This application may be copied or downloaded from the Iowa Homeland Security and Emergency Management website listed below. Questions concerning this application should be directed to the Iowa Homeland Security and Emergency Management Division, 515-725-3231. Applications must be submitted to Iowa Homeland Security and Emergency Management Division by close of business on April 1, 2009. Applications may be submitted by certified mail ore-mail (a reply message will be sent as receipt notification}. The recipient of these grant funds must submit a written report to Iowa Homeland Security and Emergency Management by January 1, 2010 specifying the allocation and uses of the monies received from this grant. Money awarded under this program shall be used for disaster-related costs not otherwise funded by federal or nonfederal sources and may be used for any of the following purposes: ® Nonprofit organization assistance ^ Assistance for the public purchase of land and accompanying structures if financial assistance for such purchases is not available from the Federal Emergency Management Agency or when a nonfederal match is required for a grant involved in the public purchase of land and accompanying structures ^ Assistance for the repair, replacement, or upgrade of public infrastructure damaged by the disaster including measures to assist in the mitigation of future damages ^ Assistance for the increased costs associated with the reevaluation and assessment of property due to the natural disasters occurring in 2008 ^ Small business assistance ^ Assistance for the replacement or rehabilitation of housing 5. Narrative of proposed work to be completed This grant will be issued to the Dubuaue Area Disaster Recovery Committee through the Community Foundation of Greater Dubuque to fund operations of the recently formed committee to include promotional literature and information committee expenses and to provide assistance to citizens who have unmet needs from the floods of 2008 6. Anticipated cost, to include details of major cost elements The committee will be able to expend the entire $3 942 86 as the reauests for unmet needs have exceeded that amount. A detailed final report will be able to be issued for the funds 7. Detail of governance structure to be used to administer funds for recipient, including designation of community disaster grant manager The City Council, City Manager Finance Department and Budget Director are established to oversee the finances of the City. The Community Foundation of Greater Dubuaue has established a fund for the committee The actual grant will be overseen by Thomas I Beraer Emergency Management Director for the City of Dubuaue as well as Joseph Featherston Chairperson of the Dubuaue Area Disaster Recoverv Committee 8. Timeline of milestones The committee will be meeting in the next few weeks and will expense the funds by the end of September 30t". 2009. 9. Performance measures and basis of evaluation for project Evaluation of the Grant will be conducted through the set procedures of the Dubuque Area Disaster Recoverv Committee. 10. Completion date of project, to be no later than June 30, 2010 September 30th, 2009 Certification and Reports A written report needs to be submitted to HSEMD by Jan. 1, 2010, detailing allocation of funds and uses of those funds.
