Van Milligen, City Manager SUBJECT: Environmental Review Determination for Continuum of Care Funded Programs — Opening Doors—Support Services DATE: June 5, 2015 Certain HUD funded activities which do not have physical impact require a Responsible Entity to determine whether the proposed activity is Categorically Excluded from the National Environmental Policy Act of 1969, as amended, and not subject to the related statutory authorities listed at 24 CFR Parts 58.5 and 58.6. The City is the Responsible Entity for those activities in the jurisdiction of Dubuque that have been awarded Continuum of Care funding. Housing & Community Development Director Alvin Nash recommends City Council authorization for the Mayor to sign the Environmental Review Determination for Opening Doors-Support Services certifying the activity is Categorically Excluded and not subject to 58.5 Statutory Authorities. The Certifying Officer's signature is requested on the attached Environmental Review Determination. The RE may use this form to make this determination. The RE is the unit of local government in whose jurisdiction the activity is located or the state. This form and a description of the activity must be retained in the RE's Environmental Review Record (see 24 CFR Part 58.38). A copy of this completed form must accompany the grantee's technical submission documentation that is forwarded to the Omaha HUD Field Office. HUD's environmental regulation (24 CFR Part 58) should also be consulted as necessary. This form is not appropriate for projects involving acquisition, rehabilitation, leasing (except tenant -based rental assistance), conversion, repair, or construction activities.
