Introduction
The purpose of this memorandum is to present the September 30, 2017 Consent
Decree Semi Annual Report to the City Council.
Background
The City of Dubuque, Iowa is under a Consent Decree according to Case 2:11-cv-
01001-EJM, United States of America and the State of Iowa v. the City of Dubuque, IA
(N.D. Iowa), Civil Action Number 2008V00041 , DOJ Case Number90-5-1-1-09339
effective June 27, 2011. Per the Consent Decree, the City is required to submit a Semi
Annual Report on March 31St on September 30th after the effective date of the Consent
Decree.
Discussion
The purpose of the Semi Annual report is to provide information related to the City's
efforts to comply with the Consent Decree. The report specifically includes
documentation on all Sanitary Sewer Overflows, a summary and status of projects
including the Water Resource Recovery Center upgrades, a listing of any NPDES
violations, and efforts and actions taken to reduce sources of infiltration and inflow.
Action to be taken
No action requested. The Semi Annual Consent Decree report is provided for the
information recorded therein.

2.0 Introduction
2.2 BACKGROUND
The City of Dubuque, Iowa is under a Consent Decree according to Case 2:11-cv-01001-EJM,
United States of America and the State of Iowa v. the City of Dubuque, IA (N.D. Iowa), Civil
Action Number 2008V00041, DOJ Case Number 90-5-1-1-09339.
The Consent Decree became effective on June 27, 2011 and requires the City to submit
semiannual reports to the Department of Justice (DOJ), Environmental Protection Agency (EPA)
and Iowa Department of Natural Resources (DNR).
2.3 PURPOSE
The purpose of this report is to satisfy Section VIII, Paragraph 25 of the Consent Decree.
Paragraph 25 requires the City to submit semiannual reports on March 31 and September 30 after
the Effective Date (June 27, 2011). The report includes summary information regarding events
and activities performed in the prior six months. Therefore, the "reporting period" shall refer to
the time between April 1, 2017 and September 30, 2017, minus the document transportation
dates to ensure deliver of the report by the deadline.
2.4 REPORT SUMMARY
Paragraph 25 of the Consent Decree requires the semiannual reports to include specific
information as well as a summary of the status and progress of all projects and programs required
by Sections V and VI of the Consent Decree. Table 2-1 lists how the required information is
organized in this report.

3.0 Sanitary Sewer Overflows
3.1 SANITARY SEWER OVERFLOW RECORD
The City of Dubuque experienced seven SSOs during the reporting period with four (4) dry
weather and three wet weather SSOs. Table 3-1 and Table 3-2 list the locations and details of the
dry weather and wet -weather overflows respectively. The times reported as Start Times
document when the call came into the Public Works department. The precise start time of the
overflow may not be reported as it is dependent upon a member of the public or City staff
observing and reporting the occurrence. The end time is documented as the time when the Public
Works department was able to return the flow back to the sewer flow line and contain any spill
that may have occurred.
3.1.1 Dry Weather Overflows
Dry weather overflows were observed at three locations during the reporting period. In one
instance, buildup of debris in the sewer pipe caused the overflow. In the other two instances, pipe
failure was the primary cause. The first resulted in a basement backup at 777 Angela Street. The
second resulted in wastewater existing the sewer through manholes on Cedar Lake Drive, a
private street. The third involved wastewater existing the sanitary sewer on Angella Street and
flowing into the storm sewer.
3.1.1.1 Basement Backup Overflow at 777 Angela Street
The basement backup at 777 Angela Street occurred on May 25, 2017. The overflow/bypass was
reported to the Public Works Department by the homeowner at 1:00 p.m. By the time the City
visited the site a short time later, the wastewater had been removed by the homeowner. The
overflow was reported to the Iowa Department of Natural Resources around 10:30 a.m. on May
26, 2017. Sewer maintenance crews found an obstruction in the City sewer due to a pipe defect.
Crews were able to remove the blockage on the day the backup was reported. Based on a follow-
up CCTV inspection the next day, a pipe defect was discovered and corrected that day. The
homeowner had already cleaned up the basement. Based on information from the homeowner, it
is estimated that 100 gallons of untreated wastewater spilled into the basement.
3.1.1.2 Plymouth Court Overflow
The basement back-up at 1761 Plymouth Court was reported to the Public Works Department
around 11:00 a.m. on Thursday, July 20, 2017. A maintenance crew was dispatched to the
location and found a blockage in the City line on Overview Court. The line was jetted open by
11:30 a.m. on Thursday. It is estimated that 50 gallons of untreated wastewater spilled into the
basement. The overflow was reported to the Iowa Department of Natural Resources around
2:OOp.m. on July 20, 2017.
3.1.1.3 Cedar Lake Drive Overflow
The overflow on Cedar Lake Drive occurred between July 29, 2017 and August 3, 2017. The
overflow/bypass was reported to the Public Works Department at 11 :30 a.m. on Saturday, July
29th. At that time, City records showed that the line in question was private and the issue was

turned over to the Homeowners Association. On Monday, July 31st, the City Engineer was
contacted by a homeowner representative and a subsequent record review was conducted. By
Thursday, August 3rd, after the reviewing City Council records, it was determined that the intent
of the developer was to make the water and sewer improvements public while the streets remain
private. On Thursday afternoon the Public Works Department dispatched crews to eliminate the
overflow. Sewer maintenance crews found untreated wastewater flowing out of the pavement on
Lake Ridge Drive, near 3158 Cedar Cross Rd. Crews were able to locate the downstream
manhole and remove the blockage from the line by 3:00 pm. The overflow was reported to the
Iowa Department of Natural Resources at approximately 3:30 pm on August 3, 2017. The line
has was cleaned and inspected with CCTV equipment. No pipe defects were found. The cause of
the blockage was a plugged pipe due to a build up of debris in the line. The area around the
manhole was cleaned, and a warning sign posted. The sign will stay up for the required six
months. It is estimated that 1,000 gallons of untreated wastewater spilled on to the ground.
3.1.1.4 Angella Street Overflow
The overflow on Angella Street occurred on September 13, 2017. At approximately 4:00 p.m. on
September 13th, the Public Works Department was notified of an odor issue in the 700 block of
Angella Street. An investigation was made and it was determined that flow from the sanitary
sewer system was entering the storm sewer system. On Thursday September 14th, the location of
the failed pipe was identified and a repair was completed by 2:00 p.m. that same day. The
overflow was reported to the Iowa Department of Natural Resources at approximately 1:45 pm
on September 14, 2017. The area around the manhole was cleaned, and a warning sign posted.
The sign will stay up for the required six months. It is estimated that 500 gallons of untreated
wastewater spilled on to the ground.

3.1.2 Wet Weather Overflows
There were three wet -weather SSOs during the reporting period. All three occurred during a 64 -
hour period over four days in July that produced 4.0 inches of rain and enough property damage
to garner a FEMA Major Disaster Declaration (DR 4334). Two of the overflows involved
wastewater backing up into basements. The third involved overflowing manholes on W. Locust
Street and Rosedale Avenue, an area within Consent Decree Sewershed 11.
3.1.2.1 Basement Backup Overflow at 85 N. Booth Street
The basement backup at 85 N. Booth Street was reported to the Public Works Department at
1:00 a.m. on the morning of July 22, 2017. Even though the N. Booth area was not one of the
five Sewersheds identified in the I&I study completed as part of the Consent Decree, the City has
in the past performed smoke testing, home inspections, and CCTV inspections of the sewer lines
in order to reduced I&I induced basement backups. While it is hard to draw conclusions based on
one data point, but this area should be watched closer and would be a good candidate for
deploying portable flow meters in the next few years to quantify the I&I in this area.
3.1.2.2 Basement Backup Overflow at 2436 Lincoln Avenue
Like the Booth Street backup, the basement backup at 2436 Lincoln Avenue was reported to the
Public Works Department at 1:00 a.m. on the morning of July 22, 2017. This area is within
Sewershed 10 which is the last of the five Consent Decree Sewersheds to be studied. While some
of the improvements outlined in the Corrective Action Plan for Sewershed 10 were completed
within the current reporting period, the majority of the improvements were not completed by the
July 19-22 rainstorms. However, both a contract to replace manholes and a contract to line
sewers throughout the sewershed have been awarded. All of the work is expected to be complete
by the end of November of 2017.
3.1.2.3 Rosedale and W. Locust Overflow
As previously mentioned, the overflowing manholes on Rosedale and W. Locust Street occurred
during a 64 -hour period over four days in July that produced 4.0 inches of rain and enough
property damage to garner a FEMA Major Disaster Declaration (DR 4334). At approximately
1:00 a.m. on July 22nd, Public Works Department staff discovered that sanitary sewer manholes
had overflowed on West Locust Street near Rosedale Avenue (2), Rosedale near W Locust and
on Rosedale near Glen Oak. The manholes were allowed to overflow into the street, with
overflow conditions ending around 2:00 a.m. While this is an area where the City has been
successful at avoiding SSOs by pumping wastewater into tanker trucks, this option was not
available due to the hour of the overflow and the inclement weather at the time. It is estimated
that 2,000 gallons of untreated wastewater spilled on to the ground and washed away with the
stormwater runoff.

3.2 SSO'S EFFECT ON PUBLIC HEALTH AND WATER QUALITY
There is always concern regarding public health, welfare and the environment with any overflow
event. City staff conducted visual inspections of the overflow areas to document the condition of
the site and to perform clean-up activities as necessary.
The SSOs were reported to the Iowa DNR Manchester Office and signs were posted in
accordance with the Consent Decree at locations near the intersections where the overflows
occurred.
3.3 MEASURES ENACTED TO MINIMIZE DURATION AND IMPACT
The City minimized the duration and impact of the SSOs by quickly responding to, and taking
steps necessary to halt, the SSOs after they were discovered by City staff. The City maintains
pumps and hose to return wastewater flows back to the sanitary sewer system and minimize the
duration of the overflow.
3.4 MEASURES ENACTED TO STOP THE OVERFLOW EVENT
The City was notified of the SSO related to basement backup by the homeowner. The site was
cleaned up before the City crew arrived. City crews jetted the line and returned the next day to
replace the section of defective pipe. In the case of the blocked sewer pipe on Cedar Lake Drive,
the City jetted the line to remove the blockage and reinstate the free-flow of wastewater. No
actions were taken during the wet weather overflow. At the time, there was severe weather in the
area creating hazardous conditions. According to the National Weather Service, "thunderstorms
quickly developed ... and moved across the region. Storms produced hail as they first
developed and then became more of a damaging wind threat as they moved eastward across the
region." Large, 3 -foot diameter street trees were felled during the storm, blocking streets and
compromising power lines. On August 27, 2017 FEMA issued a Major Disaster Declaration (DR

4334) due to "Severe Storms, Tornadoes, Straight-line Winds, And Flooding." The overflow
ended prior to the severe weather moving out of the area.
3.5 MEASURES ENACTED TO PREVENT ANOTHER RECURRANCE
As mentioned, the defective pipe that caused the basement backup at 777 Angella Street
(Sewershed 11) was repaired. The City's response to the overflow on Lake Ridge Drive was
atypical. When the overflow was first reported, it was thought that the line in question was a
private sewer line. It serves a subdivision built in the late 1970s with private streets. As such, the
matter was turned over to the private Homeowner's Association. The City Engineer was
subsequently contacted by the original developer to review the matter. As it turns out, the
original intent was for the sewer to become public. Therefore, City crews mobilized to clean and
inspect the sewer. They removed the blockage and returned the sewer to a free-flow condition.
They have also added the sewers in the subdivision to the City's City-wide collection system
maintenance schedule. Moreover, due to the unusual set of circumstances surrounding this
overflow, many of the City standard procedures were not followed. A department review of this
response is underway to better prepare the City for a timelier response for any similar
circumstances in the future.
The location of the wet weather overflow at Rosedale and W. Locust Street has historically been
an area prone to I&I induced flows in the sanitary sewer system. This area falls within
Sewershed 11 which was the subject of a comprehensive I&I investigation per the terms of the
Consent Decree and the subsequent Corrective Action Plan for the watershed which reflects a
completion date of December of 2021. While many of the improvements within Sewershed 11
have been completed, the lack of adequate storm sewers on W. Locust Street means that
stormwater ponds to depths in excess of two feet. The City has budgeted to increase the capacity
of the storm sewer system serving the area. The first, downstream section is currently under
construction. Storm sewer improvements are expected to be extended up W. Locust Street by
June 2021.
3.6 CAPTURING KEY WAY OVERFLOWS
Paragraph 10 of the Consent Decree requires the City to capture all overflows from the manhole
located on Key Way Drive between 3500 Keymer Drive and 3507 Keystone Drive. No sanitary
sewer overflows occurred at this location during the reporting period.
3.7 WARNING SIGN POSTINGS
Paragraph 10 of the Consent Decree requires that the City post a warning sign at all SSO sites for
at least 6 months after completion of the site cleanup, unless IDNR decides that the sign may be
posted for a shorter period of time or not at all. Warning signs were posted at locations near the
intersections where the overflows occurred and will remain at least 6 months.

4.0 Water & Resource Recovery Center
Construction Upgrade
The information stated in this section has been presented in previous Semi Annual reports. It is
restated herein for reference and to maintain compliance with the Consent Decree's reporting
requirements.
4.1 PROJECT PLANNING
During its Fiscal Year 2006 goal setting process, the City Council listed the Water Pollution
Control Plant (WPCP) as one of its high priorities. City staff subsequently applied for a State
Revolving Fund (SRF) planning and design loan during the summer of 2006, and plant upgrades
were placed on the DNR Intended Use Plan schedule. Following public hearings and bidding
requirements, the Facility Plan was developed and approved by DNR. The facility's name was
also changed to the Water & Resource Recovery Center (W&RRC).
In October 2008, the team of Strand Associates and IIW Engineers was approved by the City
Council to proceed with the design of the W&RRC upgrades. The NPDES permit was issued by
DNR staff on April 1, 2010, and the plant construction permit was signed by DNR staff on April
5, 2010.
Following public bidding requirements, the City Council approved a contract, and the notice to
proceed was issued August 19, 2010. Table 4-1 lists the revised significant deadlines for the
project, which take into account project delays resulting from weather related force majeure.

4.2 CURRENT PROJECT STATUS
The City of Dubuque's lead engineer, Strand Associates, certified the project as substantially
complete. A copy of the Substantial Completion letter was submitted with the March 31, 2014
Semi Annual Report. The W&RRC is fully operational and with all upgrades completed since
August 2013, well within the compliance date set by the Consent Decree.
As stated in their letter of March 23, 2017, the DNR inspected the W&RRC on 4/2/15 as a
requirement of the SRF program and prerequisite to finalizing the loan(s). It was the
determination of the DNR that the "project is considered complete for the purposes of finalizing
the loan(s)."
In March of 2017, the City Council formally accepted the improvements to the W&RRC and
established the final contract in the amount of $54,942,567.00, officially closing the contract.
4.3 BIOSOLID ACCUMULATION PREVENTION AT THE W&RRC
In February 2013, the City installed the new solids handling equipment (two centrifuges), and
entered into a contract with Nutri -Jet Systems of Hudson, Iowa to manage the bio -solids, thereby
preventing accumulation of bio -solids at the W&RRC.

System (NPDES)

5.2 NPDES VIOLATIONS AT THE WRRC DURING THE REPORTING PERIOD
On the evening of September 5, 2017, the W&RRC experienced a power outage due to the utility
grid being down; a semi tractor -trailer rolling into a utility pole in Key West area south of
Dubuque. The outage lasted for approximately 36 minutes (from 8:47 p.m. to 9:23 p.m.). Even
though the facility is equipped with backup generators, Operations staff experienced trouble with
the electrical switchgear and were not able to transfer to generator power. Maintenance staff was
called to the facility and was able to make the switch to generator power. During the power
outage, approximately 200,000 gallons of final effluent was discharged from the facility without
ultraviolet disinfection (flow through the facility was approximately 8 MGD at that time). The
issue was reported to the Iowa Department of Natural Resources within the 24-hour reporting
period followed by a follow-up letter describing the event.
There were no other NPDES permit violations at the WRRC during the reporting period.
W&RRC staff collected samples on March 17th, 19th, 24th, 27th, and 31 St, during the previous
Consent Decree reporting period, and tested for E.coli. The testing results were not known at the
time the March 31, 2017 Semi -Annual Report was submitted. The geometric mean of the five
samples was 738.8 MPN/1,000 mL. This exceeds the limit of 126 #/100mL specified in the
City's NPDES permit, representing an NPDES permit violation. The violation was reported to
the Iowa Department of Natural Resources in an April 12, 2017 letter. As stated therein, the
cause of the elevated post disinfection E. coli. counts was believed to be poor UV transmittance
of the final effluent. To improve transmittance, on 3/28/17 the activated sludge wasting was
reduced on 3/28/17 from 80,000 gal/day to 75,000 gal/day. On 4/3/17 activated sludge wasting
was reduced to 72,500 gallons/day and the activated sludge return rate was reduced from 34.4%
to 32% of influent flow. Since these changes were made, transmittance has improved, leading to
greater disinfection and compliance with the NPDES permit.

6.0 Inflow & Infiltration Reduction Program
6.1 BACKGROUND
Prior to the Consent Decree date of lodging, the City had initiated a study to identify I/I within
the sewer system utilizing flow meters strategically placed throughout the City. Depending on
location, the meters measured the flow from each Sewershed for over a month with some up to
an eleven month period. Five of the twelve sewersheds monitored were identified as receiving
elevated flows during and directly after a precipitation event. Under the Consent Decree, the City
agreed to investigate these five sewersheds, illustrated in Figure 6-1, before January 1, 2016.
As required, the City completed an investigation of the five sewersheds prior to January 1, 2016.
Based on the investigation, the City has identified the capital improvements necessary to
eliminate significant sources of inflow and infiltration of storm and ground water into the
collection system to prevent wet weather SSOs. These improvements were identified in the
Consent Decree Corrective Action Plan.
The City recognizes that its work toward the goal of eliminating sanitary sewer overflows will
continue beyond the sources identified during the investigations prior to January 1, 2016,
therefore the City has invested and continues to invest in long term permanent flow metering of
its collection system. The investment in long term flow metering will not only show the
reduction of inflow and infiltration as a result of capital improvements made to the collection
system, but also highlight areas where additional work may be necessary. Section 6.2 describes
the ongoing flow metering effort.
6.2 FLOW METERING
Even though the required investigation into the causes of I&I and their contribution to SSOs as
outlined in the Consent Decree has been completed, the City continues to voluntarily monitor
sanitary sewer flows at key locations throughout the City. This continued collection of flow
metering data will inform the effectiveness of improvements and to observe changes from earlier
flow metering data. During the reporting period, flow data was collected in Consent Decree
Sewersheds 5, 7, 11, and 12. Due to the ongoing construction of the $60 million Bee Branch
Creek Restoration Project, the re -configuring of permanent flow meters for Sewershed 10 hasn't
been completed. Therefore, it has not operational during the current reporting period.
During the reporting period, there were five notable rains with more than 1 inch of rainfall and
with rain intensities greater than 0.5 inch per hour: April 15th, July 11 ti' to 12th, July 19th to 22nd,
August 16th, and August 27t1'. The following sections document the response of the system in
each of the Consent Decree sewersheds to these events.

6.2.1 Sewershed 5 Flow Metering
The City reports Sewershed 5 flow data via a permanent flow monitor located at the intersection
of West 32nd Street and Central Avenue, using the City's extensive fiber optic network onto the
City's SCADA system. The City is capable of real time monitoring of sanitary sewer flows
during rain events. The permanent flow metering station came online November 13, 2015. The
site was selected due to its ease of access, proximity to a fiber optic network via the traffic
control system, electricity, and the ability to review flow data from the Sewershed in isolation
from other sewersheds. The City continues to use Teledyne/ISCO flow meters and installed the
latest generation of flow meter, the Signature meter. The meter body is installed in a nearby
traffic cabinet and it relays real-time flow data and alarms via the City's fiber optic network.
While there were five notable rains, more than 1 inch of rainfall and with rain intensities greater
than 0.5 inch per hour, during the current reporting period, there were no SSOs within Sewershed
5 — wet weather or otherwise. And as reported in previous reports, the I&I source identification
and removal efforts [home inspections, manhole improvements/replacements and sanitary sewer
lining] appeared to have made a difference, resulted in reduced levels of I&I. The City will
continue to track and monitor the system following improvements and significant rain events.
The permenant flow meter for Sewershed 5 is not in the same sewer line where previous wet
weather flows were recorded. While comparisons going forward may prove valuable, they
provide little insight at this point. Table 6-1 does reflect that the ratio of west weather flow
versus total rainfall has been decreasing.

6.2.2 Sewershed 7 Flow Metering
The flows through Sewershed 7 are metered at two permanent flow metering sites located at
White Street next to Fulton School (7A Master Meter) and 2404 White (7B Master Meter) as
wastewater flows through two distinct areas. Flow meter data is being collected and retained for
future comparisons to observe emerging patterns. There were no SSOs in Sewershed 7 during the
reporting period.
During the July 11th to 12' event, the peak wet weather flow measured at 7A peaked at 0.15
mgd, 3.75 times the average dry weather flow. However, the time to peak was more gradual than
the October 26, 2016 wet weather event discussed in the previous report. Because the 0.15 mgd
is the peak instantaneous wet weather flow and not the peak hourly flow, the peaking factor is
exaggerated to some degree. During the July 19, 2017 rain, the peak flow rate measured at 7B
jumped to 0.98 mgd, a 12 -fold increase from the dry weather flow rate of 0.04 mgd. During this
event, the rainfall intensity reached and remained at 1.1 inches per hour for an entire hour. Three

days later, during the rain event on July 22, 2017, the rainfall intensity never exceeded 0.5 inches
per hour. And yet, the days 1.5 inches of rain, on top of the previous three day's rain totaling 2.5
inches, resulted in a wet weather flow of 2.0 mgd, 25 times the average daily flow. The graphics
show trending where the systems recovers quickly to normal levels, therefore the focus will
remain on improvements to address inflow. Improvements are scheduled for the area upstream of
site 7B through 2026.
The City will continue to investigate the area for sources of inflow and infiltration and will look
to deploy portable flow meters within the sub -watersheds to try and further identify where the
I&I originates. However, because the wet weather induced spikes in flow, however extreme they
appear, did not result in an SSO, this area would be a lower priority than sewersheds where wet -
weather flows have resulted in SSOs.
6.2.3 Sewershed 10 Flow Metering
The Sewershed 10 drainage area changed since filing of the Consent Decree in 2009 and
completion of the required monitoring period in 2014. Construction of the Upper Bee Branch
Creek Restoration Project, one phase of the $219 million Bee Branch Watershed Flood
Mitigation Project, started in early 2015. The project involves the removal of a buried storm
sewer and daylighting the creek. The work required the relocation/reconstruction of public
utilities, including sanitary sewer that both serve and/or pass through the project footprint. As a
result, all of the flow originating in Sewershed 10 no longer passes through the meter at
Rhomburg and Kniest (10-A) as it did prior to 2015. Instead, a second meter, known as site 10-
B, was added in a 36 -inch diameter sewer located adjacent to 2256 Prince Street installed as part
of the Upper Bee Branch Creek Restoration Project. However, due to the ongoing construction of
the Upper Bee Branch Creek Restoration Project, the re -configuring of permanent flow meters
for Sewershed 10 hasn't been completed. Therefore, it has not operational during the current
reporting period.
6.2.4 Sewershed 11 Flow Metering
The City continues to collect flow data at three locations within Sewershed 11 located in and
around the Rosedale and Locust area where rain related sanitary sewer overflows have been
historically documented. Three SSOs occurred within Sewershed 11 during the reporting period,
although only one was due to an I&I induced wet weather SSO. Unfortunately, the flow meter
was unable to collect meaningful data during the storm.
Table 6-4 shows flow data collected during the April 15, 2017 rain compared to early spring
rains in the past. It reflects a downward trend in the ratio of the instantaneous peak wet weather
flow rate to the average dry weather flow, maybe showing evidence of progress made reducing
the I/I coming from the Rosedale area and W. Locust Street area west of Rosedale. However,
because of the small sample size and that the 2017 rain was spread over a longer period of time,
less I&I and a smaller ratio could be expected. Moreover, the sewer system became
overwhelmed during the July 19-22 rainstorm resulting in an SSO. This could be due in large
part due to undersized storm sewers. The City has budgeted to increase the capacity of the storm
sewer system serving the area. The first, downstream section is currently under construction.
Storm sewer improvements are expected to be extended up W. Locust Street by June 2021.

6.2.5 Sewershed 12 Flow Metering
Sewershed 12 has a permanent flow meter installed at 1St Street and Locust Street; this will
monitor all flows passing through the Sewershed. In addition, the City continues to meter flow
upstream of the master meter at four sites. A meter location map and available flow data may be
found in Appendix F. The purpose is to evaluate improvements that were made in Sewershed 12,
which included sanitary sewer CIPP lining, manhole replacement, and sanitary sewer repairs and
establish the benchmark for further work which is scheduled for the area.
Flow data collected for the August 27, 2017 rain is summarized and compared to historic August
events in Table 6-5. The comparison is tenable as the permanent flow meter is not in the same
place as the sewershed flows recorded in 2016 and 2015. Measuring the flow characteristics of
the sanitary sewer system in Sewershed 12 has been challenging due to excessive pipe slopes.
The new permanent location should result in more reliable, useful flow meter date.

6.3.1 Private Property Inspections
Private property inspection progress is reported in
Table 6-6. Private property inspections identify roof drain discharge locations, the presence of
stairwell or driveway drains, and the presence of a sump pit/pump on site. A site passes
inspection only if there are no connections of clear water to the sanitary system. A failed
inspection indicates a connection of clear water directly into the sanitary system. This may

include roof drains connected to the sanitary sewer, a stairwell or driveway drain connected to
the building floor drain, or a sump pump plumbed directly to the sanitary lateral or floor drain
within the building.

notice. The property owner then has 30 days to correct the improper connection, at which point a

6.3.2 Dye Testing
Dye testing is performed in conjunction with private property inspections if the discharge
location of a downspout, exterior stairwell drain, or driveway drain is unknown. The City will
use dye to identify the discharge location(s). If the dye is found in the sanitary sewer, the
property owner receives notice consistent with the private property inspection process described
in Section 6.3.1.
During the reporting period, no new areas were dye tested by the City. The City will perform
additional testing, as necessary, following private property improvements to confirm
improvements have had the desired effect in eliminating direct sources of storm water inflow.
6.3.3 Smoke Testing
During the reporting period, no new areas were smoke tested. Additional smoke testing is
planned for Sewersheds 5 and 10 and will be conducted following private property
improvements to confirm the improvement has the desired effect or prompt further corrective
action.
6.3.4 Manholes
During the reporting period, manhole inspections were not tied to targeted sewersheds. Instead,
they were done as part of street resurfacing projects and the manhole replacement project.
During the reporting period, the City awarded a manhole replacement project in accordance with
the corrective action schedule for Sewershed 10.
6.3.5 Sanitary Sewer
The City uses CCTV footage recorded since 2010 to help determine system maintenance and
replacement needs. The recorded footage allows the City to cracks, leaky joints, illegal lateral
connections, and damaged or broken pipes. The severity of the structural degradation of each
pipe section is rated based on a severity scale following the NASSCO (National Association of
Sewer Service Companies) rating schedule. The rating system is used to assist the city in
prioritizing which pipe sections are in need of immediate improvement. This work combined
with the flow monitoring data provides the City with direction on how and where to make valued
capital improvements.
During the reporting period, the City awarded a sanitary sewer lining project in accordance with
the corrective action schedule for Sewershed 10.
6.4 CORRECTIVE ACTION PLAN
It is the City's intent to complete the necessary work identified in the approved Corrective
Action Plans in a timely manner. If response to emergency events and unexpected issues arise
which indicate delay in implementation, the City will submit a formal request to amend the
schedule to the U.S. EPA and the Iowa DNR. The City requested modification to its Corrective
Action Plans in May 2016 and continues to implement improvements in each of the Sewersheds
per that schedule. Per the Consent Decree the City provides the Corrective Action Plan
Implementation Schedule in Section 7.0 of this report.

7.0 Corrective Action Plan Implementation
Schedule
In April of 2016, the City of Dubuque submitted a Status Update Report reflecting a modified
Corrective Action Plan for each Sewershed identified in the Consent Decree. During the course
of planning, design, and construction it became necessary to adjust project completion dates, as
clarified in the Status Update Report. For all Public Property projects to be completed by the
City, the reporting tables herein reflect the dates outlined in the City's April 2016 Corrective
Action Plan Status Update Report.
7.1 SEWERSHED 11 CORRECTIVE ACTION PLAN SCHEDULE
The Sewershed 11 Corrective Action Plan was formulated in December of 2011. It includes
corrections by private property owners based on home inspections and smoke testing. It also
includes corrections to the public sewer system, namely manhole and sewer pipe corrections.
7.1.1 Private Property Inspection Corrections
The City continues to work with property owners to make corrections to their private properties.
These corrections are made to private properties and therefore dependent upon funding by the
property owner. The City's intent is to work with owners to achieve completion of the
corrections without placing undue financial hardship on the private property owners. In 2014,
the City documented twelve private property owners with sump pump, drive way drain, or
downspout connections to the sanitary sewer system; these have since been reduced to one. The
remaining connection is a driveway drain. There are several options available to disconnect the
drain from the property's sanitary sewer lateral however each comes at a substantial cost to the
property owner, and in the event of failure or blockage of an improvement the owner is greatly
concerned that they will experience property damage. Currently the drive way drain protects the
property from flooding, as there are no storm sewers for storm water to enter in the immediate
area. As such, the City is prepared to assist the property owner and install a possible gravity
solution as a back-up or emergency overflow to the mechanical pump that would be required.
The City anticipates resolving the issue by September 2019. The total number of private property
inspection corrections required as reported in the Sewershed 11 Corrective Action Plan was 79.
Subsequent smoke and dye testing discovered 3 additional connections subject to corrective
action. Therefore, the number of properties was adjusted up from 79 to 82. As of this reporting
period a total of 81 of the 82 are corrected.

7.1.2 Private Property Corrections
Smoke testing completed in 2011 identified 27 issues on private property. A total of 13 of the 27
identified private issues have been corrected. Several of the corrective actions listed include
televising the sanitary sewer lateral. Televising the lateral may be costly for private property
owners and may not provide conclusive evidence of inflow or infiltration, similar to televising of
public mains. Therefore, the City will assist the private property owners with financial
assistance, as funds are available, to televise the lateral and perform either dye or flood testing
during the televising operation to determine the condition of the resident's lateral.

7.1.3.1 Manhole Correction Schedule
The table below includes seven (7) manholes that remain following the 2017 construction
season. These projects are directly tied to the replacement of sanitary sewers located in Wood
Street and Auburn and Custer Street areas. The sanitary sewer mains and associated manholes
are scheduled for removal and replacement by December 2021 in accordance with the schedule

7.1.3.2 Sewer Pipe Corrections
Based on CCTV inspection and scored in accordance with the National Association of Sewer
service Companies (NASSCO) Pipeline Assessment Certification Program (PACP). According
to NASSCO, "PACP is the North American Standard for pipeline defect identification and
assessment, providing standardization and consistency to the methods in which pipeline
conditions are identified, evaluated and managed." The City uses the rating system to assist i.n.
prioritizing pipe corrections. A PACP grade of 4 indicates that the pipe will probably fail in 5-10
years, while a grade of 5 suggests that the pipe is near failure or will likely fail within 5 years.
Out of the 22 sewer pipes at Grade 4, 17 have been corrected. The remaining 5 are scheduled for
removal and replacement by December 2021 in accordance with the schedule outlined in the
April 2016 Status Update Report. Of the 11 sewer pipes rated a Grade 5, 9 have been corrected.
The remaining 2 are scheduled for removal and replacement by December 2020 in accordance
with the schedule outlined in the April 2016 Status Update Report.

7.2 SEWERSHED 12 CORRECTIVE ACTION PLAN SCHEDULE
The Sewershed 12 Corrective Action Plan includes corrections by private property owners based
on home inspections and smoke testing. It also includes corrections to the public sewer system,
namely manhole and sewer pipe corrections.
7.2.1 Private Property Inspection Corrections
The City continues to work with property owners to make corrections to their private properties.
These are corrections made to private properties and therefore dependent upon funding by the
property owner. The City's intent is to work with owners to achieve completion of the
corrections without placing undue financial hardship on the private property owner. The total
number of private property inspection corrections reported in the original Sewershed 12
Corrective Action Plan was 73. This was adjusted to 72 when the City was able to verify that a

discharge line from a sump pit without pump was not connected to the sanitary sewer. As shown
in the table below, 71 of the 72 issue have been resolved.

7.2.2 Private Property Defect Corrections — Laterals
The City works directly with property owners to determine the degree with which corrective
action is required. The property owners are responsible for both the work and financial
responsibility of the sanitary sewer lateral from the building to the public sewer. The City
acknowledges the financial burden this may place on property owners, and those eligible to
receive assistance may request financial assistance from the City. Televising the lateral may be
costly for private property owners and may not provide conclusive evidence of inflow or
infiltration, similar to televising of public mains. Therefore, the City will assist the private
property owners with financial assistance, as funds are available, to televise the lateral and
perfoim either dye or flood testing during the televising operation to determine the condition of
the resident's lateral.

7.2.4.1 Sewershed 12 Storm Sewer Improvements (Catch Basins)
Smoke testing revealed issues with the sewer pipes along Hill Street with smoke coming out of
storm sewer catch basins. All 10 of the sewer pipes were subsequently cleaned, televised, and

rehabilitated by cured in place pipe lining. The work was completed by SAK Construction of
O'Fallon, Missouri. The lining contractor was also responsible for dye testing sanitary sewer
laterals prior to restoring sanitary sewer service to ensure only those active laterals were
reinstated; thereby reducing unnecessary openings to the sanitary sewer main and groundwater
inflow.

7.3.2 Private Property Defect Corrections
Smoke testing helped to identify 45 private properties that are listed in the Sewershed 7
Corrective Action Plan. To date, 17 of the 45 properties have been corrected. The property
owners are responsible for both the work and financial responsibility of the sanitary sewer lateral
from the building to the public sewer. The City acknowledges the financial burden this may
place on property owners, and those eligible to receive assistance may request financial
assistance from the City Televising the lateral may be costly for private property owners and

may not provide conclusive evidence of inflow or infiltration, similar to televising of public
mains. Therefore, the City will assist the private property owners with financial assistance, as
funds are available, to televise the lateral and perform either dye or flood testing during the
televising operation to determine the condition of the resident's lateral.

7.4 SEWERSHED 5 CORRECTIVE ACTION PLAN SCHEDULE
The Sewershed 5 Corrective Action Plan dated November 2014 includes corrections by private
property owners based on home inspections and smoke testing. It also includes corrections to the
public sewer system, namely manhole and sewer pipe corrections. The scheduled completion
dates reflect the dates outlined in the April 2016 Status Update Report.
7.4.1 Private Property Inspection Corrections
Private property inspections revealed 68 improper systems. The City continues to work with
property owners to make corrections to their private properties. These are corrections made to
private properties and therefore dependent upon funding by the property owner. The City's intent
is to work with owners to achieve completion of the corrections without placing undue financial
hardship on the private property owner. To date, 66 of the 68 corrections have been made.

Sewershed 5 inspections presented a new issue: a sump pit without a sump pump. The area was
more recently developed and properties were permitted to have pits installed without sump
pumps. These properties were inspected to ensure that the sump pumps or pits discharge lines are
not connected to the sanitary sewer and therefore are not illegal connections requiring corrective
action. The list of properties meeting this description was originally reported in the March 2015
Semi Annual report.
7.4.2 Private Property Defect Corrections
Based on smoke testing, 22 properties were identified as potential sources of I&I. To date, 17 of
the 22 amended total properties have been corrected. Again, the City acknowledges the financial
burden sewer line inspection/replacement places on property owners. Therefore, the City
attempts to work with each individually to try and arrive at cost-effective approaches to
addressing the private source of I&I.

7.4.3 Public System Corrections
Smoke testing and CCTV inspections helped identify corrections to elements of the public sewer
system included in the Sewershed 5 Corrective Action Plan. The scheduled completion dates
reflect the dates outlined in the April 2016 Status Update Report.
7.4.3.1 Manhole Corrections
In Sewershed 5, one hundred and twenty-two (122) manholes were originally determined to be
considered for correction, following smoke testing. These manholes were separated into two groups,
Sewershed 5 Manhole Corrections which are listed below, and Sewershed 5 manhole corrections
specific to the Carter Road Detention Basin Area which are listed in Section 7.4.3.2. The Manhole
Corrections have been reduced based on subsequent inspections. The table shows the remaining
56 sanitary sewer manhole corrections identified in the Sewershed 5 Corrective Action Plan. All
of the Sewershed Manhole corrections have been completed.

7.5 SEWERSHED 10 CORRECTIVE ACTION PLAN SCHEDULE
The Sewershed 10 Corrective Action Plan dated December 2015 includes corrections by private
property owners based on home inspections and smoke testing. It also includes corrections to the
public sewer system, namely manhole and sewer pipe corrections. The scheduled completion
dates reflect the dates outlined in the April 2016 Status Update Report.
7.5.1 Private Property Inspection Corrections
The City continues to work with property owners to make corrections to their private properties.
These are corrections made to private properties and therefore dependent upon funding by the
property owner. The City's intent is to work with owners to achieve completion of the
corrections without placing undue financial hardship on the private property owner. In
Sewershed 10, there were 163 failed private property inspections as identified in the Corrective
Action Plan. To date, 160 of the 163 have been corrected.

Based on smoke testing, 55 private property sites were identified for further investigation. Identifying
the source of I&I in a sanitary sewer lateral can be especially difficult and expensive for an
individual property owner. Financial assistance may be available in the City's budget, for work

7.5.3 Public System Corrections
Smoke testing and CCTV inspections helped identify corrections to elements of the public sewer
system included in the Sewershed 10 Corrective Action Plan. The scheduled completion dates
reflect the dates outlined in the April 2016 Status Update Report.
7.5.3.1 Sanitary Sewer Manhole Corrections
Fifty-five (55) high priority manholes were identified in the Corrective Action Plan for
Sewershed 10. The locations are spread throughout the Sewershed reflecting the consistent age
of the system. During the current reporting period 33 of the 55 manholes have been addressed
with another 15 currently under contract to be either replaced or spray -sealed.

7.5.3.3 Sanitary Sewer Replacement
In addition to sanitary sewer rehabilitation, the following table lists sanitary sewer improvements
where CIPP method is not recommended. These locations will require removal and
replacement/relocation of the existing sanitary sewer. The schedule reflects funding, surveying,
design, permitting, and construction of the improvements. The projects may also require

8.0 CMOM Update
8.1 THE CMOM PROGRAM
The City of Dubuque's Capacity, Management, Operation and Maintenance Plan (CMOM)
provides a summary of the City's policies, procedures, practices, and responsibilities associated
with its sanitary sewer collection system. It is the blueprint for the City's managers and field
workers to better manage, operate, and maintain Dubuque's sanitary sewer collection system,
investigate capacity -constrained areas, prevent SSOs, respond to SSOs that may occur, and plan
for future system expansion.
The EPA approved the City of Dubuque's CMOM plan on June 4, 2012.
8.2 CMOM IMPLEMENTATION
As required by Paragraph 13 of the Consent Decree, the City fully implemented the CMOM
within two calendar months (August 4, 2012) of approval by the EPA. Section 6.4 of the City of
Dubuque's CMOM Program, requires the City to annually inspect fifty (50) miles of gravity
sanitary sewer through various methods including visual inspection, pipeline jetting, or closed-
circuit televising (CCTV). The inspection is in addition to the predictive and preventive
maintenance also prescribed in the CMOM. The total of miles cleaned, televised, and inspected
in 2016 is 103.02 miles.
The City developed the CMOM to serve as a "living" document. As the City reviews its current
procedures and codes, the CMOM will be updated and new procedures implemented as needed.
Any revisions to the CMOM will be submitted to EPA, DOJ, and DNR for review and approval.
8.2.1 Fats, Oils, and Grease
The City purchased software and approved an ordinance amendment to support the W&RRC,
Building Department, Health Services Department and the Engineering Department in managing
and enforcing a FOG program. In addition, the City held a series of three public meetings with
local food establishments, vendors, plumbers and grease haulers on the revised FOG program,
the meetings were held on September 2, October 14, and November 4, 2014.
As part of the proactive program, the City has a dedicated staff member that inspects food
establishments to help ensure the proper use of grease interceptors. At inspections, food
establishments are provided with a Best Management Practice poster that is required to be visible
in the food establishment. The FOG Program requires food establishments to apply for an annual
grease discharge permit along with a $100 permit fee. The fee covers the direct cost of staff time
to perform inspections, managing the software and hardware, and vehicle expenses. During the
reporting period, grease was not responsible for any SSOs.

9.0 Assessments and Engineering Analyses
The City's Engineering Department, Public Works Department and Water &Resource Recovery
Center (W&RRC) staff continue to evaluate and analyze the sanitary sewer collection system
and W&RRC to ensure compliance with all legal requirements.
No new assessments or engineering analyses were completed during the reporting period.
