Leisure Services Manager Gil Spence is recommending submission of a National
Recreation and Park Association ACHIEVE Healthy Communities Grant.
The National Recreation and Park Association, with support from the Centers for
Disease Control and Prevention (CDC), plans to award $30,000 each to ten public park
and recreation agencies to:
1) Build or strengthen a local coalition of community stakeholders/partners
2) Develop a community action plan to address chronic disease risk factors, such
as physical inactivity, poor nutrition, and tobacco use at the policy, systems and
environmental change levels.
The $30,000 is a one-time award, but the community is expected to remain committed
to the effort for three years. During this three-year period, we will continue to receive
technical assistance from NRPA and CDC.

INTRODUCTION
The purpose of this memorandum is to request City Council approval to submit an
application to become an ACHIEVE Healthy Communities Demonstration Site.
DISCUSSION
The National Recreation and Park Association, with support from the Centers for
Disease Control and Prevention (CDC), plans to award $30,000 each to ten public park
and recreation agencies to:
1) Build or strengthen a local coalition of community stakeholders/partners
2) Develop a community action plan to address chronic disease risk factors such as
physical inactivity, poor nutrition, and tobacco use at the policy, systems and
environmental change levels.
The $30,000 is a one-time award but the community is expected to remain committed to
the effort for three years. During this three-year period we will continue to receive
technical assistance from NRPA and CDC.
Organizations that will be invited to be a part of the coalition include: City of Dubuque
Health Services Department, the Finley Hospital, Mercy Medical Center, Crescent
Community Health Center, Hy-Vee Food Stores, Dubuque Community School District,
Holy Family Catholic School System, and the Dubuque Area Chamber of Commerce.
Budget for the $30,000 would break down as follows:
• Travel
• Physical Fitness Director position
• Expansion of the Live Healthy Dubuque program
• Assistance at the Crescent Community Health Center

Two leaders from the coalition will travel to Atlanta, Georgia for a Coaches' Training
meeting and up to ten representatives will need to travel to an Action Institute at a
location yet to be determined.
Program Supervisor Janna Beau from our Recreation Division will lead this effort. She
has been an active member of the collaborative effort of the Live Healthy Dubuque
program which has a mission to lead our community in adopting a healthier lifestyle by
teaching participants how to incorporate health into everyday life to create a culture of
wellness. This grant will help expand that effort.
Our department's goal is to continue to build partnerships and ultimately make health,
wellness, and prevention a personal and community priority.
ACTION STEP
The action requested is that the City Council approve an application to become an
ACHIEVE Healthy Community Demonstration Site.

13. Besides the statistics and information provided above, please describe any other
issues relevant to chronic disease, overweight and obesity, and their risk factors
(physical inactivity and poor nutrition) facing your community so we better
understand your community's health needs.
Even though the general economy in Dubuque County is positive, the
percentage of families living in poverty increased by 3.6% and the percent
of children living at poverty level increased 2.4 % from 200 - 2006. People
living at poverty level often have more difficulty planning ahead to ensure
food is available for their families and often end up buying more expensive
fast food, eating at meal sites such as the City Mission, or getting food
from the community food pantry. The Iowa Youth Survey for Dubuque
County indicates diets are often deficient in fruits and vegetables (8% in
youth in 6, 8 and 11th grades reported eating 5-a-day fruits and
vegetables compared to 18.9% youth in Iowa and 21.4% in the U. S. Data
Source: IDPH data 2005 from Iowans Fit for Life). In addition, Dubuque,
similar to the Western habits of the U. S., has embraced a diet high in
processed sugars with sugared soda pop and other drinks replacing
water. Cultural changes have resulted in many families rarely eating
meals at home as a family. The effect on overall nutrition is disastrous
and the resulting decline in family life is alarming.
The Department of Health and Human Services bulletin Advance Data
jNo. 395, May 20, 2008, edition) reported that "Less than one-third of
these older adults were meeting the HEI recommendations for meats,
dairy, fruits, vegetables, and grains.....Five out of six older adults in this
survey had diets that were rated as poor or needed improvement." Even
though this data is not specific for Iowa or Dubuque County, the trend of
poor nutrition in this age group is likely been seen in this community.

The Dubuque County 2005 Community Health Needs Assessment and
Health Improvement Plan addresses chronic disease as a priority.
Several of the objectives addressing chronic disease are related to
nutrition and physical activity.
14. Please describe experiences your agency has had in implementing policy, systems
and environmental change strategies to impact the health and wellbeing of your
community. What has been the impact of using policy, systems and environmental
change strategies to sustain healthy eating, active lifestyles and community
livability practices in the community?
Policy Changes
The State of Iowa recently passed a law prohibiting smoking in public
buildings and vehicles. Our agency has applied that new law to all our
buildings and vehicles. The City of Dubuque in its Code of Ordinances
requires developers to donate land in a new subdivision for park purposes,
has adopted a master plan for bike/hike trails throughout the city, adopted
a plan for water trails along the Mississippi River and Catfish Creek (for
paddle boats), and is establishing a wellness program for city employees.
This program will provide health screening for all 615 City employees with
recommendations on actions to be taken.
The City of Dubuque has aself-funded health care plan for its employees
and families. A Health Care Committee representing five unions,
management and general employees set policy on the operation of the
health plan benefits.
System and Environmental Changes
The Iowa Sports Foundation (non-profit) has a 100-day program called
Live Healthy lows. Our agency is part of a local coalition that promotes
Live Healthy Dubuque (this program is described in question #20). All our
city parks have been renovated with play equipment and facilities that
promote physical activity, including walking paths where appropriate. We
have constructed fifteen new parks in the last twelve years and have
seven more scheduled in the next several years. Many of these parks are
the result of the above mentioned policy. Whenever possible, a new park
will include a walking path (three of the new parks have this feature),
basketball court, and all include play equipment designed to improve the
fitness of children as they use the equipment. We partnered with the
local Rescue Mission to establish a community garden and are looking for
opportunities to expand the program and the recreation division has
offered gardening classes for children. We have a program to provide a
number of bike racks in the downtown area and to place bike racks on city
buses to encourage more bike riding.

The City of Dubuque was a major partner in the establishment of the
Crescent Community Health Center, and the Health Services Manager
serves on the board of directors. This Center has improved the health
care services (medical and dental) to low income citizens of our
community.
A budget request has been submitted to hire a physical fitness director
that would plan and direct physical fitness programs and to promote
physical activity community wide, educate the community on fitness and
nutrition, work with companies on how they can incorporate fitness into
their employees' work day, partner with the school district on more fitness
activities, and general promotion of health and fitness community wide.
15. If awarded this grant, what policy, systems and/or environmental changes would
your agency look to implement to reduce incidence of chronic disease within your
community?
The coalition that will be formed will advise the community on what
changes should be made. These recommendations will be reviewed by
the City Council and area businesses for action. Those recommendations
may include, but are not limited to:
• An advertising campaign to educate the community on health related
issues.
• Allowing employees to attend awareness classes on physical fitness,
nutrition, and chronic diseases during work hours.
• Review of the school lunch program and products available in vending
machines.
• Encourage employers to reward employees that engage in a physical
fitness program and healthy eating.
• Support the City of Dubuque's Hike/Bike Trail Master Plan and
expansion of the park system.
• Search for a location and sponsor for a community gardening program.
• Incorporate physical fitness and nutrition into every recreation program
offered.
• Work with fitness centers to establish discounted rates for a person
based on the amount they are overweight.
• Expansion of the Live Healthy Dubuque Coalition.

energy, and resources on improving healthy eating, active lifestyles and livable
communities through changes in policies, systems and the physical environment.

engage in this effort?

Our park division comprises 23 full time and 22 part-time employees, 47
parks with 898 acres of park land, 19 tennis courts, 3 outdoor ice rinks, in-
line hockey rink, two skate parks, 285 units of play equipment, 8 softball
fields, 1 baseball field and 25 miles of trails. The recreation division has 9
full time and 137 part-time employees that plan and direct a full range of
programs for all ages, operates two outdoor swimming pools, and an 18-
hole golf course.
Our agency has made a budget request to hire a physical fitness director
(see question #14) that will lead this effort. If that position is not funded
we will assign more of one of the program supervisor's time (we have 3) to
this effort and reassign some of their current duties to the other
supervisors to create time for this effort.
It seems that the community is aware of the obesity problem in our
country, state and city, and wants to know how to deal with it. The entire
country has a greater awareness of health issues so this seems to be a
great time to begin this effort. We would look to build on the success of
the Live Healthy Dubuque program, making it a year-round effort, and to
assist the Crescent Community Health Center with prevention programs.
Dubuque has recently hired a sustainability coordinator to lead the city's
effort to make Dubuque a more sustainable community. It seems natural
that these two efforts partner in our efforts.
The program supervisor that will be assigned this effort has a keen
interest in health and fitness issues and is a leader of the Life Healthy
Dubuque effort. Our City Manager is supportive of this effort and the City
Council has approved submission of this application.
17. What strengths or assets do you think your agency possesses that will help you
succeed in implementing policy, systems and environmental changes that will
reduce the incidence of chronic disease in your community? What barriers or
weaknesses do you believe are present?
Our agency has begun to work on the physical fitness and nutrition side of
this issue. The partnership we currently have with the Live Healthy
Dubuque coalition will serve as the take-off point for this effort. Many of
the needed relationships are in place and a general understanding of the
issues has been developed.
The City's partnership with the Crescent Community Health Center and
the City having a Health Services Department will aid the chronic disease
side of the issue. Our Health Services Manager has a good relationship
with state and county health agencies that will prove beneficial.

partnerships:
• Free Flight (a bicycle shop) to offer a bicycle mechanics class to teach
basic bicycle care and maintenance.
• Fever River Outfitters to offer kayak lessons and promote the sport of
kayaking.
• The Dubuque Water Ski Club to offer water ski lessons for youth.
• Boys' and Girls' Club of Greater Dubuque to offer swim lessons for
their membership.
• We work with the Mississippi Valley Running Association to offer
several runs and the youth/family triathlon.
• We make our park facilities available for many benefit walks.
• We partnered with the Dubuque Youth Hockey Association to construct
an in-line hockey rink.
• We partner with the Boys' and Girls' Club of Greater Dubuque and the
State of Iowa to provide a free eight-week hot, healthy lunch program
at three playground sites. We served 2,3051unches in 2008.
• A partnership with Iowa State University Extension -Dubuque County
provides nutrition and food classes during the summer.
20. Does your agency have currently or in the past been part of a coalition to promote
healthy lifestyles within your community? Please describe the coalition and
include a few examples of the work the coalition has undertaken.
The City of Dubuque has been an active part in the Live Healthy Dubuque,
formerly Lighten Up, Iowa, for six years. The ultimate vision of this
program is to improve the health productivity and qualities of life for our
community through organizational alliances in prevention, healthy
behaviors and health promotion.

Live Healthy Dubuque is a collaboration of the efforts supported by the
Finley Hospital, Hy-Vee Foods, John Deere Dubuque Works, Bike Shack/
Shoe Shack, Cumulus Broadcasting, Westside Fitness and Women's
Wellness Center, Dubuque Community Y, Dubuque Internal Medicine,
Lamar Advertising, Dubuque Community School District, McDonald's,
Mississippi Valley Running Association, Loras College, Clarke College,
University of Dubuque, the Dubuque Chamber, Mercy Medical Center,
A.Y. McDonald Manufacturing, Rousselot, Cottingham and Butler,
American Red Cross of the Tri-States, Wellmark BCBS, Dubuque Racing
Association and community members at large.
The mission of Live Healthy Dubuque is to lead our community in adopting
a healthier lifestyle by teaching participants how to incorporate health into
everyday life to create a culture of wellness. We have fulfilled our mission
by the numerous activities that we offer such as kick-off events,
community walks, exercise classes, health screening checks, featured
speakers and incentive offers throughout the program's 100-day
challenge. In addition to the various activities offered, we have also had a
significant role in organizing "Go the Distance Day". This free one-day
event is a result of the partnership between Live Healthy Dubuque, Live
Healthy Iowa Kids, and the Dubuque Community School District.
Together, we serviced over 800 area fourth-graders by inviting them to
participate in an assembly to celebrate healthy lifestyles among our youth.
National and local athletes spoke to the kids as an effort to increase
activity awareness while incorporating the aspects of fun and goal setting.
Because of the cooperative efforts throughout the city, Live Healthy
Dubuque has ranked second highest in participation in the state of Iowa
for three years, averaging 2, 600 participants per year. !t is the City of
Dubuque's goal to continue to build partnerships and ultimately make
health, wellness and prevention a personal and community priority.
21. A requirement of funded agencies will be to form a local coalition of stakeholders
and community leaders or CHART (Community Health Action and Response
Team). The team will collaborate in order to make policy, systems and
environmental changes within your community and will travel to an Action Institute
(tentatively scheduled for July 2009, location, TBD). What organizations within
the community do you feel are essential to this collaboration? Please list the
organization names or sectors you are considering and a brief explanation of
their potential contribution.
The following organizations will be asked to be a part of the coalition:
• City of Dubuque Health Services Department for their knowledge of
chronic diseases and prevention.

• Dubuque Area Chamber of Commerce for connection to the business
community.
22. Are there community organizations or groups that would assist in addressing or
improving the health and wellbeing of underserved or at-risk populations (i.e. low-
income, and/or racial/ethnic groups in your community?
The Crescent Community Health Center was developed to provide health
care for the under-served or at-risk populations. They will be an important
member of this coalition and will be the connection to low income citizens.
There are eleven neighborhood associations in Dubuque that will be used
to communicate to citizens and promote the program. Local service clubs
will also be used in this manner.
23. Do the Mayor and local leaders support health promotion initiatives? If your agency
becomes an ACHIEVE site, is the Mayor's support vital to the success of your
efforts?
The mayor and city council are very supportive of health initiatives and
have approved submission of this application. The city was very involved
with establishment of the Crescent Community Health Center, both in
terms of dollars and staff time. This is a low or no cost health clinic for low
income citizens. Sustainability is a top priority of the mayor and city
council and they provide the financial support for this effort. The city's
health services manager is involved with health issues on the state and
local levels and presents many health related programs for broadcast or
cable television.
24. Has the park and recreation agency recently (within the past 5 years) conducted a
Community Needs Assessment OR has the agency participated in a Needs
Assessment? Please describe what the Needs Assessment unveiled or how it has
influenced your agency.
We have not conducted a needs assessment specific to health but we
have completed an extensive community visioning process called Envision
2010 which set ten goals for the community to achieve in ten years.
Establishment of the Crescent Community Health Center was one of these
goals. Integrated walking/hiking/biking trail system was another goal. A
non-profit group called the Tri-State Trail Vision has been established to
assist in this effort.

The coalition that will be formed will lead the effort to conduct the Needs
Assessment. The agencies identified to be a part of the coalition should
make the assessment a fairly easy process.
25. Has your agency instituted a health program or initiative that did not go as planned
or did not succeed?
No
26. The funded NRPA ACHIEVE demonstration sites will each receive a $30,000 one-
time award. However, funded agencies will be asked to make athree-year
commitment to the ACHIEVE initiative by continuing work on policy, systems and
environmental changes. Agencies will receive guidance and technical assistance
from NRPA and CDC beyond year one and will be asked to continue evaluating
their work for at least 3 years. Please explain what efforts you will take to
ensure sustainability of the initiative within your community beyond the one-
time funding.
As mentioned before, we hope to hire a physical fitness director that will
lead this effort as part of their responsibilities. If the position is not funded,
one of our present program supervisors will be assigned the duties of this
grant. The coalition members of Live Healthy Dubuque are committed to
this effort and will remain involved. If actions of the coalition are as
successful as anticipated, some fund raising maybe needed to sustain the
effort.
It seems that work on policy, systems and environmental changes will
note be real costs. Putting those changes in place may have some cost
but would need to be budgeted.
27. How do you plan to spend the one-time funding you'll receive of $30,000 to support
local efforts to address the chronic disease risk factors of physical inactivity, poor
nutrition, and tobacco use at the policy, systems, and environmental change level?
Budget would include travel ($10,000); funding for the physical fitness
director, contracted services, or program costs ($10,000); expansion of
the Live Healthy Dubuque effort ($5,000); and assistance at the Crescent
Community Health Center with a disease prevention effort ($5, 000).

We are interested in learning about your understanding of utilizing policy, systems and environmental change strategies in
creating sustainable practices that promote health and wellbeing, particularly through healthy eating and active living, within
a community.
14. Please describe experiences your agency has had in implementing policy, systems and
environmental change strategies to impact the health and wellbeing of your community.
What has been the impact of using policy, systems and environmental change strategies to sustain
healthy eating, active lifestyles and community livability practices in the community?
Consider the broader and greater impact that can be made from implementing a policy or making
changes in the physical environment of your community versus using programs alone. Additionally
consider how using policy and environmental change strategies can be more sustainable than creating
stand alone programs.
Please refer to page 6 of the Application FAO for definitions and more information on policy, systems
and environmental change strategies. Reminder: "policy changes" can refer to changes in a broad range
of organizational policies or practices -not just government policies -that promote healthy eating and
active living.
The State of Iowa recently passed a law prohibiting smoking in public buildings and

' Our park division comprises 23 full time and 22 part-time employees, 4i parxs wl~ii
park land, 19 tennis courts, 3 outdoor ice rinks, in-line hockey rink, two skate pax
of play equipment, 8 softball fields, 1 baseball field and 25 miles of trails. The
division has 9 full time and 137 part-time employees that plan and direct a full rar
for all ages, operates two outdoor swimming pools, and an 18-hole golf course.

Our agency has begun to work on the physical fitness and nutrition side of this isst

The mayor and city council are very supportive of health initiatives and have appro~
of this application. The city was very involved with establishment of the Crescent
Health Center, both in terms of dollars and staff time. This is a low or no cost hE

26. The funded NRPA ACHIEVE demonstration sites will each receive a $30,000 one-time award.
However, funded agencies will be asked to make a three year commitment to the ACHIEVE initiative
by continuing work on policy, systems and environmental changes. Agencies will receive guidance and
technical assistance from NRPA and CDC beyond year one and will be asked to continue evaluating
their work for at least 3 years.
Please explain what efforts you will take to ensure sustainability of the initiative within your community
beyond the one-time funding.
As mentioned before, we hope to hire a physical fitness director that will leaa Lni~
of their responsibilities. If the position is not funded, one of our present progr~
will be assigned the duties of this grant. The coalition members of Live Healthy DL
committed to this effort and will remain involved. If actions of the coalition are
as anticipated, some fund raising may be needed to sustain the effort ._
