Water Department Manager Denise Ihrig recommends City Council adoption of the
revised City of Dubuque Water Department Standard Operating Procedures for Sanitary
and Unsanitary (inc. Water Main Breaks) Conditions.
The Iowa Administrative Code (the Code) 567-42.1(2) identifies the notification and
procedure requirements for publicly owned and operated water supplies. The purpose
of the notification procedure is to protect public health.
In August 2016, the Iowa DNR issued revised guidance available to community water
supplies titled the Iowa Main Break and Depressurization Guidance. The guidance
document "is intended to provide guidance to public water supply owners and operators
in evaluating and responding to pressure loss situations from distribution system issues
such as main breaks, valve repairs, or extreme fire flows, and operational disruptions
such pump failure, power outage, telemetry [SCADA] failure, source failure, or storage
depletion". Anytime, the water supply is subject to disruption, the protection of public
health and safety is the priority and the City of Dubuque is working to ensure these
disruptions are minimized and follow the necessary critical steps throughout the process
to restore service to its customers as quickly and safely as possible.
The development of the City's Standard Operating Procedures for sanitary and
unsanitary conditions includes specific details including need to know information; a
communication plan; a list of situations that would lead to issuance of a boil water
advisory; contact lists; a step by step process on how to handle sanitary and unsanitary
situations including main breaks; and reference materials used to assist Water
Department staff in responding to main breaks or other situations where the public
water supply has been interrupted.

PURPOSE
The purpose of this memorandum is to request the formal adoption by City Council of the
revised City of Dubuque Water Department Standard Operating Procedures for Sanitary
and Unsanitary (inc.Water Main Break) Conditions.
BACKGROUND
The Iowa Administrative Code (the Code) 567-42.1(2) identifies the notification and
procedure requirements for publicly owned and operated water supplies. The purpose of the
notification procedure is to protect public health in the event the public water supply does
not comply with: maximum contaminant levels, maximum residual disinfection level,
treatment technique, monitoring requirements, and testing procedures meeting the federal
Safe Drinking Water Act.
The Code requires that "Each owner or operator of a public water system must give notice
for all violations of public drinking water rules and for other situations". The Code is further
described and relayed to communities by the Iowa Department of Natural Resources (Iowa
DNR), which identifies other situations, as a treatment or distribution system failure that
"significantly increases the potential for drinking water contamination" and "with significant
potential to have serious adverse effects on human health as a result of short-term
exposure".
The Iowa DNR assists local water supplies with the issuance of guidance on how to
correctly manage instances where a community water supply may not be able to meet the
Code, such as in the instances of a main break or pressure loss in the distribution system.
In August 2016, the Iowa DNR issued revised guidance available to community water
supplies titled the Iowa Main Break and Depressurization Guidance. The guidance
document "is intended to provide guidance to public water supply owners and operators in
evaluating and responding to pressure loss situations from distribution system issues such
as main breaks, valve repairs, or extreme fire flows, and operational disruptions such pump
failure, power outage, telemetry [SCADA] failure, source failure, or storage depletion".
Anytime, the water supply is subject to disruption, the protection of public health and safety
is the priority and the City of Dubuque is working to ensure these disruptions are minimized
and follow the necessary critical steps throughout the process to restore service to its
customers as quickly and safely as possible.
DISCUSSION
City experiences a Significant Main Break
The City experienced a significant disruption to its water service on May 3rd, 2016. A 20-
inch water main break occurred at the intersection of Johnson Street and 22nd Street. The
event resulted in loss of pressure in the distribution system and depletion of storage tank.
Working with the Iowa DNR, the City issued a boil advisory in the impacted pressure zone.
The City was able to localize the main break reducing the area impacted and was able to
restore service to the localized area within 16 hours.
During the main break, City departments, including the City Manager's office, Public Works,
Engineering, Health Services and the Public Information office assembled to determine the
extent of the required public notification. With assistance from the City's Public Information
Officer, residents and businesses were notified via a news release, the City's Facebook
page, the City's Twitter feed, and other news outlets including the Telegraph Herald, Radio
Dubuque, KCRG and KWWL. A CodeRed notification was also issued in the boil advisory
area which reached more than 8,000 registered users.
Development of a City Standard Operating Procedure
Following the event, the Water Department, Fire Department, Emergency Management,
Health Services Department, Engineering and the City Manager convened to analyze the
May 2016 water main break event and review other situations that may lead to an
unsanitary condition.
From the discussion, it was determined, a standard operating procedure be developed from
the Iowa DNR's Iowa Main Break and Depressurization Guidance Document (August 2016)
(Guidance Document) to assist the City in responding to routine and significant main
breaks. The standard operating procedure, would centralize all information necessary to
respond to a main break or other situations that may lead to an unsanitary condition. The
DNR's field office representative Amber Sauser was consulted during development of the
procedure and provided additional guidance on how to incorporate the Iowa DNR's
Guidance Document.
The Iowa DNR's Guidance Document identifies the AWWA Standard to be followed; boil
water advisory situations; minimum bacteria sampling requirements; when to contact the
DNR field office; and information to be included in a communication plan. In addition, it
summarizes how to respond to an escalating situation from a sanitary water main repair to
an unsanitary condition leading to a boil water advisory. The summary is an appendix to the
Guidance Document and is titled Appendix 3: Main Break Types and Responses. A copy of
Appendix 3 is attached.
The development of the City's Standard Operating Procedures for sanitary and unsanitary
conditions includes specific details including need to know information; a communication
plan; a list of situations that would lead to issuance of a boil water advisory; contact lists; a
step by step process on how to handle sanitary and unsanitary situations including main

breaks; and reference materials used to assist Water Department staff in responding to
main breaks or other situations where the public water supply has been interrupted. The
document was made possible with assistance from key City departments including the City
Manager's office, Utility Billing, Public Information, Information Service, Health Services,
Fire Department, Emergency Management, and City Engineering. Each department
lending expertise in communication and emergency response. The Communication Plan
details how residents, businesses, regulatory bodies, and City departments are to be
notified including what method of notification.
The City has several notification tools available. The use of each notification tool is further
identified based upon the area affected and if there is an escalating situation. For example,
door hangers are a popular method of notifying residents and businesses of a water supply
disruption. The City has created different door hangers for each situation.
Using the Guidance Document's Appendix 3, a Controlled Pipe Repair with
depressurization after Shutdown without Opening the Pipe would prompt the City to advise
customers to flush their plumbing. The appropriate door hanger the City would issue for
residents or businesses located in the shutdown area would be an Out of Service: Notice to
Flush Service Line. Whereas, in an area where there is an Uncontrolled Pipe Break with
Loss of Sanitary Conditions during Repair would require a Boil Water Advisory. Again, the
City has a door hanger with clear direction on customers should proceed with boiling water
until further. Copies of each type of door hanger may be found in the attachments located in
the Standard Operating Procedure for Sanitary and Unsanitary (inc. Water Main Break)
Conditions document.
Other notification tools available to the City include Facebook, Twitter, News Releases, the
Water Department Website, Code Red, and in Barrington Lakes, Cedar Crest, and
Wildwood Estates (Vernon Water system) mobile, free standing signage.
A copy of the Water Department's proposed Standard Operating Procedure for Sanitary and
Unsanitary (Inc. Water Main Break) Conditions is attached.
REQUESTED ACTION
I respectfully request the formal adoption by City Council of the revised City of Dubuque

BOIL WATER ADVISORY SITUATIONS
Unsanitary Condition - Level IV
Situations that may require a boil water advisory and bacterial sampling include:
• Water main breaks with controlled pipe repair with depressurization after shutdown with
opening the pipe or uncontrolled pipe break with a likelihood of water contamination or
loss of sanitary conditions during repair.
• No or low pressure in the distribution system and not contained (time or geographical
area).
• Situations where the source(s) or treatment facilities are interrupted or unusable,
including power loss situations.
• Situations where a tower or storage facility has been emptied.
• Pressure losses located near a contaminant source, such as near a river crossing,
feedlot, co-located with a sewer line break, etc.
• Pressure losses located in an area with significant elevation differences.
• Confirmed backflow or backsiphonage situation backflow or backsiphonage with
bacterial contamination. If the backflow or backsiphonage situation is chemical or
unknown, issue a "No Use" advisory instead.
• Pressure losses located in an area with the potential to create a high hazard backflow
incident.
• Pressure losses where a high hazard business is located in affected area (boiler, funeral
home, industry, etc.)
• A situation where the minimum disinfectant residuals cannot be maintained in or around
the impacted area.
• Situations including high risk or vulnerable populations, such as a school, day care,
hospital, nursing home, dialysis center, other medical facilities.
• An inexperienced operator or individual dealing with the situation.

• Water Plant Operations to contact Distribution first on-call crew member of possible water
main break followed by issuance of a WebQA notice.
• Distribution crew to locate the leak or repair area. Before any work commences, the
affected area must be determined and isolated to the fewest number of customers and the
least number of water mains by locating and operating the appropriate main valves while
maintaining positive pressure in the system.
• In the event of a need for excavation, Distribution crew must notify Iowa One Call for utility
locations.
• Once utility locations are clearly marked, carefully dig trench around break/repair area while
dewatering the hole. Dig to a depth of 12" minimum below and around the perimeter of the
pipe repair area. Water main must remain clean and free of trench water and debris until
repair has been made.
• Confirm that situation is not an "Unsanitary Condition". If necessary, crew lead or supervisor
shall determine the need to elevate the main break status to an Unsanitary Condition,
LEVEL IV in which case refer to procedure for "Unsanitary Conditions".
• Distribution crew lead or delegate to report to Operations at the Water Plant confirming main
break status and possible shut down area based on probable valve closures.
• Make repairs using standard practices
• Return line to service.

• Main Flushing: All water mains within the area of the water main break/pressure loss
should be thoroughly flushed following restoration of repair. Flushing will allow for a
satisfactory chlorine residual throughout the affected area and should remove any
contamination that may have infiltrated the water main. The recommended flushing velocity
is at least 3.0 feet per second. Flush all dead-ends/branches in the repair area.
• Sample water downstream and possibly upstream from impacted area making sure that at
least a 0.3 mg/L free chlorine residual is maintained.
• If Main break was classified as a Level III
• Sampling: WRRC certified lab staff member, Water Distribution staff member or Water
Treatment staff member to collect (1) coliform bacteria sample from the affected area.
The sample shall be taken after checking chlorine residuals. If repair is completed after
hours then sample can be collected during the early morning hours of the next day. The
sample collected must be stored in a cooler and taken to the WRRC Lab within 12 hours
of collection and maintained at 10 C° and falling.
• If Positive Bacteria sample: Follow Iowa Main Break and Depressurization Guidance DNR
Form 542-0535, dated August 2016 or Latest Edition. (Re-test sample sites) Refer to the
"Unsanitary Procedure" on next page.
a. Level III: Contact Manchester DNR and City's Health Department. Indicate that
the original site will be re-tested. Re-test at the Positive-Bacteria site. One
sample only. If re-test is positive, contact Manchester DNR.

UNSANITARY PROCEDURE
Unsanitary Conditions

1. If onsite conditions have been determined to be Unsanitary or condition has elevated up
from a "Sanitary Condition" to an "Unsanitary Condition" LEVEL IV.
2. In the event of a Loss of Sanitary Conditions, refer to the Additional Contact Group via
Water Department Main Break Code-Red. The Department Manager or Secondary shall
contact the City Manager to determine if initiation of EOC activation is required following
latest edition of the Administrative Policy#: Partial or Limited Level of Activation. EOC team
members are listed in the Additional Contact Group listing.
3. Command post staff (if required) shall report to EOC.
4. EOC Initial duties:
A. Issue WebQA: Main Break/Boil WaterAdvisory notice including probable shut
down area map.
B. Assist with Media Notices and identifying at risk users i.e. schools, dialysis centers,
hospitals, etc. See Health Services for full list.
C. Assist with contacting outside contractors as necessary and locating parts, materials
and supplies as requested by Command Post staff.
D. If additional traffic control is required, contact Engineering staff member listed under
additional contact group.
5. Notify affected area with red door hanger (Water Department Boil Water Advisory). If
necessary also via method door knocking, reverse phone, news release, City social media
channels or Code-Red phone notification assisted by Public Information Officer.
6. Distribution crew to locate shut off valves and isolate the break as soon as feasible.
7. Follow"Procedures" in DNR Form "Appendix 3 modified, LEVEL IV (Unsanitary).

flushed following restoration of repair. Flushing will allow for a satisfactory chlorine residual
throughout the affected area and should remove any contamination that may have infiltrated
the water main. The recommended flushing velocity is at least 3.0 feet per second. Flush
all dead-ends/branches in the repair area.
11. Sample water downstream and possibly upstream from impacted area making sure that at
least a 0.3 mg/L free chlorine residual is maintained.
12. Sampling: WRRC certified lab staff member, Water Distribution staff member or Water
Treatment staff member to collect (2) sets of consecutive coliform bacteria samples from the
affected area. The first set shall be taken immediately after checking chlorine residuals.
The second set of samples must be taken no sooner than 16 hours following the first sample
set; and no more than 24 hours. The samples collected must be stored in a cooler and
taken to the WRRC Lab within 12 hours of collection and maintained at 10 C° and falling.
13. Boil Water Advisory Lifted: Boil Water Advisory shall be lifted following (2) sets of
consecutive "absent' coliform bacteria sample results.
14. If Positive Bacteria sample: Follow Iowa Main Break and Depressurization Guidance DNR
Form 542-0535, dated August 2016 or Latest Edition. (Re-testsample sites)
a. Contact Manchester DNR and City's Health Department. Indicate that the original
site will be re-tested. Re-test at the Positive-Bacteria site. If re-test is positive,
contact Manchester DNR.
