MEMORANDUM

The Community Partnership (cp2) program was implemented on August 5, 1991, to
provide financial support to new and innovative private and non-profit efforts to create a
more livable environment for community residents. Funds are allocated for the cp2
Program on a yearly basis through the CDBG Annual Plan process. A total of
$1,185,273 has been awarded since the program began. The program has leveraged
over $3.8 million in community service and accessibility projects. The Fiscal Year 2004
Annual Action Plan allocates $86,000 for the cp2 Program beginning in July 1, 2003. An
additional $48,605 has recently been reallocated in the FY 2003 Annual Plan
amendment for the FY 2004 cp2 program, creating a total of $134,658 in cp2 funding.
Twelve applications for funding were received. A review committee was established
consisting of: Mary Rose Corrigan, Public Health Specialist; Pat Prevenas, Recreation
Division Manager; Joleen Patterson, Rehabilitation Specialist, Aggie Kramer,
Community Development Specialist, Jim Giesen, a member of Community
Development Advisory Commissioner and Harry Neustadt, a member of the Community
Development Advisory Commission and Housing Commission.
The cp2 review committee evaluated each of the proposals and ranked them in
accordance with the guidelines adopted by the City Council.
The Community Development Advisory Commission is recommending approval of the
allocation of Community Partnership Program (cp2) funds for Fiscal Year 2004 as

INTRODUCTION
The purpose of this memorandum is to request the City Council's approval of the
recommendation of the Community Development Advisory Commission for allocation of
Community Partnership Program (cp2) funds. A resolution providing for the approval is
attached for the Mayor's signature.
BACKGROUND
The Community Partnership (cp2) program was implemented on August 5, 1991, to
provide financial support to new and innovative private and non-profit efforts to create a
more livable environment for community residents. Funds are allocated for the cp2
Program on a yearly basis through the CDBG Annual Plan process. A total of
$1,185,273 has been awarded since the program began. The program has leveraged
over $3.8 million in community service and accessibility projects. The Fiscal Year 2004
Annual Action Plan allocates $86,000 for the cp2 Program beginning in July 1, 2003. An
additional $48,605 has recently been reallocated in the FY 2003 Annual Plan
amendment for the FY 2004 cp2 program, creating a total of $134,658 in cp2 funding.
The Housing and Community Development Department announced the availability of
cp2 funds on February 3rd and held an informational meeting in the auditorium of the
Carnegie Stout Public Library on February 13th. Applications were accepted through
March 27th.
A review committee was established consisting of: Mary Rose Corrigan, Public Health
Specialist; Pat Prevenas, Recreation Division Manager; Joleen Patterson, Rehabilitation
Specialist, Aggie Kramer, Community Development Specialist, Jim Giesen, a member
of the Community Development Advisory Commission and Harry Neustadt, a member
of the Community Development Advisory Commission and Housing Commission.

The cp2 review committee evaluated each of the proposals and ranked them in
accordance with the guidelines adopted by the City Council. The committee's report
and a brief description of the proposals and ranking of each are attached. Also included
are sections of the applications that wilt provide you with more detailed information.
The Community Development Advisory Commission met on Wednesday Apdl 30th to
consider the cp2 applications. At that time, the Commission reviewed the review
committee's recommendations and a final recommendation to the City Council for
allocation of cp2 funds was approved.
RECOMMENDATIONS
The Commission recommends the following allocation of Community Partnership (cp2)

The recommended funding for projects is summarized below. The cp2 program requires
25% match for the initial year of funding and provides an opportunity for one additional
year of funding with an increased match of 40%.
1. The Tri State Community Health Center is recommended to be fully funded with
modifications. This project provides for rehabilitation of an existing building at 225 West
6th Street for a health center/clinic for pdmary health and dental care for Iow-income
individuals. The Committee recommended approval of the grant with the condition that
the a five year lease will need to be obtained to guarantee use of the property for the
intended use as required by CDBG regulations
2. Scenic Valley Aoencv on A(~incl is recommended for full funding to provide assistive
devices for Iow- income elderly.
3. Four Oaks Inc of Iowa is recommended for full funding. This Housing Education And
Rehabilitation Program (HEART) project would provide vocational training for one year
to six (6) at-risk students from the Four Oaks School Behavioral Classroom. The
students will renovate and rehab housing suitable for Iow-income families.
4. Proiect Concern is recommended for full funding. This project would assist low-
income elderly with obtaining free Manufacturer's Assistance Cards for prescription
discounts on brand name drugs
5. The Visitin.q Nurse Association is recommended for full funding. This project would
provide an dental oral assessment of homebound, Iow/moderate income elderly and
referral/case management for routine and emergent dental care in connection with the
existing CareNet division of the VNA.
6. Audubon Elementary School is recommended for partial funding. This project
provides for a guided summer reading program for at risk students at Audubon School.
7. Four Mounds, Openinq Doors, Central High School, Dubuque County Historical
Society, St Mark Community Center and Consumer Credit Counselin.q were not
recommended for funding.
The Commission approved the review committee's recommendations.
ACTION STEP
The action step for the City Council is to adopt the aEached resolution approving the
award of $134,605 of cp2 funds to the aforementioned organizations and authorizing
the City Manager to prepare and execute the necessary funding agreements.

RESOLUTION AUTHORIZING THE AWARD OF COMMUNITY PARTNERSHIP
PROGRAM (cp2) FUNDS FOR FISCAL YEAR 2004.
Whereas, under the provisions of Title I of the Housing and Community Development
Act of 1974, as amended, the City of Dubuque has received Community Development Block
Grant (CDBG) funds for the development and maintenance of a viable urban community
through the provision of decent housing, a suitable living environment and expanded
economic opportunities for its residents, particularly those persons of Iow and moderate
income; and
Whereas, the Community Partnership Program (cp2) was developed in the City of
Dubuque in response,~to the national goals and objectives of the CDBG program; and
Whereas, guidelines for the implementation of the Community Partnership Program
(cp2) were approved and amended by the City Council.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
DUBUQUE, IOWA:
Section 1. That the City Council hereby awards Community Partnership Program
(cp2) monies to the following organizations. The award shall consist of a grant to the
organization for the identified project and shall be for the amount noted below:

authorized and directed to disburse program funds in support of the approved projects and in
accordance with the terms and conditions established in said contracts.

success. The cp2 funds would allow another year of funding for the expansion of the program
for elderly .with an identified need for accessibility devices such as walker skis, lift chairs, hand-
