Casualty Group is requesting reimbursement in the amount of $1'1,690.82, as a result ora
bl0~ked sewer line at their insured's place of business.
Also enclosed are earlier letters from both Patsy Frith and Claim~ .aziminisfl-ator, Jim
8hatto, and my responsive correspondence informing them that in order to file'a claim
against the City of Dubuque, they would need to fill out a City of Dubuque 'Claim Form,
which I had sent to each of them. We have yet to receive s completed claim form. from
Ohio casualty Group regarding this matter.
