MEMORANDUM OF UNDERSTANDING
MOU-2018-ELC23
BETWEEN THE
IOWA DEPARTMENT OF PUBLIC HEALTH
AND
City of Dubuque
This Agreement is made and entered into by and between City of Dubuque, hereinafter called the
CONTRACTOR and the Iowa Department of Public Health, hereinafter called DEPARTMENT.
PURPOSE: It is the mutual desire of the CONTRACTOR and the DEPARTMENT to provide
surveillance for mosquito-borne diseases in Iowa.
I. The term of this Agreement shall be June 1, 2017 to July 31, 2017, with an option to amend and
extend the contract if funding is available after July 31, 2017.
II. Contract Administrators
Ken Sharp, Division Director, Division of Acute Disease Prevention and Emergency Response
and Environmental Health is the Authorized State Official for this agreement. The Authorized
State Official must approve any changes in the terms, conditions, or amounts specified in this
contract. Negotiations concerning this contract should be referred to this person at telephone
(515) 281-5099.
Mary Rose Corrigan has been designated by the CONTRACTOR to act as the Contract
Administrator. This individual is responsible for administrative matters of this contract. Mary
Rose Corrigan is responsible for fiscal matters of this contract. Negotiations concerning this

III. The DEPARTMENT agrees:
A. To reimburse the CONTRACTOR for expenses incurred for collection of mosquitoes
from BG Sentinel and Gravid Aedes traps during the 2017 arboviral disease surveillance
season. Reimbursement shall not exceed the contract amount listed in Article V.
IV. The CONTRACTOR agrees to:
A. Collect mosquitoes for Aedes surveillance and send the mosquitoes to Iowa State
University for speciation. Mosquitoes shall be collected from at least:
Ltwo (2) BG Sentinel traps (collect mosquitoes from BG Sentinel traps a minimum
of three (3) times per week: Monday, Wednesday, Friday), and
2.ffve(5) Gravid Aedes traps (collect mosquitoes from Gravid Aedes traps once per
week: Monday)
V. The total budget amount is $1,000. The budget is as follows.

VI. It is mutually understood and agreed that:
A. Claim vouchers submitted for reimbursement shall:
1. be reported in budget line items as shown in Article V,
2. reflect actual expenses, and
3. be received no later than 30 days from the date of services.
B. This agreement can be amended by the mutual written consent of both parties only.
C. Federal funds made available under this contract shall be used to supplement and increase
the level of state, local and other non-federal funds that would in the absence of such
federal funds be made available for the programs and activities for which funds are
provided and will in no event take the place of state, local and other non-federal funds.
D. The disbursement of funds under this contract is contingent upon the continued
availability of federal, state, or private funds to the DEPARTMENT.
E. Any use of the DEPARTMENT'S name, logo, or other identifier must have prior written
approval from the DEPARTMENT.
F. The terms and provisions of this contract shall be construed in accordance with the laws
of the State of Iowa. Any and all litigation or actions commenced in connection with this
contract shall be brought in Des Moines, Iowa, in the Iowa District Court in and for Polk

County, Iowa. If, however,jurisdiction is not proper in the Polk County District Court,
the action shall only be brought in the United States District Court for the Southern
District of Iowa, Central Division, provided that jurisdiction is proper in that foru 1. This
provision shall not be construed as waiving any immunity to suit or liability that may be
available to the DEPARTMENT or the State of Iowa.
