INTRODUCTION
Crescent Community Health Center (CCHC) is submitting a competitive continuation
grant for a FY19 Federally Qualified Health Center (FQHC) Services Area Competition
(SAC) grant.
BACKGROUND
CCHC's last federal SAC grant was 2015. Community health centers must reapply for
federal funding approximately every three years.
DISCUSSION
CCHC provides oral and medical health services to over 6,000 patients annually and is a
vital partner in the Dubuque area health care community.
RECOMMENDATION
Approve sending a letter of support for CCHC SAC grant to Health Resources and
Services Administration.
COUNCIL ACTION
Authorize the City Manager to sign and submit the letter of support.

Please accept this letter as our commitment in full support of Crescent Community Health
Center's FY19 Service Area Competition application to the US Department of Health and
Human Services/ Health Resources and Service Administration to continue to provide primary
and preventive health care services to the low-income target population throughout Dubuque
County, Iowa; and surrounding areas.
Crescent Community Health Center has been a primary safety-net health care provider for
over 12 years with a clinic located in the most underserved area of the City of Dubuque as
well as a dental clinic located at Stonehill Care Center which serves an elderly population.
Crescent Community Health Center provides our community access to adult and pediatric
primary healthcare, behavioral health, oral health, pharmacy, women's health services, and
· extensive enabling services, as well as a Wellness Center. Crescent also serves a large and
growing senior population.
The City of Dubuque values access to high quality, cost effective health care services in our
area and remains a dedicatedcpartner in continuing a strong safety-net health care delivery
system. Health equity in the continuum of health care services is very important to us.
The City of Dubuque continues to support Crescent Community Health Center's mission to
those in need through its FY19 Service Area Competition. Crescent will ensure effective
collaboration and coordination of services with the City and throughout the region to assure and
improve access to care and eliminate health disparities among the most vulnerable populations
in the service area.
