Purch of Serv Helping Serv NEMEMORANDUM May 2, 2002 TO: FROM: SUBJECT: The Honorable Mayor and City Council Members Michael C. Van Milligen, City Manager Purchase of Services Agreement - Helping Services of Northeast Iowa, Inc. During the Fiscal Year 2003 budget process, the Mayor and City Council provided $1,964 for the Purchase of Services Agreement with Helping Services of Northeast Iowa, Inc. Administrative Services Manager Pauline Joyce is recommending execution of the attached contract with Helping Services of Northeast Iowa, Inc. to help provide substance abuse prevention services to the residents of Dubuque. These services include consultations, training, presentations and early intervention programming. I concur with the recommendation and respectfully request Mayor and City Council approval. Michael C. Van Milligen, City Manager Pauline Joyce, Administrative Services Manager Purchase of Services Agreement - Helping Services of Northeast Iowa, Inc. Attached for City Council approval is the Purchase of Services Agreement with Helping Services of Northeast Iowa, Inc. for Fiscal Year 2003. The adopted Fiscal Year 2003 operating budget reflects a payment of $1,964 to Helping Services for Northeast Iowa, Inc., for financial assistance to cover operating expenses of that agency. The agency has received financial assistance from the City for the past eight years. Helping Services of Northeast Iowa, Inc., utilizes these funds for their Education Services Division, which provides substance abuse prevention services to the residents of Dubuque. These activities include consultations, training, presentations, and early intervention programming. It is my recommendation that the City Council approve the attached Purchase of Services Agreement with Helping Services of Northeast Iowa, Inc., and authorize the Mayor to execute the contract on behalf of the City. PJ/ksf Attachment THE CITY OF DUBUQUE, IOWA, AND HELPING SERVICES FOR NORTHEAST IOWA PURCHASE OF SERVICES AGREEMENT THIS AGREEMENT, entered into this 1st day of July 2002, by and between the City of Dubuque, Iowa, herein referred to as the "City" and Helping Services for Northeast Iowa, Inc., herein referred to as the "Agency." WHEREAS, the Agency represents itself as technically and professionally qualified to provide training and consultation in the area of substance abuse prevention to community task forces and schools; and WHEREAS, the City has deemed it to be in the public interest to provide financial assistance to be used to cover general operating expenses of the Agency's program. NOW THEREFORE, the parties hereto agree as follows: 1. Scope of Service The Agency shall provide, within the City of Dubuque, educational services related to a substance abuse prevention program for the purpose of teaching persons to make sensible decisions for themselves concerning the role that substances play in their lives. 2. Time of Performance The services of the Agency are to commence on July 1, 2002, and terminate June 30, 2003. 3. Compensation The City shall pay the Agency $1,964.00. This amount shall constitute full and complete compensation for the City's share of all services to be rendered. Such sum shall be payable by the City to the Agency within ten (10) days of the signing of this contract, upon receipt of a requisition for such sum. 4. Discrimination In carrying out the program, the Agency shall not discriminate against any employee or applicant for employment because of race, creed, color, sex, age, national odgin, religion, or disability. The Agency shall take affirmative action to ensure that applicants for employment are employed and that employees are treated during employment without regard to race, creed, color, sex, age, national origin, religion, or disability. Such action shall include, but not be limited to, the following: employment, upgrading, demotion or transfer; recruitment or recruitment advertising; layoff or termination; rates of pay or other forms of compensation; and selection for training, including apprenticeship. The Agency assures that all qualified applicants will receive consideration for employment without regard to race, creed, color, sex, age, national origin, religion, or disability. The Agency agrees and will undertake whatever affirmative measures are necessary so that no person shall be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activities funded in whole or in part with any of the funds made available to the Agency under the terms of this Agreement because of race, color, creed, religion, sex, national origin, age, degree of handicap or disability.
