Dear lowa Cancer Consortium Board of Directors,
On behalf of the City of Dubuque I am pleased to offer this letter in strong partnership
with Crescent
Community Health Center (CCHC) on their proposed project, which will leverage the
Azara Healthcare Population Health Platform to identify screening gaps, address
disparities, and improve screening completion and follow-up across five counties in lowa
and beyond. This project aligns with the FY27 lowa Cancer Plan Implementation Grant's
priorities related to early detection, health equity, and systems-level collaboration.
CCHC's approach is compelling because it connects actionable data to coordinated
community action. By using population-level insights (e.g., geography, age, and
screening status), partners can focus outreach and resources on people who are
overdue for recommended screenings and on communities experiencing
disproportionate barriers to care — helping ensure that prevention and early detection
efforts extend beyond the clinical setting.
Improved data workflows supported by Azara will promote timely screening completion,
appropriate diagnostic follow-up, accurate tracking of outcomes, and strengthened care
coordination between the Cancer Center's patient navigation program and each
patient's primary medical home, including during treatment when needed.
The City of Dubuque will fulfill the following roles in this partnership:

• Encouraging residents to get recommended screenings through social media
outlets and newsletters
The City of Dubuque has a long-standing partnership with Crescent Community Health
Center to provide health care access and wellness services to our residents. The City
does this through contracted funding, Community Development Block Grant allocation
of funds for special projects and board of directors' participation.
The partnership with CCHC — paired with the integration of Azara into ongoing clinical,
public health, and community-based workflows — create a sustainable, scalable
infrastructure to improve screening rates, reduce disparities, and advance lowa Cancer
Plan goals beyond the grant period. We appreciate the opportunity to support CCHC's
application and are committed to partnering to ensure data-driven insights lead to
action, action leads to access, and access leads to measurable cancer outcomes.
