CLAIM AGAINST THE CITY OF DUBUQUE, IOWA
This written report constitutes your claim against the City of Dubuque, Iowa. You
should complete this form in full and attach any additional information that
supports your claim.
The claim must be filed with the City Clerk at City Hall, 50 West 13th St.,
Dubuque, IA 52001. It will then be referred to the appropriate department for
investigation and to the City Attorney's Office. Once that investigation is
completed, a report and recommendation will be submitted to the City Council.
You will be provided with a copy of that report and recommendation.
The final decision on all claims is made by the City Council. No employee of the
City of Dubuque has the authority to make any representation to you as to

Dear Don:
We purchased a property at 2010 Green Street in 2007. At the time we purchased it the
previous owners had a backflow stopvalve already in the property. They never had a problem
With sewage and water backing up into the basement. (they had the property built). The
basement at the time of purchase showed no signs of ever having water/sewage backup.
In 2007 we had sewer back up twice in our basement. We contacted you and you felt there
was a problem with the old backflow stopvalve malfunctioning. You explained the sewer
backflow stopvalve rebate program. We hired Total Plumbing, (Don Jacquinot) who installed
the new valve on September 17, 2007.
On April 25th 2008, Our Tenant Renee Manders, discovered raw sewage covering the floor
and seeping up into her upholstered furniture. We called the City and Bill Kelly stopped by for a
second time with this problem. He did jet the line. We believe the city storm sewer is backing
up into the sanitary sewer and backing into our basement.
We feel the city should pay this claim as we have done everything we were instructed to do.
We have enclosed a letter from our plumber who inspected the situation after the problem
occurred, and enclosed pictures. Our tenant also is included in this claim.
Please help us Solve this problem before it happens again.

2010 Green St: Sewage damage and cleanup
Rec room, bathroom, bedroom, laundry room
Wet vac and squeegee sewage and water from ceramic floor, 1200 square feet.
Scrub and disinfect and reseal grout and the throughout. Remove all baseboard and Place
fans throughout to dry wood walls. Brought 2x4's in for tenant and put all furniture up,
so tenant can work on saving any items which are salvageable. Replace baseboard.
Materials New Baseboard removed and installed $240.
Bleach, rags, brushes, squeegee etc. $ 58.
2x4's Cut and placed under all furniture $ 52.
Labor $450.
Total $800.

REQUEST FOR REIMBURSEMENT-TENANT
Tenant Renee Manders, Worked with our employees scrubbing the floor four times using
her own Lysol, bleach and floor scrubber. She lost 9 hours of work @ $14.00 per hour.
She used her own upholstery cleaner to clean her furniture. She currently only lets her
kids into this living area with flip flops on. The smell was horrible, she washed every
article of clothing and rugs in her own machine using her own water. Some items where
Not salvageable.

and volunteers and the fire
department had assisted in
pumping out the water.
Osterloh said the damage
thankfully didn't rival the 3 to 4
feet in the 2004 flood. Many of
the residents were bought out
after floodwaters washed over
the towns.
"It didn't do that this time,
thank God," he said.
Dubuque received .close to
2 inches of rain in the last few
days and remained at a mod-
erate flood category Saturday,
according to David Sheets, a
meteorologist with the Quad
Cities office of the National
Weather Service.
Dubuque had 0.88. inches
of rain on Thursday and I.02
inches on Friday, Sheets said.
Water levels remained high
on most rivers and tributaries
in the area. Sheets said mod-
erate flooding was spanning the
Mississippi River along most of
the length of Iowa and Illinois.
"It will crest over the next few
days," he said.
Sheriff's department officials
in Jo Davies County in Illinois
and Jackson and Delaware
Counties in Iowa reported that
there were no problems with
clean-up or additional flooding
