Van Milligen, City Manager FROM: Alvin Nash, Director Housing & Community Development SUBJECT: Bee Branch Healthy Homes (BBHH), Community Development Block Grant- National Disaster Recovery (CDBG-NDR) Housing Projects Bid Set#1: DATE: July 31, 2017 INTRODUCTION The attached resolutions authorize the initiation of the public bidding procedure for the following housing rehabilitation projects: CIP# 2642690, PROJECT# 1X0016- 2499 Pinard St 1X0017- 2665 White St 1X0015- 671 Roberts St 1X0025- 2613 Stafford St 1X0024- 2108 Washington St 1X0012- 575 Kaufmann St BACKGROUND In January 2016, the State was awarded $95.7 million in HUD National Disaster Resiliency Competition grant funds. Per the award, the City of Dubuque is to receive $8.9 million for private home improvements associated with reducing water intrusion in to private properties. Through June of 2021 the Bee Branch Healthy Homes (BBHH) program will complete 320 housing units. In October of 2016, the City Council passed Resolution 363-16 authorizing the execution of the funding contract with the Iowa Economic Development Authority (IEDA) for the City's receipt of $8,900,165.00 in Community Development Block Grant Program Disaster Recovery - National Disaster Resiliency (CDBG-NDR) funds for the BBHH program housing contract. Each property has been approved by the BBHH committee approval team due to location in the Bee Branch watershed and the occupant being income eligible for programming. All projects have gone through an inspection, environmental assessment, State Historic Preservation Office (SHPO) and duplication of benefit review by IEDA prior to approval to go out to bid. DISCUSSION Each property will be bid individually and have a separate set of work specifications (attached) that will include work with intent to reduce future water intrusion into the structure as well as repair damage from past weather events. This work may include for example: excavation and grading, concrete flat work, mechanical repairs of furnace and water heaters, foundation repair, gutters and downspouts as well as a number of other eligible repairs. The BBHH inspector has prepared a cost estimate as part of the pre- bid package work write up to assist in budgeting with each project. PROJECT SCHEDULE The schedule for the afore mentioned projects has been developed in coordination with the owners of the property and East Central Intergovernmental Association (ECIA) that is sub-contracted to complete the technical service inspections for the program. The schedule for the Projects are as follows: Initiate Public Bidding Process August 7, 2017 Publish Notice to Bidders, Advertise for Bids August 9, 2017 Publish Public Hearing Notice on Plans & Specs August 9, 2017 Public Hearing on Plans & Specifications August 21 , 2017 Receipt of Bids (Bid Letting) August 28, 2017 Estimated Award Construction Contract September 5, 2017 Estimated Completion 60 days November 4, 2017 Subsequent properties will follow in 2017 and beyond. As noted, all the improvements for all properties must be complete by June 30, 2021 . RECOMMENDATION I recommend preliminary approval of the proposed plans, specifications, form of contract, and construction estimate for the following projects. I further recommend establishing the date of a public hearing and authorize ECIA to advertise for bids for the Project. SPEC# DESCRIPTION BID AMOUNT BASEMENT 01 Remove the concrete floor about 18 inches from the wall or as necessary to install the drain tile all the way around the basement. Remove all material and to a depth of 12 inches below the existing floor. Install a 3-inch perforated drain tile with a cap on open end and discharge into a 24-inch sump jar, that provides an airtight seal, installed at the right rear comer of the basement Install a floor drain with two-inch pipe to discharge into the sump. Fill the area around the sump jar and around the tile with clean gravel. Recement the concrete floor and finish smooth. Install a 3/4 HP sump pump with backflow valve and discharge to the exterior near grade level through the foundation wall. Discharge about 4 foot behind the house onto a splash block. Also, install a battery backup sump pump to discharge in the same location. Install with backflow valves. Install a 1.5-inch air admittance valve near the ceiling to vent the sealed sump jar. Provide a dedicated 15 AMP receptacle in accordance with NEC requirements. Use conduit drop and install to code and city requirements. A City permit and inspection may be required on some of these items. 02 Remove the carpet from the basement floor and dispose of material. 03 Remove the drywall base trim and framing from the exterior basement walls. 04 Install a dimpled drain board on the walls to grade. The base of the drain board must end within the stone and drain tile area to provide full drainage. Install along the rear wall to the top of the wall depth offset and secure to the wall. Caulk,neatly foam, or use other positive sealing method to seal the drain board to the top or seams for purposed of air sealing. 05 Reframe the exterior walls with ground contact treated base plate,then a second untreated plate,then 2 by 4 framing to the ceiling in all areas were wall were previously installed. Insulate to R-15. Install smart vapor barrier on the inside. Install mold resistant gypsum. Finish smooth. Paint with two coats of paint to cover with mold inhibitor added to the paint Note that the Gypsum must terminate two inches above the concrete floor. Base trim will not be installed. 06 Install a new basement exterior door at the interior location. Foam the jam and caulk the threshold. This will be a special-order door because of its reduced size. Install keyed lockset. Paint the jamb on all surfaces with exterior acrylic latex paint before installing the door. Paint all surrounding trims. The existing is about 30 by 72. PRESUMED LEAD HAZARD 07 Install a 95 %or better two stage natural gas forced air furnace. Install all to manufacturer requirements. Use a conduit drop secured to the ceiling with bushing on top of the conduit. Install disconnect switch or as required by manufacturer. Install on separate circuit Install a condensate pump(or drain to a floor drain), receptacle, and necessary ducting. Install anew programmable thermostat Clean the AC coil. Rebates, if available go to the owner with contractor to provide documentation. Install a capon the existing vent/chimney. A City permit and inspection is required. Add a cold air return and conditioned air duct in the basement. Install the furnace off of the floor as high as functionally possible. 08 Add an UV Purifier, Honeywell 24V UV Air Purifier with AirBRIGHT Odor Absorption system or approved equal. Install in accordance with manufacture installation guidelines. 09 Install continuous 5 inch gutters with leaf guards and large down pouts on the house. Discharge all to the rear of the house. Install four foot extensions with splash blocks. DUBUQUE BBHH HOUSING RESILIENCY — 2499 Pinard Street, Dubuque 10 Grout to fill the void,near the door area,within the exterior stairwell. Cover the entire stairwell(walls, steps, and floor)with water proofing paint to cover the area and existing paints. 11 GFCI protect all basement receptacles. 12 Remove the right front basement window and fill the void with cement block to fill. Paint the exterior with concrete seal paint. Color the paint to as close to the exterior wall as possible. This same paint maybe used within the exterior stairwell. PRESUMED LEAD HAZARD 13 Provide GFCI protection for all receptacles above the kitchen counter. Do not GFCI protect the refrigerator circuit. 14 Bedroom right rear. L1 *Post proper sign on the exterior doors. *Provide cleaning of all areas of the house before the request for lead testing is requested. This will include providing plastic protection when removing windows, completing wet wash - Hepa Vacuum - wet wash - Hepa vacuum cleaning at all areas within the house and vacuuming with an approved heap filter vacuumed. *This also includes providing exterior ground protection while exterior work is being completed. *Contractor is to cover all personal items within the structure while interior work is being completed. *If the contractor fails the dust test the contractor will be required to pay the testing firm $500.00 for each subsequent testing cycle until the house passes lead dust testing passed from a certified lab. *LEAD work must be completed in the shortest length of time as possible. Ideally the lead work should take no more than two working days with lead inspection on the third day at the latest. *The contractor has the right to temporarily change the house locks until lead testing is passed. *The contractor is completely responsible to comply with all State Lead Hazard Reduction requirements as required by the State of Iowa or as directed by IEDA protocol. Neither the City nor ECIA may be held liable for contractors said failure to follow Lead protocol. The contractor must have a state approved HEPA vacuum while lead work is being complete. DUBUQUE BBHH HOUSING RESILIENCY — 2499 Pinard Street, Dubuque TOTAL LEAD CLEANING BID Notice of Lead testing guidelines Lead work line items must be completed within a few days as possible. On or about three days after the lead work is started, or as requested by the contractor, ECIA will complete lead dust sampling on the project you are bidding. We will randomly select four window troughs, four window sills, four floor areas, and outside the front or back door. These randomly selected areas must test at or below the required micrograms per foot squared as determined by a certified independent laboratory. Currently ECIA uses EHS Laboratories in Maryland for testing. The contractor will not be billed for the first test. If the first lead test fails, the contractor will be billed $400.00 for retesting payable to ECIA by cashier's check before the next testis completed. The second and all subsequent testing's will result in a$400.00 fee pavable to ECIA by cashier's check before the next test is completed. Remember that the training you received in LEAD Safe Renovation is important and that the best way to prepare for the lead dust sampling is to HEPA VAC- wet wash- HEPA VAC. In addition, if siding was replaced we will sample the soil on the outside of the house. We will schedule this inspection at your request and you must be finished cleaning at least one hour before we can test the home for dust. It is the contractors responsibility to cover all personal items until the house is ready for lead dust testing. If a surface fails within the house, such as a window sill, all other like components within the house must be cleaned, tested, and found to be below Iowa limits or an additional test will be required. The owners will not be allowed to move into or back into the home until the lead dust tests come back below acceptable levels. In addition, final payment will not be made until lead testing is completed and certified at or below acceptable levels as directed by IEDA. I also understand that if lead hazards are noted within these specifications that the contractor submitting this bid will not complete any work within this house unless the owner has been temporarily relocated as required by IEDA regulations and in association with the City of Dubuque's BBHH grant This is a mandatory requirement and funds are available to the owner for temporary relocation expenses. The owner may not move back into the structure until the house is approved LEAD SAFE by an ECIA Rehabilitation Specialist. Any citations, fines, or none compliance issues are the responsibility of the firm completing lead work not the testing agency or qualified lab. By submission of his BID, each BIDDER certifies and, in the case of a joint BID, each party thereto certifies as to his own organization, that this BID has been arrived at independently, without consultation, communication, or agreement as to any matter related to this BID with any other BIDDER or with any competitor. BIDDER hereby agrees to commence work under this CONTRACT on or before a date to be specified in the Notice to Proceed and to fully complete the project with 60 consecutive calendar days thereafter. The Notice to Proceed will be issued within 10 days of the bid opening. One exception applies, notice to proceed, may be delayed if only 1 bid is received as additional approvals will be needed prior to contract signing. BIDDER acknowledges receipt of the following ADDENDUM: BID SCHEDULE NOTE: 1. Bids shall include all applicable taxes and fees. 2. BIDDERS shall not add any conditions or qualifying statements or modify this proposal or the proposal may be declared irregular as being not responsive to the Notice. 3. This bid does NOT have Lead Hazard Reduction. 4. Do not include sales tax in your line item bid amounts. The City will issue a sales tax exemption certificate for this project. DUBUQUE BBHH HOUSING RESILIENCY-2665 WHITE STREET 1 BIDDER agrees to perform all the WORK described in the CONTRACT DOCUMENTS for the following unit prices or indicated sums: Purchasing a building permit form the City of Dubuque as necessary for this project. It is the contractor's responsibility to schedule all inspections, as required,with the City of Dubuque Building Services Department. Payment will not be made without permit and inspection as required. Have the City Cc inspector sign off on your permit for proof of inspection. Basement specs Remove the concrete floor, about 15 inches wide, from base of the rear steps to the area near the existing sump pip and from that point to the front wall. Within this area dig out about 12 inches deep and install the following. Install a three-inch perforated drain tile. Install a floor drain near the basement entry with pipe directly to the sup jar and vent with an air admittance valve installed near the ceiling. Also, install a second floor drain near the old sump location. Install 18-inch-wide sump jar in the existing location that will be able to completely seal (remove the old concrete pit). Install a second vent to the sump jar with air admittance valve(install close to the ceiling). Fill the area around the sump jar, drill holes in the sump jar to allow water to enter the top half, and around the tile with clean gravel. Re-cement the concrete floor and finish smooth. Install a 1 HP sump pump with backflow valve and discharge to the exterior near grade level through the foundation wall. Discharge about 3 feet in front of the house onto a splash block. Also, install a battery backup sump pump to discharge in the same location. Install with backflow valves. Provide a dedicated 20 AND receptacle in accordance with NEC requirements (install as high as possible but still allow direct connection to the sumps. Use conduit drop and install to code and city requirements. A City permit and inspection may be required on some of these items. The floor drain near the rear entry door should be graded to capture any water that may enter for the width of the door. Install a new insulated door at the base of the exterior entry. Weather strip to seal. Also, replace all jambs. Caulk to seal trims and trims to foundation. Paint all painted trims inside and out with two coats of paint to cover. This will be a special order flush metal clad entry door. Replace the stairs to the basement as close to current code as possible. Use treated wood material. Dispose of existing material. Install handrail to meet code as best as possible. Install a new natural gas water heater that is R-8 or better and a minimum of 40 gallons. Vent to existing and install all to manufacturer requirements. Reuse the'/inch copper drop. Install breakers or fill the open spaces with fillers made for the electrical panel. Clean the basement walls and repaint previously painted areas with two coats of concrete sealing paint. Install a 95 % or better two stage natural gas forced air furnace. Install all to manufacturer requirements. Use a conduit drop secured to the ceiling with bushing on top of the conduit. Install disconnect switch or as required by manufacturer. Install on separate circuit. Install a drain to a floor drain, receptacle, and necessary ducting. Install a new programmable thermostat. Clean the AC coil. Rebates, if available go to the owner with contractor to provide documentation. Install a cap on the existing vent/chimney. A City permit and inspection is required. Add a cold air return and conditioned air duct in the basement. Install the furnace off of the floor as high as functionally possible. Replace the wood tread at the basement of the stairs. Exterior and other: Remove the deteriorated exterior stairwell entrance at the rear of the house. Pour an 8-inch-deep concrete footing/floor that is about one 20 inches wider and further out from the house. Pour an 8-inch concrete wall to about the existing angle of the existing wall. Paint the inside and outside with two coats of concrete sealing paint. Install a Bilco, or approved equal, metal door to cover. Water shall run away from this entry. The basement entry will need to be sized to work with an available Bilco or approved equal door system. Seal to DUBUQUE BBHH HOUSING RESILIENCY-2665 WHITE STREET 1 stone wall as best as possible. Grade the existing driveway soil from the sidewalk to the rear comer of the house and with sidewalk to the replaced basement entry. Install a 6-inch-deep driveway and reduce to four-inch-deep in sidewalk areas. Slope the drive so that water will not flow off of the side and toward the houses but direct water toward the street. Use number four rebar 24 inches on center both directions. Install about 4 inches of compacted clean gravel under the new driveway. Install about 6 inches away from the house and up to the neighboring property line on the left side house. Finish concrete with a light broom finish. Seal the concrete to finish. Regrade topsoil as necessary and seed. The rear concrete elevation shall be such that the rear yard is able to drain onto the driveway during heavy rain events. Install continuous gutters with large downspouts. Discharge so that the water will run away from the house. Install with gutter guards to reduce collection of leaf and other. All gutters must discharge to grade without interconnection. It is the contractors responsibility to schedule all inspections, as required, with the City of Dubuque Building Services Department. Payment will not be made without permit and inspection as required. Have the City code inspector sign off on your permit for proof of inspection. Remove the concrete floor, about 18 inches, or as necessary to install the drain tile, out from the wall, from the front wall by the exterior basement entry door to the area directly under the top of the basement stairs to the I" floor. Remove all material and to a depth of 12 inches below the existing floor. Install a 3-inch perforated drain tile with a cap on open end and discharge into an 18 inch or larger sump jar, that provides an airtight seal, installed at the lowest point. Install a floor drain with two-inch pipe to discharge into the sump. Fill the area around the sump jar and around the tile with clean gravel. Recement the floor and finish smooth. Install a 1/2 HP sump pump with backflow valve and discharge to the exterior near the front of the house or as necessary so that the water will not recirculate into the basement. Also, install a battery backup sump pump, minimum 30 GPM, to discharge in the same location. Install with backflow valves. Install a 1.5-inch air admittance valve near the ceiling to vent the sealed sump jar. Provide a dedicated 15 AMP receptacle in accordance with NEC requirements. Bury a 4 inch drain tile about 4 inches under the existing soil around the driveway and install a pop up water discharge about six feet from the sidewalk. Have the City Code Inspector sign off on your permit for proof of inspection. SPEC# DESCRIPTION BID AMOUNT BASEMENT 01 Replace the basement window with vinyl replacement windows. Caulk to seal window to trims and trims to foundation. Paint all painted trims inside and out with two coats of paint to cover. PRESUMED LEAD HAZARD. 02 +Duct the dryer vent to the outside. It is currently in the window area. Use solid aluminum pipe and seal. 03 Replace three wood posts with treated 6 by 6 material. Use Ground contact rated with cut end up. Secure with approved galvanized fasteners. 04 Replace the water heater with a short power vented natural gas water heater. Install as high as physically possible. Separate the water heater from concrete with approved separation. 40-gallon minimum R-8. Use a'/inch copper drop to six inches above the floor for the relief valve. L1 *Post proper sign on the exterior doors. *Provide cleaning of all areas of the house before the request for lead testing is requested. TOTAL LEAD CLEANING BID DUBUQUE BBHH HOUSING RESILIENCY - 2613 Stafford Street, Dubuque Notice of Lead testing guidelines Lead work line items must be completed within a few days as possible. On or about three days after the lead work is started, or as requested by the contractor, ECIA will complete lead dust sampling on the project you are bidding. We will randomly select four window troughs, four window sills, four floor areas, and outside the front or back door. These randomly selected areas must test at or below the required micrograms per foot squared as determined by a certified independent laboratory. Currently ECIA uses EHS Laboratories in Maryland for testing. The contractor will not be billed for the first test. If the first lead test fails, the contractor will be billed $400.00 for retesting payable to ECIA by cashier's check before the next testis completed. The second and all subsequent testing's will result in a$400.00 fee pavable to ECIA by cashier's check before the next test is completed. NO, pavment will not be made until the house is determined lead safe. Remember that the training you received in LEAD Safe Renovation is important and that the best way to prepare for the lead dust sampling is to HEPA VAC- wet wash- HEPA VAC. In addition, if siding was replaced we will sample the soil on the outside of the house. We will schedule this inspection at your request and you must be finished cleaning at least one hour before we can test the home for dust. It is the contractors responsibility to cover all personal items until the house is ready for lead dust testing. If a surface fails within the house, such as a window sill, all other like components within the house must be cleaned, tested, and found to be below Iowa limits or an additional test will be required. The owners will not be allowed to move into or back into the home until the lead dust tests come back below acceptable levels. In addition, final payment will not be made until lead testing is completed and certified at or below acceptable levels as directed by IEDA. SPEC# DESCRIPTION BID AMOUNT BASEMENT 01 Remove the concrete floor,about 18 inches,or as necessary to install the drain tile,out from the wall, from along the front wall to the rear of the house and along the rear wall. Remove all material and to a depth of 12 inches below the existing floor. Install a 3-inch perforated drain tile with a cap on open end and discharge into an 18 inch by 30-inch-deep or larger sump jar,that provides an airtight seal,installed near the center of the front wall. Drill half inch holes,about 2 inches apart, around the sump jar about 12 inches under the floor then fill the area around the sump jar and around the tile with clean gravel. Recement the floor and finish smooth. Install a half HP sump pump with backflow valve and discharge to the exterior near the front of the house or as necessary so that the water will not recirculate into the basement. Also, install a battery backup sump pump,minimum 30 GPM,to discharge in the same location. Install with backflow valves. Install a 1.5-inch air admittance valve near the ceiling to vent the sealed sump jar. Provide a dedicated 20 AMP receptacle in accordance with NEC requirements. Use conduit drop and install to code and city requirements. A City permit and inspection may be required on some of these items. 02 Replace several sections of 4-inch cast iron pipe in the basement. Replace from the vertical PVC riser with Fernco fittings to the corner or as needed within the building to replace the broken pipe. Install in similar configuration with clean out. 03 Have two steel brackets made to go cover the pipe so that the floor support is not resting on the pipe. use welded '/4 steel. Cut the post and secure to the steel bracket. 04 Block in the old stair well to about three feet above the concrete floor. Fill the block with grout and seal both sides of the block with sealing paint to cover. Pin to the wall. 05 Cover the old window opening,which now goes to the crawl,with %inch treated plywood to cover. foam and caulk shut. 06 Remove all loose paint from the exterior walls to a solid substrate. Spray all exterior walls and floor with LastiSeal product by Radon Seal to cover. Remove all surface spray foam. www.radonseal.com. Use a minimum or two coats at about 200 square feet per gallon,per coat of product. Save all the containers for inspection purposes. Everything must be vacuum clean before applying product. Use the PowerEtch product to help prep the surface for allocation. 07 Remove all the sidewalks but the not the City walk. Raise the walks so that they have positive drainage, about 1/4 inch per foot minimum away from the house. Reinstall in similar layout but at an elevation slightly under the existing siding elevations. Also,pour a 3' walk around the AC unit. The AC unit will need to be elevated to the new sidewalk height. Lower the soil in the front area so that water will flow away from the house. fill the side with new soil,compacted then loosen the top and seed and cover all. Seal the concrete and cut in equal squares. Use number 4 bar in 18 inch grids both ways. Add gravel under the new walk as needed. Pin the new walk to the walls. Install a steel window well for both front windows and cover with same size covers. 08 Install new continuous gutters on the main(upper)house with large down spouts. Drain the main house gutters to a drain tile with pop-up in front yard. Install before pouring the new sidewalks. Extend the rf down spouts to about four feet behind the house with folding down spouts. install life guards over all gutters. This will be done from the open block wall under the stairs,the bathroom wall,to the area behind the existing sump pump through the unfinished area. Drill access holes large enough to install a'/inch piece of PVC from the block voids to the existing drain tile and tum the end down with a 90 degree elbow. Take the time to make sure these will drain well into the drain tile system and do not plug the holes when reinstalling the concrete. If they are not open on the interior side of the block,we gain no value on this installation so be absolutely sure to open drainage from the block voids to the drain tile. Reinstall the concrete floor and finish smooth like existing. This is about 60 feet of wall. 02 Paint the wall areas above the floor repair with two coats of heavy duty wall sealing paint to cover. Caulk the floor to the wall before painting. Use a water proofing crack filler. 03 Provide and install a sealed lid for the sump pump that will seal this sump from the house air. Contractor shall farnish a signed certificate of insurance to the department responsible for the contract for the coverage required in Exhibit I prior to commencing work and at the end of the project if the term of work is longer than 60 days. Contractors presenting annual certificates shall present a certificate at the end of each project with the final billing. Each certificate shall be prepared on the most current ACORD form approved by the Iowa Department of Insurance or an equivalent approved by the Finance Director. The certificate must clearly indicate the project number,project name, or project description for which it is being provided Eg:Project # Project name: or Project Location at or construction of 2. All policies of insurance required hereunder shall be with an insurer authorized to do business in Iowa and all insurers shall have a rating of A or better in the current A.M. Best's Rating Guide. 3. Each Certificate required shall be furnished to the Housing Department of the City of Dubuque. 4. Failure to provide the coverages described in this Insurance Schedule shall not be deemed a waiver of these requirements by the City of Dubuque. Failure to obtain or maintain the required insurance shall be considered a material breach of this contract. 5. Contractor shall require all subcontractors and sub-subcontractors to obtain and maintain during the performance of work insurance for the coverages described in this Insurance Schedule and shall obtain certificates of insurance from all such subcontractors and sub-subcontractors. Contractor agrees that it shall be liable for the failure of a subcontractor and sub-subcontractor to obtain and maintain such coverage. The City may request a copy of such certificates from the Contractor. 6. All required endorsements to various policies shall be attached to the certificate of insurance. 7. Whenever an ISO form is referenced the current edition must be provided. 8. Contractor shall be required to carry the minimum coverage/limit, or greater if required by law or other legal agreement,in Exhibit I-Insurance Schedule F.If the contractor's limits of liability are higher than the required minimum limit then the contractor's limits shall be this agreement's required limits. Nonwaiver of Governmental Immunity. The insurer expressly agrees and states that the purchase of this policy and the including of the City of Dubuque, Iowa as an Additional Insured does not waive any of the defenses of governmental immunity available to the City of Dubuque,Iowa under Code of Iowa Section 670.4 as it is now exists and as it may be amended from time totime. 2. Claims Coverage. The insurer further agrees that this policy of insurance shall cover only those claims not subject to the defense of governmental immunity under the Code of Iowa Section 670.4 as it now exists and as it may be amended from time to time.
