This is a claim in which the claimant alleges that as she was walking in front of 86 Main
Street, the heel of her shoe caught in a hole in the street, causing her to fall forward,
resulting in injuries to her face, mouth, hip, lef[ wrist and knees.
This claim has been referred to Public Entity Risk Services of Iowa, the agent for the Iowa
Communities Assurance Pool.
