During the Fiscal Year 2004 budget process, the Mayor and City Council provided
funding for Purchase of Service agencies. Administrative Services Manager is
recommending execution of the agreements and budget amounts for the following
agencies:
Dubuque Area Labor-Management Council ($15,443)
Helping Services of Northeast Iowa [$1,964)
Operation: New View ($16,708)
Project Concern - Foster Grand Parent Program ($5,435)
Retired and Senior Volunteer Program ($7,341)

Attached for City Council approval is the Purchase of Services Agreement with the
Dubuque Area Labor-Management Council for the City's financial support for Fiscal
Year 2004.
The adopted Fiscal Year 2004 operating budget provides a payment of $15,443 to the
Dubuque Area Labor-Management Council to provide services related to the
maintenance and expansion of a cooperative labor-management resource center, the
maintenance of a speakers bureau, publication of a bimonthly newsletter,'expansion of
educational incentives and outreach, expansion of existing membership, and facilitation
of a cooperative effort between labor and management in the City of Dubuque.
It is my recommendation that the City Council approve the attached purchase of
services agreement with the Dubuque Area Labor-Management Council and authorize
the Mayor to execute the contract on behalf of the City.

PURCHASE OF SERVICES AGREEMENT
THIS AGREEMENT, entered into this 1st day of July 2003 by and between the
City of Dubuque. Iowa, herein referred to as the "City" and the Dubuque Area Labor-
Management Council, herein referred to as the "Agency".
WHEREAS, the Agency represents itself as technically and professionally
qualified to provide a forum to improve and expand labor-management cooperation in
the public and private sector within the Dubuque area; and
WHEREAS. the City has deemed it to be in the public interest to provide
financial assistance to be used to cover general operating expenses of the Agency's
program.
NOW THEREFORE, the parties hereto agree as follows:
1. Scope of Service
The Agency shall provide, within the city of Dubuque, services related to the
maintenance and expansion of a cooperative labor-management resource
center: the maintenance of a speakers bureau: publication.of a bimonthly
newsletter: expansion of educational incentives and outreach; expansion of
existing membership: and facilitation of a cooperative effort between labor and
management in the city of Dubuque.
2. Time of Performance
The services of the Agency are to commence on July 1, 2003. and terminate
June 30. 2004.
3. Compensation
The City shall pay the Agency the amount of $15,443, which amount shall
constitute full and complete compensation for its share of all services to be
rendered.
Such sum shall be payable by the City to the Agency within ten (10) days of the
sigmng of this contract, upon receipt of a requisition for such sum.
Discrimination
In carrying out the program, the Agency shall not discriminate against any
employee or applicant for employment because of race, creed, color, sex. age,
national origin, religion, or disability. The Agency shall take affirmative action to
insure that applicants for employment are employed and that employees are
treated during employment without regard to their race. creed, color, sex, age,
national origin, religion, or disability. Such action shall include but not be limited
to the following: employment, upgrading, demotion or transfer: recruitment or
recruitment advertising; layoff or termination; rates of pay or other forms of
compensation; and selection for training, including apprenticeship. The Agency
assures that all qualified applicants will receive consideration for employment
without regard to race, creed, color, sex, age, national origin, religion, or
disability.
The Agency agrees and will undertake whatever affirmative measures are
necessary so that no person shall on the grounds of race, color, religion, sex,
national origin, age, degree of handicap or disability be excluded from
participation in. be denied the benefits of. or be subjected to discrimination under
any program or activities funded in whole or in part with any of the funds made
available to the Agency under the terms of this Agreement.

Attached for City Council approval is the Purchase of Services Agreement with Helping
Services of Northeast Iowa, Inc. for Fiscal Year 2004.
The adopted Fiscal Year 2004 operating budget reflects a payment of $1,964 to
Helping Services for Northeast Iowa, Inc.. for financial assistance to cover operating
expenses of that agency. The agency has received financial assistance from the City
for the past eight years.
Helping Services of Northeast Iowa, Inc., utilizes.these funds for their Education
Services Division, which provides substance abuse prevention services [o the residents
of Dubuque. These activities include consultations, training, presentations, and early
intervention programming.
It is my recommendation that the City Council approve the attached Purchase of
Services Agreement with Helping Services of Northeast Iowa Inc.. and authorize the
Mayor to execute the contract on behalf of the City.

WHEREAS, the Agency represents itself as technically and professionally
qualified to provide training and consultation in the area of substance abuse prevention
to community task forces and schools; and
WHEREAS, the City has deemed it to be in the public interest to provide
financial assistance to be used to cover general operating expenses of the Agency's
program
NOW THEREFORE, the parties hereto agree as follows:
1. Scope of Service
The Agency shall provide, within the City of Dubuque;, educational services
related to a substance abuse prevention program for the purpose of teaching
persons to make sensible decisions for themselves :concerning the role that
substances play in their lives.
2: Time of Performance
The services of the Agency are to commence on July I 2003, and terminate
June 30, 2004.
3. Compensation
The City shall pay the Agency $1.964. This amount shall constitute full and

PURCHASE OF SERVICES AGREEMENT
THIS AGREEMENT, entered 'into this 1st day of July 2003. by and between the

Community Action Agency, herein referred to as the "Agency".
WHEREAS the Agency represents itself as technically and professionally
qualified to provide services to help Iow income and elderly citizens achieve self
sufficiency; and
WHEREAS, the City has deemed it to be in its best interest to provide financial
assistance to be used to cover general operating expenses of the Agency's programs.
NOW THEREFORE, the parties hereto agree as follows:
1. Scope of Services
?:~he Agency shall use the funds to be provided under this Agreement in the
following ways:
a) to offset operating costs so that more program monies can be allocated to
direct client services;
b) to maintain a positive cash flow to assure that "up front" monies are available
to reimbursable programs;
c) to provide for capital acquisitions for more efficient operations; and
d) to provide special programs and assistance which are not otherwise funded.
2. Time of Performance
The services of the Agency are to commence on July 1, 2003, and terminate on
June 30, 2004.
3. Compensation
The City shall pay the Agency the amount of $15,438, which amount shall
constitute full and complete compensation for its share of all services to be
rendered. Such sum shall be payable to the Agency on or after July 1st
following the signing of this agreement and upon receipt by the City of a
requisition for such sum.
Audit and Evaluation
Inasmuch as the funds to be provided the Agency by the City are public funds.
the Agency agrees that ii will maintain books of account in accordance with
generally accepted accounting standards together with necessary
documentation to support all expenditures.
Discrimination

twenty hours each week with children of special or exceptional needs as part of the
Foster Grandparent Program; and

financial assistance to be used to cover general operating expenses of the Agency's
program.

part-time staff to help place retired persons or persons 55 years of age and older as
volunteers in nonprofit public and private agencies,

THIS AGREEMENT, entered into this 1st day of July 2003, by and between the
City of Dubuque, Iowa, herein referred to as the "City" and the Retired and Senior
Volunteer Prog ram. herein referred to as "RSVP."
WHEREAS RSVP represents itself as technically and professionally q ualified to
provide meaningful volunteer work for retired persons and/or persons age 55 and over
in nonprofit public or private agencies: and
WHEREAS, the City has deemed it to be in the public interest to provide RSVP
with financial assistance to enhance its services to the elderly.
NOW THEREFORE, the parties hereto agree as follows:
1 Scope of Service
RSVP shall employ a part-time assistant for its office to assist the director in
administering a volunteer program to place retired persons or persons age 55
and over in nonprofit public or private agencies. RSVP shall ensure that the
part-time assistant is under appropriate supervision.

Substance Abuse Services Center
Attached for City Council approval is the Purchase of Services Agreement with
Substance Abuse Services Center for the City's financial support for Fiscal Year 2004.
The adopted Fiscal Year 2004 operating budget provides a payment of $12 390 to help
fund the Substance Abuse Services Center. The City's contribution funds a half-time
substance abuse counselor to work with youth in the'City who are then referred to any
of the Center's high risk youth programs. The agency, provides a number of services
including early outpatient substance abuse treatmentl~or troubled adolescents and their
parents.
It is my recommendation that the City Council approve the attached purchase of
services agreement with Substance Abuse Services Center and authorize the. Mayor to
execute the contract on behalf of the City.

WHEREAS the City has deemed it to be in the best interest to provide financial
assistance for a half-time adolescent specialist to work in the City of Dubuque.
NOW THEREFORE. the par~ies hereto agree as follows:
1. Scope of Service
The Agency shall provide an early intervention program and outpatient treatment
for troubled adolescents in the area of substance abuse within the City of
Dubuque.
2. Time of Performance
The services of the Agency are to commence on July 1. 2003, and terminate on
June 30, 2004.
3. Compensation
The City shall pay the Agency the amount of $12.390. which shall constitute full
and complete compensation from the City for all services to be rendered by the
Agency. Payment will be made in four quarterly installments of $3,097.50
beginning with the first payment on October 1. 2003.
4. Audit and Evaluation
Inasmuch as the funds to be provided the Agency by the City are public funds, the
Agency agrees that it will maintain books of account in accordance with generally
accepted accounting standards together with necessary documentation to support
all expenditures.
The Agency shall provide the City quarterly written reports detailing its activities
beginning for the quarter ending September 30: 2003. Such report shall be due
ten days after the end of each quarter. Accompanying each quarterly report shall
be copies of any studies, brochures, or other non-confidential documents, which
the Agency has produced during the relevant quarter.

This contract may be terminated by either party by giving the other party a written,
ninety (90) day notice of such termination or upon such other terms as may be
mutually acceptable.
If, through any cause, the Agency shall fail to fulfill in timely and proper manner its
obligations under this Agreement or if the Agency shall violate any of the
covenants, agreements, or stipulations of this Agreement, the City shall have the
right to terminate this Agreement by g~wng written notice to the Agency of such
termination at least five (5) days before the specified effective date of such termina-
tion.

The Agency shall not assign any interest in this Agreement and shall not transfer
any interest in the Agreement without prior written approval from the City.

The adopted Fiscal Year 2004 operating budget provides a payment of $15,443 to the
Dubuque Area Labor -Management Council to provide services related to the
maintenance and expansion of a cooperative labor-management resource center, the
maintenance of a speakers bureau, publication of a bimonthly newsletter, expansion of
educational incentives and outreach, expansion of existing membership, and facilitation
of a cooperative effort between labor and management in the City of Dubuque.
It is my recommendation that the City Council approve the attached purchase of
services agreement with the Dubuque Area Labor -Management Council and authorize
the Mayor to execute the contract on behalf of the City.

THIS AGREEMENT, entered into this 1st day of July 2003 by and between the

NOW THEREFORE, the parties hereto agree as follows:
1 Scope of Service
The Agency shall provide, within the city of Dubuque, services related to the

Helping Services for Northeast Iowa, Inc., for financial assistance to cover operating
expenses of that agency. The agency has received financial assistance from the City
for the past eight years.
Helping Services of Northeast Iowa, Inc., utilizes these funds for their Education
Services Division, which provides substance abuse prevention services to the residents
of Dubuque. These activities include consultations, training, presentations, and early
intervention programming.
It is my recommendation that the City Council approve the attached Purchase of
Services Agreement with Helping Services of Northeast Iowa, Inc., and authorize the
Mayor to execute the contract on behalf of the City.

The Agency shall provide, within the City of Dubuque, educational services
related to a substance abuse prevention program for the purpose of teaching
persons to make sensible decisions for themselves concerning the role that
substances play in their lives.
2. Time of Performance
The services of the Agency are to commence on July 1, 2003, and terminate
June 30, 2004.
3. Compensation
The City shall pay the Agency $1,964. This amount shall constitute full and
complete compensation for the City's share of all services to be rendered.

PURCHASE OF SERVICES AGREEMENT

WHEREAS, the Agency represents itself as technically and professionally
qualified to provide services to help low income and elderly citizens achieve self
sufficiency; and
WHEREAS, the City has deemed it to be in its best interest to provide financial
assistance to be used to cover general operating expenses of the Agency's programs.
NOW THEREFORE, the parties hereto agree as follows:
1 Scope of Services
The Agency shall use the funds to be provided under this Agreement in the
following ways:
a) to offset operating costs so that more program monies can be allocated to
direct client services;
b) to maintain a positive cash flow to assure that "up front" monies are available
to reimbursable programs;
c) to provide for capital acquisitions for more efficient operations; and
d) to provide special programs and assistance which are not otherwise funded.
2. Time of Performance
The services of the Agency are to commence on July 1, 2003, and terminate on
June 30, 2004.
3. Compensation
The City shall pay the Agency the amount of $.1-which amount shall
constitute full and complete compensation for its share of all services to be

rendered. Such sum shall be payable to the Agency on or after July 1st
following the signing of this agreement and upon receipt by the City of a
requisition for such sum.
4. Audit and Evaluation
Inasmuch as the funds to be provided the Agency by the City are public funds,
the Agency agrees that it will maintain books of account in accordance with
generally accepted accounting standards together with necessary
documentation to support all expenditures.
5. Discrimination
In carrying out its program, the Agency shaii not discriminate against any
employee or applicant for employment because of race, creed, color, sex, age,
national origin, religion, or disability. The Agency shall take affirmative action to
insure that applicants for employment are employed and that employees are
treated during employment without regard to their race, creed, color, sex, age,

The adopted Fiscal Year 2004 operating budget provides for the payment of $7,341 for
the Retired and Senior Volunteer Program to enable that agency to continue to retain
part-time staff to help place retired persons or persons 55 years of age and older as
volunteers in nonprofit public and private agencies.
The volunteers help in a variety of ways which include commodity distribution, providing
services to the Department of Human Services, providing help at senior citizen meal
sites, schools, hospitals, and nursing homes, and providing clerical support in offices of
nonprofit agencies and City departments.
It is my recommendation that the City Council approve the attached Purchase of
Services Agreement with RSVP and authorize the Mayor to execute the contract on
behalf of the City.

with financial assistance to enhance its services to the elderly.

1 Scope of Service
RSVP shall employ a part-time assistant for its office to assist the director in
administering a volunteer program to place retired persons or persons age 55
and over in nonprofit public or private agencies. RSVP shall ensure that the
part-time assistant is under appropriate supervision.
Time of Performance
The services of RSVP are to commence on July 1, 2003, and terminate June 30,
2004.

Attached for City Council approval is the Purchase of Services Agreement with
Substance Abuse Services Center for the City's financial support for Fiscal Year 2004.
The adopted Fiscal Year 2004 operating budget provides a payment of $12,390 to help
fund the Substance Abuse Services Center. The City's contribution funds a half-time
substance abuse counselor to work with youth in the. City who are then referred to any
of the Center's high risk youth programs. The agency provides a number of services
including early outpatient substance abuse treatment for troubled adolescents and their
parents.
It is my recommendation that the City Council approve the attached purchase of
services agreement with Substance Abuse Services Center and authorize the Mayor to
execute the contract on behalf of the City.

NOW THEREFORE, the parties hereto agree as follows:
1. Scope of Service
The Agency shall provide an early intervention program and outpatient treatment
for troubled adolescents in the area of substance abuse within the City of
Dubuque.
2. Time of Performance
The services of the Agency are to commence on July 1, 2003, and terminate on
June 30, 2004.
3. Compensation
The City shall pay the Agency the amount of $12,390, which shall constitute full
and complete compensation from the City for all services to be rendered by the
Agency. Payment will be made in four quarterly installments of $3,097.50,
beginning with the first payment on October 1, 2003.
4. Audit and Evaluation
Inasmuch as the funds to be provided the Agency by the City are public funds, the
Agency agrees that it will maintain books of account in accordance with generally
accepted accounting standards together with necessary documentation to support
all expenditures.
The Agency shall provide the City quarterly written reports detailing its activities
beginning for the quarter ending September 30, 2003. Such report shall be due
ten days after the end of each quarter. Accompanying each quarterly report shall
be copies of any studies, brochures, or other non -confidential documents, which
the Agency has produced during the relevant quarter.

mutua!!y acceptable.
If, through any cause, the Agency shall fail to fulfill in timely and proper manner its
obligations under this Agreement or if the Agency shall violate any of the
covenants, agreements, or stipulations of this Agreement, the City shall have the
right to terminate this Agreement by giving written notice to the Agency of such
termination at least five (5) days before the specified effective date of such termina-
tion.

8. Assignability
The Agency shall not assign any interest in this Agreement and shall not transfer
any interest in the Agreement without prior written approval from the City.
