TASK ORDER #6 — AUBURN-CUSTER WATER MAIN REPLACEMENT
This TASK ORDER is issued under the PROFESSIONAL SERVICES AGREEMENT (dated June 7,
2021 agreement reference number 9290.00) between the City of Dubuque ("Client") and WHKS & Co.
("WHKS") for Services to be provided by Consultant for Client under the Auburn -Custer Streets Water
Main Replacement ("Project"), described below to this Task Order.
A. Project Information
The Client requested WHKS to provide design, details, plan set and bid documents, for the replacement
of the water main on Auburn and Custer Streets based on said marked up area of City of Dubuque's
map (Attached).
B. Scope of Services.
Project Management and Meetings
• Perform general project administrative duties including supervision and coordination of the
project team, review of project costs and billings, prepare invoices using Consultant's
standard forms, preparation of status reports, attend meetings as necessary and general
administrative activities.
• Correspondence with City of Dubuque staff to discuss project and details as needed.
2.Topographic Survey and Research of Existing Conditions
• Add water main to already develop base map for the project.
• Collect, obtain and review relevant information from the Client.
3. Preliminary and Final Design
• Prepare preliminary and final plans and specifications to show the character and scope of
work to be performed by contractors on the Project. Plans will follow the City of Dubuque
Water Main Distribution Division Standard and Specifications (2011), SUDAS Standards and
City of Dubuque Supplemental Specifications.
• Prepare opinion of probable construction cost on completed plans and specifications.
• Furnish two (2) original signed copies of the plans, specifications, and bid documents to the
Client.
• Prepare and submit DNR water main construction permit.
• Prepare SWPPP for Project.
C. Basis of Compensation:
For the services described above, the Client shall remunerate WHKS as follows:
This Task Order is for Civil Engineer work with a Not -to -Exceed Fee of $15,500.00. Expenses are
not included and will be billed at actual cost and mileage at the current published IRS rate per
mile.

All terms and conditions of the Professional Services Agreement shall continue in full force and effect.
This Task Order is accepted, and the Consultant is authorized to proceed.
