INTRODUCTION
The purpose of this memorandum is to request the City Council's authorization of a
renewal application to the U.S. Department of Housing & Urban Development (HUD) for
the administration of the Continuum of Care (CoC) Special Needs Assistance Program
(SNAP).
BACKGROUND
The CoC-SNAP provides rental assistance to homeless persons with disabilities. The
Phoenix Housing Project has been in operation in Dubuque since 2007. The projed's
goal is to provide permanent, supportive housing to homeless individuals/families with
disabilities. Agreements in place clarify duties of participants, private housing providers,
the City of Dubuque Public Housing Authority (PHA), the Sponsor Agency, East Central
Development Corporation (ECDC), and referring agencies. ECDC is the primary
contact with the client. The City of Dubuque serves as the grantee with administration,
inspedion and fiscal responsibilities.
DISCUSSION
The one-year renewal application is due to HUD by September 18, 2018. The renewal
application will assist approximately 14 households. ECDC provides case management
to participants which includes: setting goals related to housing, increasing income
and/or skills and setting personal goals in order to create an overall better quality of life.
The goals are set by the participants; the coordinator is there to assist with referrals,
skill building and for overall support. The coordinator prepares a monthly supportive
service report for each participant to keep track of services accessed by the participant,
including mental health services, substance abuse treatments and/orjob training
programs, as needed. ECDC works with apartment owners to secure suitable housing
located at scattered sites throughout the community and schedules appointments at
least monthly to assist participants with housing needs or concerns.
Inspections of these units are performed by the City, as well as conducting rent
reasonableness comparisons and rent calculations. The duration of the rental
assistance is unlimited although the goal is to provide enough support, guidance and
services so that the individual/family is able to live independently without fear of
becoming homeless again. Included in the priorities of the City of Dubuque
Consolidated Plan is to increase the housing options and related services for disabled
persons and homeless individuals/families.
ACTION
I respectfully request adoption of the resolution allowing submission of an application to
the Department of Housing and Urban Development (HUD) for the CoC-SNAP grant for
the one-year renewal application.

HOUSING AND COMMUNITY DEVELOPMENT AND THE CITY MANAGER TO
APPROVE THE APPLICATION
Whereas, the U.S. Department of Housing and Urban Development has published the
availability of the Continuum of Care grant for the Collaborative Applicants including the
Iowa Balance of the Continuum of Care; and
Whereas, the U.S. Department of Housing and Urban Development will publish the
availability of the Continuum of Care grant for the Project Applicants including the City of
Dubuque Phoenix Housing Project; and
Whereas, the City of Dubuque has administered the Continuum of Care grant since
2007; and
Whereas, the City of Dubuque proposes to continue administering the Continuum of
Care grant to assist homeless, disabled persons with permanent supportive housing; and
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
DUBUQUE IOWA:
Section 1: That the Mayor is hereby authorized and directed to submit an application
to HUD for the Continuum of Care program grant; and
Section 2: That the Director of Housing and Community Development and the City
Manager are hereby authorized to approve the application that is submitted to HUD.
Section 3: That the City Manager is hereby authorized and directed to forward said
application and resulting standard executed contract to the respective agency in a timely
fashion and as required by U. S. Department of Housing and Urban Development.

Introduction:
This memorandum requests the Certifying Officer's signature on an environmental review
record. The Mayor is the Certifying Officer for the City of Dubuque.
Background:
The City of Dubuque Housing & Community Development receives HUD Continuum of Care
funding to provide permanent supportive housing through the Phoenix Housing Special Needs
Assistance Program. HUD Continuum of Care Grants require an environmental review to
determine any impact on occupants and the neighboring environment as a result of the HUD
funded Activity.
The attached environmental review record was completed for the City's Continuum of Care
grant number IA0061 L7D011707. This is a leasing project that requires a limited scope review
for environmental conditions categorically exclusion subject to the Federal laws and authorities
cited in §58.5.
I have completed the environmental review and determined the leasing activity conducted by
Phoenix Housing Special Needs Assistance Program provides no extraordinary circumstances
that would require the completion of an Environmental Assessment, and further, the Housing &
Community Development Department's policy is to lease units requiring no mitigation of
environmental conditions. Therefore, the Phoenix Housing Special Needs Assistance program
is categorically excluded and converts to EXEMPT per section 58.34(a)(12).
Requested Action:
I respectfully request the Mayor sign the attached Tiered Environmental Review Determination
and the Environmental Review Record for Continuum of Care leasing/rental projects. Please

In accordance with HUD policy, all housing will be free of hazardous and toxic materials
that could affect the health and safety of occupants. A Housing Inspector will visit and
assess the property and include documentation on: 1. EPA Envirofacts or NEPAssist
report of nearby toxic sites and releases 2. Multifamily housing with 5+ units will
include documentation showing past use of site, including historical photographs, letter
from someone knowing sites history, Phase !Environmental Site Assessment All sites

The U.S. Department of Housing and Urban Development (HUD) has determined that
leasing and rental assistance activities in the Continuum of Care (CoC) Program that include no
maintenance, repairs, or rehabilitation to the leased or rented properties have "No Potential to Cause
Effects." as described in 36 CFR 800.3(a)(1). These leasing and rental assistance activities provide
transitional or pennanent housing for the homeless in existing scattered site buildings with no
associated physical changes to the buildings. and have no potential to cause effects on historic
properties, assuming such properties were present. Neither HUD nor the grantees participating in
the CoC Program have any further obligations for these projects under Section 106 of the National
Historic Preservation Act (Section 106) or 36 CFR Part 800. No consultation with the State Historic
Preservation Officer (SHPO), Tribal Historic Preservation Officer (THPO), or other interested
parties is required.
A copy of this memorandum in the Environmental Review Record for a qualifying
individual CoC project will document compliance with Section 106, 36 CFR Part 800, 24 CFR
Part 50, and 24 CFR Part 58 regarding historic properties.
Leasing and rental assistance with associated unit or building repairs, rehabilitation, or
new construction do not qualify for the treatment described in this memo. Likewise, other types

Introduction:
This memorandum requests the Certifying Officer's signature on an environmental review
record. The Mayor is the Certifying Officer for the City of Dubuque.
Background:
The City of Dubuque Housing & Community Development receives HUD Continuum of Care
funding to provide permanent supportive housing through the Phoenix Housing Special Needs
Assistance Program. HUD Continuum of Care Grants require an environmental review to
determine any impact on occupants and the neighboring environment as a result of the HUD
funded Activity.
The attached environmental review record was completed for the City's Continuum of Care
grant .number IA0061 L7D011808. This is a leasing project that requires a limited scope review
for environmental conditions categorically exclusion subject to the Federal laws and authorities
cited in §58.5.
I have completed the environmental review and determined the leasing activity conducted by
Phoenix Housing Special Needs Assistance Program provides no extraordinary circumstances
that would require the completion of an Environmental Assessment, and further, the Housing &
Community Development Department's policy is to lease units requiring no mitigation of
environmental conditions. Therefore, the Phoenix Housing Special Needs Assistance program
is categorically excluded and converts to EXEMPT per section 58.34(a)(12).
Requested Action:
I respectfully request the Mayor sign the attached Tiered Environmental Review Determination
and the Environmental Review Record for Continuum of Care leasing/rental projects. Please

Potential to Cause Effects" under Section 106 of the National Historic
Preservation Act and its implementing regulations 36 CFR Part 800
The U.S. Department of Housing and Urban Development (HUD) has determined that
leasing and rental assistance activities in the Continuum of Care (CoC) Program that include no
maintenance, repairs, or rehabilitation to the leased or rented properties have "No Potential to Cause
Effects," as described in 36 CFR 800.3(a)(1). These leasing and rental assistance activities provide
transitional or permanent housing for the homeless in existing scattered site buildings with no
associated physical changes to the buildings, and have no potential to cause effects on historic
properties, assuming such properties were present. Neither HUD nor the grantees participating in
the CoC Program have any further obligations for these projects under Section 106 of the National
Historic Preservation Act (Section 106) or 36 CFR Part 800. No consultation with the State Historic
Preservation Officer (SHPO), Tribal Historic Preservation Officer (THPO), or other interested
parties is required.
A copy of this memorandum in the Environmental Review Record for a qualifying
individual CoC project will document compliance with Section 106, 36 CFR Part 800, 24 CFR
Part 50, and 24 CFR Part 58 regarding historic properties.
Leasing and rental assistance with associated unit or building repairs, rehabilitation, or
new construction do not qualify for the treatment described in this memo. Likewise, other types
of projects participating in the CoC Program, including repairs, rehabilitation, and new
construction, do not qualify for the treatment described in this memo. These activities must be
fully reviewed under the Section 106 process, including consultation with a SHPO, THPO and/or
tribal authority, interested parties and the public, to aid HUD or the Responsible Entity in
making an effect determination.
