This written report constitutes your claim against the City of Dubuque, Iowa. You should

Facts of loss: The insured discovered a small pothole starting to develop in the street in front of this home the pothole was
approximately 5 inches in diameter. The city of Dubuque street department - city of Dubuque came out and took a video of the
insured sewer line and discovered that the sewer line had two small breaks one under the street in front of the insured dwelling
and another in the yard of the insured's dwelling. The photo of the hole appears to be outside the insureds property line. The hole
in street is clearly visible, the cityput up warning signs. The damage extends beyond the insured's property line
