begin work on the following project:

The estimated fee for the work is: $24,780
This authorization for the work described above shall serve as the Agreement between MSA
and OWNER. All services shall be performed in accordance with the Master Professional
Services Agreement currently in force. Any attachments or exhibits referenced in this
Agreement are made part of this Agreement. Payment for these services will be on a time &
materials basis.
Approval: MSA shall commence work on this project in accordance with your written
authorization. This authorization is acknowledged by signature of the authorized representatives
of the parties to this Agreement. A copy of this Agreement signed by the authorized
representatives shall be returned for our files.

SCOPE OF SERVICES
This project scope contains all work necessary to provide design, permitting, bidding, construction administration and on -
site construction services for the Creek Crossing Restoration Project.
PROJECT MANAGEMENT ($1,280)
Project Manager, Sarah Fosbinder, will serve as the single point of contact for the City on this project. Sarah will provide a
Work Breakdown Structure invoice complete with tasks and fees as a progress update to the City of Dubuque on a monthly
basis. This will be a way to keep open lines of communication to track progress on major deliverables and a way to notify
of any time -sensitive information that might be needed from the City.
TOPOGRAPHIC SURVEY & BASEMAPPING ($9,240)
MSA will request any and all underground utility documentation from the City to consider in the site investigation. Photos
will be taken ahead of construction to generally document pre -construction conditions with each site ranked based on
priority/condition. Topographic survey will be conducted for each restoration area to establish the extent of restoration
needed, with right-of-way boundaries defined. A base map will be created for use in design. Survey crew will document
creek profile as best possible, however determination of existing water main location will be based on City as -built
records. Topo survey excludes access routes. City to provide all right-of-way and easement boundary information for
each location.
PRELIMINARY DESIGN PHASE ($1,200)
In this phase of the design, MSA will draft preliminary plans for the replacement of fabric and rip rap along the creek banks and
overtop the water main crossings at the sites labeled on the exhibit herein. MSA will prepare plans utilizing SUDAS standards
with reference to the City's Supplemental Specifications. Design shall include 1 plan & profile sheet for each location with
typical rip rap detail and City -provided as -built notes/sketches. Drawings shall include an access overview map generally
showing the City's designated access routes (from City GIS). Preliminary plans will be submitted to City staff to solicit Owner
feedback.
FINAL DESIGN PHASE ($1,200)
In the final design phase, MSA will incorporate Owner comments from the preliminary. The bid -ready set of plans will be
provided to the City staff for final review and comment prior to bidding.
In addition to developing construction documents for the creek crossing restoration, MSA will prepare and submit DOT Utility
Work in Right -of -Way permit, Joint Flood Plain Permits (one permit for all sites), and Work in Railroad Right -of -Way permit
(Radford Road only) on the City's behalf. DNR Water Supply Permit is not anticipated to be required.
CONSTRUCTION COST ESTIMATE ($240)
MSA will develop itemized preliminary and final construction estimates based on the documents developed during design.
PROJECT BIDDING ($1,400)
Bid form is anticipated to be structured such that each creek crossing restoration area will be bid as a lump sum item. It is assumed
that the project will not exceed public bidding thresholds and that MSA will provide bid documents to a list of Contractors of
the City's choosing to gather competitive quotes. MSA will administer bidding efforts including answering bidder questions
and developing addenda if formal clarification is required (additional cost). MSA will conduct a bid opening, recommend
Contractor award to City staff, and assist with preparing the Short Form Vendor Service Agreement.
CONSTRUCTION ADMINISTRATION ($560)
Construction administration efforts include review of fabric & rip rap shop drawings and preparing change orders if
needed (additional cost). Written project updates will be provided via email to keep the Owner informed of Contractor
progress.
CONSTRUCTION AS -BUILT DRAWINGS ($280)
City will collect GIS data points from existing water main locations. MSA will provide before/after photos to the City following
construction to serve as a record of the work.
PART TIME ON -SITE CONSTRUCTION SERVICES ($6,800)
On -site Engineer, Hunter Nix, will be available to oversee Contractor means and methods, document quantities installed,
and take daily field notes and photos made available to the Owner. This estimated fee for construction observation is
based on 4hrs/day with active construction assumed to progress for 12 working days (48 hours). MSA will keep Owner
updated on onsite time through informal emailed project updates. The contract amount may be adjusted upon Owner's
request based on the Contractor's project schedule or the Owner's desired level of time commitment onsite. Part-time
construction observation is more economical for the City. By bidding each creek crossing as a lump sum item, collection
of rock tickets will not be required, therefore making part-time observation more feasible.
ONE-TIME CONSTRUCTION STAKING SERVICES ($2,300)
Construction staking is included in onsite construction services, assuming one-time staking of the extent of stream bank
stabilization for each of 6 sites. Staking excludes access routes; City shall stake access route boundaries for Contractor
use prior to construction.
PROJECT CLOSEOUT ($280)
MSA will review 1 Contractor Pay Application and provide recommendation of payment to the City of Dubuque.
