DATE: August 28, 2018
City Engineer Gus Psihoyos is recommending acceptance of the construction contract
for the 2018 Asphalt Overlay Access Ramp Project Two, as completed by Midwest
Concrete, Inc., in the final contract amount of$138,711 .39, which is an 18.9°k increase
over the original contract amount of$112,508.25. The increase was due to two change
orders, one for 559 linear feet of additional curb and gutter replacement and bedrock
removal that accounted for 13.31 °k of the overage, the other 5.59°k was for additional
curb and gutter, top soil and seeding needed for landscaping and reshaping hill sides at
the back of the walk.
I concur with the recommendation and respectfully request Mayor and City Council
approval.

DISCUSSION
The 2018 Asphalt Overlay Access Ramp Project Two was completed by Midwest
Concrete, Inc. of Peosta, lowa in the final contract amount of$138,711 .39. This is an
18.9°k increase over the original contract amount of$112,508.25. The increase was
due to two change orders, one for 559 linear feet of additional curb and gutter
replacement and bedrock removal accounted for 13.31 °k of the overage, the other

and reshaping hill sides at the back of walk. Seven storm sewer intakes required
reconstruction because of their proximity to, and the inability to construct complaint
