City Engineer Gus Psihoyos is recommending acceptance of the University Avenue
Extension -Phase II, PCC Pavement Project, as completed by Horsfield Construction,
Inc, in the final contract amount of $890,833.71, which is a 1.23% increase from the
original contract in the amount of $880,000.00.
I concur with the recommendation and respectfully request Mayor and City Council
approval.

INTRODUCTION
The enclosed resolution provides for the acceptance of the construction contract of the
University Avenue Extension -Phase II, PCC Pavement Project.
BACKGROUND
The University Avenue Extension Project was a multi-phase project coordinated
between the Iowa Department of Transportation (Iowa DOT) and the City of Dubuque to
improve the flow of traffic on US 20 and to decrease intersection congestion at US 20
and John F. Kennedy Road. Phase I provided for the construction of adouble-barrel,
12- by 12-foot concrete box culvert as well as mass grading and the construction of the
roadway embankment. Phase II provided for: the construction of a new concrete
roadway that extended University Avenue to the south to intersect with US Highway 20
across from Theisen's driveway; the installation of new traffic signals at the new US 20
and University Avenue intersection; the removal of traffic signals on US 20 at the
Midway Motor Lodge; and building demolition at 2792 University Avenue.
DISCUSSION
The University Avenue Extension -Phase II, PCC Pavement Project has been
completed by Horsfield Construction, Inc. of Peosta, Iowa, in the final contract amount
of $890,833.71, which is a 1.23% from the original contract in the amount of
$880,000.00.
RECOMMENDATION
I recommend that the City Council accept the improvements and authorize the payment
of the contract amount of $890,833.71 to Horsfield Construction, Inc.

PROJECT (IOWA DOT NO. STP-U-2100 (635)--70-31 AND AUTHORZING THE PAYMENT OF THE CONTRACT AMOUNT TO THE
CONTRACTOR
Whereas, the Public Improvement Contract for the University Avenue Extension
- Phase II, PCC Pavement Project (Iowa DOT No. STP-U-2100 (635)--70-31) (the
Project) has been completed and the City Engineer has examined the work and filed ~ a
certificate stating that the Project has been completed according to the terms of the
Public Improvement Contract and that the City Engineer recommends that the Project
be accepted; and
NOW THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF DUBUQUE, IOWA:
Section 1. The recommendation of the City Engineer is approved and the Project
is hereby accepted.
Section 2. The Finance Director is hereby directed to pay to the Contractor from
the University Avenue Extension to US 20 Capital Improvement Project (CIP 3001254),
and Surface Transportation Funds (STP) appropriations for the contract amount of
$890,883.71 less any retained percentage provided for therein as provided in Iowa
Code chapter 573, and to pay such retainage only in accordance with the provisions of
Iowa Code chapter 573.

he has inspected the University Avenue Extension -Phase II, PCC Pavement Project
(Iowa DOT No. STP-U-2100 (635)--70-31), that the Project has been performed in
compliance with the terms of the Public Improvement Contract, and that the total cost of
the completed work is $1,193,060.63.
