RULES AND REGULATIONS: Paid With Confirmation
1. Renters using the Masonic Temple facilities are responsible for any damages.
2. Parents are to be responsible for their children and their children's conduct at all times.
3. Room setup and decorating may be performed on the day prior m the event unless otherwise approved.
4. A maximum of five practice sessions maybe permitted prior to a performance or event requiring such unless otherwise agreed upon. All
practice sessions are to be scheduled in advance and must be commensurate with normal building operations.
5. Decorations may NOT be attached to any electrical fixtures.
6. Duct tape, tacks, pins, nails, screws or any other damaging materials may not be used to attach decorations to walls woodwork or other
painted surfaces.
7. Room fiumishings must be repositioned as they were prior to the event
8. The use of pyrotechnic or open flame displays other than the use of ceremonial candles will not be permitted. If candles are used in the
carpeted rooms, care must be taken to assure that wax is not allowed to drip onto the floor.
9. The setup of tables chairs and other owner equipment will be performed by owner personnel unless otherwise agreed to.
10. Catered food services may be used. If a food caterer is used, he! she must be made aware of the provisions of this agreement
11. The renter and/or caterer must provide servers and bus persons.ll~IPORTANT
12. When kitchen facilifies are used, they must be under the supervision of an owner designated representative.
13. The use of the building's rooms must be limited to those so stated in tins agreement
14. If the use of the coat check room is required, the renter is responsible for it's open~tion and security.
15.Organizations or other groups renting the facilities must provide a minimum of 5500,000 general liability insurance. A certificate of
liability insurance must be provided, naming the Dubuque Masonic Temple as additional insured.
16. The use of tobacco products is prohibited in the Masonic Temple. Smoking is permitted only outside the Twelfth Street door. Receptacles
must be used.
17. All gambling activities within the Masonic Temple must be licensed according to Iowa law. This includes raffies and games of chance.
18. No illegal activities may be undertaken in the Masonic Temple at any time.
19. Food or beverages are not allowed in the billiard room at any time.
20. The south exit doors are to be used only for emergencies. The stage door is to be used only when delivering or removing equipment for
the event
21. Riotous, boisterous or roughhouse activities are not permitted.
22. The use of a PINATA will not be allowed at any time.
23. The owner requires that the renter provide a uniformed law enforcement officer, to be present for this event YES ENO
24. A representative of the owner will be present during all events.
25. All rates are based on a five hour event Overtime charges will apply to excess hours or a part thereof.
ALCOHOL USE:
If the use of alcoholic beverages is requested, specific approval is required. Alcoholic beverages must be limited to canned been, wine or
champagne. Guests of the renter are not allowed to bring their own beverages.
Alcoholic beverages maybe delivered on the day prior to the event and must be removed immediately following the event
Owner personnel will not dispense or serve any alcoholic beverages.
ALCOHOL USE IS REQUESTED: YES / NO INITIAL APPROVED
RENTER AGREES TO THE FOLLOWING:
All provisions outlined herein will be adhered to.
Fifty percent of the established rental fee must be paid at the time of the scheduled event confirmation. That amount is refundable up to thirty
days prior to the event In case the event is cancelled after the 30 day period begins, one half of the confirming fee is refundable. Final
payment must be made no later than fourteen days prior to the eves.

This is a formal proposal in which we, Heather and Amanda, will describe the details of our project, A
Motion Connection.
Purpose: A Motion Connection is a fundraising benefit for a local non-profit, the Dubuque Art Center,
sponsored by Monks Kaffee Pub and Rondinelli Music/Audio.
Mission: A Motion Connection is a social gathering in which all are welcomed to support a cause through a
community-focused and acommunity-driven event. A Motion Connection strives to build a positive social
network of people that work together to empower the community. Our goal for this event is to fundraise a
minimum of $2,000 for the Dubuque Art Center, with the hope of much more!
Non-Profit Description: The local organization, which serves as A Motion Connection's beneficiary is the
Dubuque Art Center. The Dubuque Art Center is a legal entity registered in Iowa as a small corporation. Their
fiscal agent is the Community Foundation of Greater Dubuque, which is a local non-profit. The Community
Foundation of Greater Dubuque serves as the parent non-profit to the Dubuque Art Center, which is currently
working on obtaining its own independent non-profit status. Formally the Brick Oven Studios, the Dubuque Art
Center is a local community collective of artists and art supporters. With their motto being, "Community
Through Creativity," the mission statement of the Dubuque Art Center "is to stimulate and sustain artistic growth
in order to build our community." A Motion Connection wants to financially support their endeavors, projects,
and sustainability. The Dubuque Art Center is a community organization in which artists join to have a
workspace for their own creative means to then exhibit. These same artists also serve as the instructors for a
range of art courses open to the community. Along with the courses and events currently offered, the Dubuque
Art Center aims to create children's programs that are neighborhood-focused. These programs include free after
school art classes and weekend mentoring.
Description of Donation Use: The money fundraised at A Motion Connection will serve as funding for a
scholarship program to support the after school art classes and weekend mentoring.
Sponsorship Description: Monks Kaffee Pub is sponsoring A Motion Connection. Monks is a locally owned
and operated coffee shop and pub on Bluff Street. The motto of Monks is "Good coffee, good beer, and good
music." With intentional artwork showcased on the walls, Monks offers a place for the community to enjoy both
local and touring musicians.
Beer and Wine Details: Monks sponsorship includes a transfer of their dram shop and alcohol license for this
event. The transfer will occur on Saturday April 18, 2009 at Sam until Sunday April 19, 2009 at Sam. For this to
occur, the transfer paperwork must be submitted to the City Clerk's Office no later than March 23, 2009, with all

inspections completed by March 20, 2009. The transfer will then go before the City Council for approval on
Apri16, 2008. With this approval, Monks sponsorship allows alcohol to be legally served and sold at A Motion
Connection covered under their dram shop insurance. Monks Kaffee Pub will not be selling or displaying any
alcohol at their Bluff Street location on April 18~' or April 19~' due to the transfer to the Masonic Temple. We
will be selling canned beer and glasses of wine at the bar, as well as bottled water, canned pop, and cans of Red
Bull. We will be carding at the door, for this is a 21 and older event and no alcohol will be served after tam. We
will continue selling the non-alcoholic beverages until 4am. There will only be a cash bar after tam. Up until
then, patrons will have to purchase drink tickets from a separate volunteer run ticket booth and those tickets will
then be traded for beverages. The ticket booth will be closing at tam and all purchases afterward will be made
directly at the bar with cash.
Legal Issues: A Motion Connection has created strong and positive ties with the local city government. To
ensure that this event not only goes smoothly, but also is legal within all regards, we have been working with
Captain Russ Stecklein of the Dubuque Police Department, Sue Winters of the City Clerk's Office, and Assistant
Fire Marshal Mike McMahon with the Dubuque Fire Department. We have also been in communication with the
other inspectors including the City Building Inspector, City Electrical Engineer, and the Health Department
Inspector. This network of people and agencies share in the same like-minded goal as A Motion Connection - to
create and maintain safe events that sustain and empower our community. Due to the legalities involved with the
dram shop and alcohol license transfer, all city inspectors and Monks insurance agent required a walk through and
inspection of the space to ensure that all safety codes are met. Due to fire and building code issues, the capacity
of this event may not exceed 299 persons at any time. This issue will be resolved at the event by using
monitoring solutions including a front door clicker, number system, and head counts.
The Event Details: A Motion Connection will occur Saturday night, April 18, 2009 from 8pm until 4am. This
fundraising benefit will have a $10 cover at the door which will include four tickets (ticket value = $1 per ticket)
and donated food. This is a 21 and older event. For the purpose of monitoring our attendees, we will only be
allowing entrance at one door with the other doors available for emergency exit only. We will be charging
between $1 and $4 for any beverages beyond those included with the cover charge. There will be other
opportunities for donations throughout the night. This event will feature local musicians Val Voshell & Maureen
Kilgore, Nate Jenkins, David Zollo, local rapper casethejoint, and the local bands Lost Appirations and The Wild
Animals. There will also be dj's and dancing featuring DJ Bizeone, DJ Matt Michels, DJ Gazelle, DJ Shawn
Shady, and DJ Justin Deworak. Three local artists will be exhibiting at the event, which are Elizabeth Roberts,
Brian Cluff, and Matt Mikulice. The night will begin with a champagne toast at 8pm for all volunteers and
sponsors. Food will be donated by 180 Main and the Pepper Sprout and will be offered in the main lobby. This
food will already be cooked and prepared and will be covered under our temporary food license. Acoustic music
will begin at 8:30pm in the main hall, where patrons can enjoy the food and bar. Music upstairs in the Grand
Theater will begin at 8pm. The food will be offered until midnight or until it is gone, and no food will be allowed
on the second floor. The bar will serve alcohol until tam, and water, pop, and Red Bull will be available
throughout the duration of the event. The agreed upon rooms within this space will be available for the patrons'
use from 8pm until 4am. The city will be providing trash and recycling receptacles for this event. We will
require the use of the space beginning the a8ernoon/night of Friday April 17~', with all clean up and furniture
realignment completed by Sunday evening Apri120`~'.
Room Usage: The use of these rooms have been discussed with and confirmed by the Masons of the Dubuque
Masonic Temple.
Main Hall -This is where we will check id's, collect cover, and have a beverage ticket station. The food
will be available on a centralized table. The beverage bar will also be located here. There will be three
acoustic musical sets on the platform stairs in front of the museum from 8:30pm until midnight. There

Men's Parlor -This will be a lounge for people to gather and we will be bringing in two projectors and
two Wii stations that patrons will be able to purchase tickets for use.
Women's Parlor -This will be a lounge for people to gather.

Billiards Room -The room will be open for use, but no food or beverage will be allowed in this space.
Patrons will be able to purchase tickets for use of the tables.
Grand Theater -This room will serve as the main gathering room. We will have all the dj's and the
bands performing on this stage. We will be bringing in a removable floor for the preservation of the
carpet.
Upstairs Living Room -This will be a lounge for people to gather.
Coat Room -This room will be curtained off and serve as the bar. All the volunteers and sponsors will
be able to use the back for personal storage.
Museum -The museum will be open for patrons from 8pm until midnight (tentatively closing at that
time).
All available bathrooms on main and second floor -The use will be for all patrons.
Lower Level -The only use for this space will be for the storage of alcohol in the kitchen. The persons
allowed in this space will be limited only to specific volunteers.
Rooms not for use -Egyptian Room, Women's Dressing Room, Main Lodge.
We will use all of the furniture permitted. We are aware of the importance of the preservation of the building and
understand that the furnishings of the Masonic Temple are very valuable.
Other Sponsors and their Affiliations/Donations: Rondinelli Music/Audio will be donating the use of all
additional sound and lighting equipment. 180 Main and the Pepper Sprout will be donating food for the patrons.
Monks Kaffee Pub will be donating coffee for the volunteers. We are working on finalizing confirmation of
additional donations. These donations include party supplies, cleaning supplies, ticket rolls, canned pop, bottled
water, Red Bull, ice, printed materials, floor remnants for the creation of a removable floor to be used in the
Grand Theater, and pizza for our volunteers and sponsors.
Our Promise: We promise to create a safe environment that everyone is welcome to enjoy. We promise to
fundraise a substantial amount for the Dubuque Art Center. Monks Kaffee Pub will then make a donation to the
Dubuque Art Center for an amount after event expenses have been recouped. The generated donation will then be
matched by a supporting foundation to ensure the sustainability of the free after school art classes and weekend
mentoring provided by the Dubuque Art Center. We promise to adhere to all rules and regulations established
throughout our communications with the Masonic Temple Association and the local government. We promise to
work diligently and respectfully with these organizations, as we are grateful for the opportunity to build more
lasting relationships. We promise to go about this in the most appropriate fashion. We promise to keep all lines
of communication open throughout this process. Finally, we promise that A Motion Connection will make the
Dubuque community a better place.
