The City of Dubuque enthusiastically endorses the Dubuque Community Y's application
for ACHIEVE grant funding through the YMCA-USA. Dubuque was recently designated
one of the "100 Best Communities for Children" in the country. Dubuque has embraced
a collaborative model to promote community change and create a sustainable
movement to improve the quality of life for the citizens of our city. The ACHIEVE grant
will add significant momentum to this collaborative movement.
We appreciate the Dubuque Community Y's initiative in seeking these grant funds and
look forward to participating as a member of the national CHART team.

Introduction
The purpose of this memorandum is to request the City Council to approve a letter of
support in behalf of the Community Y's application for ACHIEVE Healthy Communities
Program funding.
Discussion
The Community Y is applying for the fifth consecutive year for a Pioneering Healthy
Communities Program grant. ACHIEVE (Action Communities for Health, Innovation and
Environmental changE) is a program sponsored by the national Centers for Disease
Control. This program provides funding, technical assistance and support to organiza-
tions to develop and implement health-related policy, systems and environmental
change strategies. Specific strategies include promoting physical activity, reducing
tobacco use, healthy eating and improving access to preventive healthcare.
If their application is funded, the Y will field a Community Health Action and Response
Team (CHART) which will participate in national trainings. The City will be asked to
appoint a representative to serve on this team. According to Andy Gillip, Health and
Wellness Director at the Y, their goal is to achieve designation for Dubuque as the
Healthiest City in the country, a title currently held by Burlington Vermont.
Because the submission deadline for this application was December 4, the attached
support letter has already been forwarded. The City Council is asked to approve this
indication of support by approval of the Community Y's request at this time.
