This is a claim in which claimant alleges that the basement of its building sustained
water damage due to substantial leaking around a rusted casing on the water main
located on the 8t" Street side of the Telegraph Herald building.

This claim has been referred to Public Entity Risk Services of Iowa, the agent for the
Iowa Communities Assurance Pool.

This written report constitutes your claim against the City of Dubuque, Iowa. You
should complete this form in full and attach any additional information that
supports your claim.
The claim must be filed with the City Clerk at City Hall, 50 West 13th St.,
Dubuque, IA 52001. It will then be referred to the appropriate department for
investigation and to the City Attorney's Office. Once that investigation is
completed, a report and recommendation will be submitted to the City Council.
You will be provided with a copy of that report and recommendation.

To all concerned:
10/20/2014-Substantial water was leaking around a rusted casing on the water main coming into the building
on the 8th street side of the Telegraph Herald basement and causing damage to the stored rolled paper on
located in that room.
10/20/2014-MMC was called in to assess the leakage and determine the cause, which was thought to be in the
sleeve between the interior and exterior wall due to the level of the corrosion that exists there.

Since this isn't the first patch put onto that line in recent history it was recommended by MMC that we dig
down to investigate the leak before the leak worsened and caused extensive damage to our paper in storage.
10/21/2014-McDermott Excavating was then called in,to remove the section of sidewalk above the pipe and
also to expose the pipe for another repair. Note: Paul Newhouse, and Chad Carlson were on site from the city
water crew to aide in stopping the leak and Brant Schueller was on site for 15 minutes to assess the problem.
Paul stated at the time that it looked to be leaking from the sleeve around the pipe as well where it penetrated
the wall into the basement. After digging down to pipe level and pumping a tremendous amount of water out j
of the hole to expose pipe, it was monitored for leakage where hours later it was found to be dry.
The water main leaks (3) were then discovered up the street to the extent of filling up a 12" waste line with
drainage signifying that the water flowing into our building was a result of a different main break, or multiple
ones.

Please note: Due to the vast amount of water main problems in the past year on 8th street and the age of the h
piping coming into our building, we were being proactive in the digging and repair as recommended by the
contractor. All signs indicated (to even the City Water Department Crew)that the leak was coming from the 9
main leaking between the pipe and the sleeve coming through the wall into our basement (indicated by the
corrosion between the two pieces and the steady water coming in.) We are not asking for refunds due to
damages to our paper in storage,just reimbursement for bill from McDermott Excavating.
