This is a claim in which the claimant states that the lid to his garbage can was lost during
refuse collection.
According to the report of Paul Schultz, Solid Waste Management Supervisor, the Lead
Sanitation Driver investigated Mr. Conklin's loss of the plastic lid that was apparently
previously attached to a rigid solid waste container. He reports that the lid possibly got
caught in the dumped refuse, and became detached from the container.
It is therefore the recommendation of Mr. Schultz to approve the claim in the amount of
$8.48. The Legal Department concurs with this recommendation.

Claim of Eugene Conklin
Claim and estimate can be approved as filed.
The Lead Sanitation Driver investigated Eugene Conklin's loss of a
plastic lid that was apparently previously attached to a rigid sold waste
container.. Possibly the attached lid got caught up in the dumped refuse and
came off in the hopper. Attached lids are not acceptable on hand-dumped
containers for this reason among others.
