As required by the Amended and Restated Lease Agreement between the City of
Dubuque and Dubuque Racing LTD., Dubuque Racing Association President and CEO
Jesus Aviles requests City Council approval to expend money from the Depreciation
and Improvement Fund. In this case it is to repair the grease trap system at the
Q Casino at a cost of$75,000.
I concur with the recommendation and respectfully request Mayor and City Council
approval.

Issue: Settling in the Casino parking lot and Hilton front entrance
As required in the Amended and Restated lease Agreement between the City of Dubuque and
the Dubuque Racing LTD., I am formally requesting City approval to repair the grease trap
system at the Casino. The original equipment was undersized giving the ability for grease to
bypass the trap system and enter into the city sewer. This has been an ongoing conversation
with the City Engineering Department to determine the root cause and come up with an action
plan for correction. My recommendation for this expenditure is $75,000.
The source of the funding for the project is the Depreciation and Improvement Fund (D&I Fund)
which has a balance of$992,553 due to pending projects the available amount is $822,553.
Upon approval for this project the fund would have a new available amount of$747,553. The
required minimum for the D&I Fund is $250,000 per the City/DRA Lease Agreement.

Dear Jason:
Thank you for the opportunity to present this Engineering Design Services proposal for the Q Casino
Grease Interceptor Revisions Project. We appreciate your confidence in Hometown Plumbing & Heating
and look fonvard to working for you and building a successful relationship.
We respectfully propose the following:
1. Scope of Work: We will provide plumbing design services for the Grease Interceptor Revisions project. This
proposal is based on our understanding that the proposed construction cost is approximately$60,000.00.
2. Our proposed fee for engineering services as described above is$15,000.00 based on:
a. Recent reviews of the current installation.
b. One bid package for the entire scope of work.Additional bid packages requested may be cause for
additional services.
c. Attendance of two design phase meetings with members of your staff and/or the City of Dubuque. A site
visit for the verification of existing systems is part of the basic contract.
3. Bid phase services include answering contractor questions, issuing addenda and reviewing bids received.
4. Hometown Plumbing & Heating requires a minimum level of construction phase services to ensure the proper
implementation of our design documents and success of the projecYs overall goals. Construction phase
services include pre-construction meeting, shop drawing review, response to requests for information,
preparation of requests for proposals, periodic site visits to review work in progress or attend contractor
coordination meetings, and final review of the project construction.
5. Document reviews will be at approximately 50% and 95% complete. Hometown requires your comments
within five business days of releasing documents for your review for incorporating any changes into the
bidding documents.
6. Document changes after completed or partially completed work is reviewed or approved may be cause for
Additional Services.
7. Additional Services shall be provided on an hourly basis as follows:
a. ENGINEER-$175.00/hr
b. CAD TECHNICIAN -$125.00/hr

8. State submittal requirements and written response to review comments pertaining to the design are not
included in aur scope of work.
9. Preliminary estimates of the probable construction cost are included in our scope of work.
10. Technical specifications shall be provided on the drawings.
11. This proposal is based on the assumption that you will provide us with hard copies of existing architectural
and plumbing plans for use in developing our plans. Dimensions will be field verified during a design-phase
site visit.
12. The Contractor(s) are responsible for permits.
13. Reproduction of documents for Owner review shall be included as a reimbursable.
14. Reproduction of documents for bidding or canstruction purposes is not included.
15. Terms and Conditions of Services: See attached.
