SUBJECT: EMS Award Given to Fire Department Member
DATE: November 26, 2018
This past summer Fire Equipment Operator Jeff Breitbach assisted a citizen while off-
duty. Jeff was present on an EMS call involving a patient in a wheelchair. Jeff noted
that the patient did not have satisfactory accessibility into his home using the
wheelchair. Jeff returned to the home off-duty and assisted the citizen by creating a
ramp, some shelving and other items that made his home more accessible. The Fire
Department received a thank you from the patient/citizen for Jeffs actions.
EMS Supervisor Wayne Dow nominated Jeff for an annual award through the lowa
EMS Association. The Fire Department was recently informed that Jeff Breitbach was
awarded the IEMSA Career EMS Provider of the Year Award. This award was
presented to Jeff at the annual conference last week in Des Moines. Only one career
EMS provider is awarded this recognition each year.
Fire Chief Rick Steines and EMS Supervisor Wayne Dow will present Fire Equipment
Operator Jeff Breitbach the EMS Provider of the Year Award at this time.

INTRODUCTION
This letter is to formally notify you of an EMS award bestowed on a Dubuque
Firefighter.
BACKGROUND
This past summer Fire Equipment Operator Jeff Breitbach assisted a citizen
while off-duty. Jeff was present on an EMS call involving a patient in the wheelchair.
Jeff noted that the patient did not have satisfactory accessibility into his home using the
wheelchair. Jeff returned to the home off-duty and assisted the citizen by creating a

RECOMMENDATION
I recommend that Fire Equipment Operator Jeffry Breitbach be presented the
award by EMS Supervisor Wayne Dow and I at a Dubuque City Council meeting for his
outstanding show of compassion and caring as a member of the fire department.
ACTION REQUESTED
I respectfully request recognition of this award be presented to Jeff at the
December 3, 2018 city council meeting.
