I talked to the water department about hooking up to the city water at 604
eru Rd. They said there was no stop box that we could hool< up to. They said we
would have to tap the main. I talked to the previous owner and he said there was
a stop box put in already, so I contacted the water department two different
times about it and they said that there was no stop box. They did not come check
it out, so we dug th street up and tap a new one ard as we where back filling the
dirt, we found a stop box that was already there, that the water department said
; there was not one at this location. I have $7,413.08 paid out in this and I should of
I
i only had the expense from the stop box to the house. I should be reirribursed for

k the expenses because they should have had a record of a stop box being there.

'' The only expense that I SHOULD HAVE HAD was from the stop box to the building.

I should not have had to tap in to the main and replace the street an pay
ij $3,086.72 for hookup fees. I feel like the water department did not do their job by

Claimant Date of Claim Date of Loss Nature of Claim
SPT Real Estate LLC/ 01/16/19 12/03/18 Property Damage
Shawn Thomas Sr.
This is a claim in which claimant states he incurred an unnecessary expense when he
was advised by the Water Department that a stop box did not exist for 604 Peru Road to
hook up to City water. Claimant later discovered after excavating and tapping a new stop
box that one already existed. Claimant states that he should have only incurred the
expense from hooking up 604 Peru Road to the existing stop box.
This claim has been referred to Public Entity Risk Services of lowa, the agent for the lowa
Communities Assurance Pool.
