this Agreement and work performed with funds from this Agreement. Conflict of Interest No member, officer or employee of City, or its designees or agents, no member of the Dubuque City Council and no other public official of the City who exercises any function or responsibilities with respect to the program during such person's tenure, may have any direct or indirect interest in this Agreement, the proceeds from this Agreement, or work performed in connection with the program assisted under fhis Agreement. 13. Insurance At the time of execution of this Agreement by Agency, Agency shaii provide City copies of Agency's insurance certificates showing compliance with the insurance requirements of Insurance Scheduie G, included as Attachment A.
