Whereas, for nearly 50 years, community health centers have provided high quality and accessible
primary and preventative care to all, regardless of insurance status or ability to pay; and
Whereas, Health Centers are located in medically underserved areas and locally controlled by patient-
majority boards, making each Health Center responsive to the needs of the specific
community it serves; and
Whereas, Crescent Community Health Center serves as a critical economic engine contributing to our
local economy by generating economic impact and creating jobs through its 52 employees;
and
Whereas, the Health Center model continues to prove an effective means of overcoming barriers to
access including geography, income, and insurance status, and in doing so, improves health
care outcomes and reduces health care system costs; and
Whereas, Crescent Community Health Center saves the local health system valuable health care
dollars by managing chronic conditions and keeping patients out of costlier health care
settings; serving 6,000 patients with over 16,000 patient visits. As the demand for Health
Centers continues to grow, Crescent Community Health Center will be essential to meet the
needs of new patients; and
Whereas, National Health Center week offers the opportunity to recognize Crescent Community Health
Center, their dedicated staff, board members, and all those responsible for the continued
success and growth of the organization since its creation nine years ago.
NOW, THEREFORE, I, ROY D. BUOL, MAYOR OF THE CITY OF DUBUQUE, ON BEHALF OF THE
CITY COUNCIL, STAFF AND CITIZENS OF DUBUQUE, DO HEREBY PROCLAIM AUGUST 9
THROUGH 15, 2015 AS
"HEALTH CENTER WEEK"
IN THE CITY OF DUBUQUE, IOWA, AND ENCOURAGE COMMUNITY LEADERS TO VISIT
CRESCENT COMMUNITY HEALTH CENTER OR THEIR LOCAL HEALTH CENTER AND CELEBRATE
THE IMPORTANT PARTNERSHIP BETWEEN AMERICA'S HEALTH CENTERS AND THE
COMMUNITIES THEY SERVE.
IN WITNESS THEREOF, I have hereunto set my hand
and caused the Great Seal of the City of Dubuque to be
affixed this 3rd day of August, 2015.
