Housing & Community Development Director Alexis Steger is recommending City
Council accept and approve $1,431,743.21 for the Older Adult Home Modification grant
from the U.S. Department of Housing and Urban Development and retroactively
approve the HUD-1044 Assistance Award for the grant.
The City of Dubuque was among the first cities awarded the Older Adult Home
Modification Program in 2021. The program was designed to help residents over the
age of 62 safely in their homes. This program focuses on changes in the home that cost
less than $5,000 but provide a safe environment for those with limited mobility that are
low -moderate income. Changes that are made to homes using this program include
things such as grab bars, bathtub cutouts, raising electrical outlets, and installing chair
elevators.
The program requires a certified occupational therapist to meet with the resident and
determine the needs in the home to help with safety. The city partners with Unified
Therapy Services to complete this evaluation. Once completed, the HEART program
installs the needed improvements, and the occupational therapist returns to evaluate
the home in its new state to ensure it is safe for the residents.
The 2021 award was for $1,000,000 and could only be used in homeowner occupied
units. This award ends February 16, 2025, so the city reapplied for funding in August of
2024.
Recently the City of Dubuque was awarded $1,431,743 over a 36-month period to
continue the Older Adult Home Modification Program. This round of funding will allow
the city to assist homeowner and renter occupied structures.

INTRODUCTION
This memorandum provides for the approval of the Older Adult Home Modification Grant
and HUD-1044 Form from the U.S. Department of Housing and Urban Development
(HUD).
BACKGROUND
The City of Dubuque was among the first cities awarded the Older Adult Home Modification
Program in 2021. The program was designed to help residents over the age of 62 safely in
their homes. This program focuses on changes in the home that cost less than $5,000 but

The City of Dubuque was awarded $1,431,743 over a 36-month period to continue the
Older Adult Home Modification Program. This round of funding will allow the city to assist
homeowner and renter occupied structures.
Typically, the City of Dubuque staff participate in a negotiation meeting with HUD
representatives and at the end of that negotiation a HUD-1044 Assistance Award is sent to

the City to be executed within 30 days.
However, this year HUD sent the negotiation package to city staff, but then followed up
with an email that negotiations will not take place until a HUD-1044 is signed by the City.
The email from HUD with this information provided the City one (1) day to complete the
form and return it. Due to the change in process and faster than usual turn around, the
City Manager executed the HUD-1044 Assistance Award for the City of Dubuque on
February 6t", 2025, with the understanding that the City Council would have the option to
retroactively approve the awarded funds/agreement with HUD. This approval ensured the
City of Dubuque would be able to accept the $1,431,743.21 to continue a program already
accepted by the City Council to serve the goals and priorities of the city.
RECOMMENDATION
I respectfully request City Council accept and approve $1,431,743.21 for the Older Adult
Home Modification grant from the U.S. Department of Housing and Urban Development
and retroactively approve the HUD-1044 Assistance Award for the grant.

RETROACTIVELY AUTHORIZING THE FILING OF A HUD-1044 ASSISTANCE
AWARD FOR THE OLDER ADULT HOME MODIFICATION GRANT WITH THE
UNITED STATES DEPARTMENT OF HOUSING AND URBAN DEVELOPMENT
WHEREAS, applications for certifications, funding, and grants are set by outside
entities and outside the control of the City; and
WHEREAS, the deadlines set by outside entities do not always coincide with the
meetings of the City Council; and
WHEREAS, the City was provided the Older Adult Home Modification Grant
HUD-1044 Assistance Award from the U.S. Department of Housing and Urban
Development on February 6t', 2025 with a deadline of February 7th, 2025; and
WHEREAS, the City Manager signed the form and seeks retroactive approval of
the HUD-1044 Assistance Award from the City Council.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF DUBUQUE, IOWA:
Section 1. The Older Adult Home Modification Grant HUD-1044 Assistance Award
submitted to the United States Department of Housing and Urban Development is
approved retroactive to February 6th, 2025.

in the Welcome Package. You are required to submit a negotiation package that must include a
Budget Narrative and Budget Workbook for approval by HUD. Your grant's period of performance
will be adjusted after your formal negotiation is complete.
