Attached is the second "Budget Increase Request for Continued Environmental
Consulting Services — Mercury Release" between the City of Dubuque and MWH
Americas, Inc. (MWH) for services related to remediation of the mercury spill at the
Water Pollution Control Plant.
MHW is anticipating that the attached time - and - materials fee cost increase of $48,700
will be the final contract amendment. The total of $126,200 (original contract of $20,000,
first approved increase of $57,500, and the second increase of $48,700) will be funded
through the Waste Pollution Control Plant Facility Study Capital Improvement Project
(CIP 7101837).
Upon execution of the noted document, please return to the Engineering Department for
further processing.

Dear Mr. Brown:
MWH Americas, Inc. (MWH) appreciates the opportunity to submit this proposal to provide
continuing environmental engineering assistance to the City of Dubuque, Iowa (City) relating to
the mercury spill at the City's water pollution control plant site. This proposal is intended to
include oversight, verification sampling, and reporting activities for the remediation of impacted
trickling filter rock. Activities anticipated for this request include the following:
• MWH personnel will be on site periodically to oversee the field screening and
impacted rock treatment activities. It is assumed MWH personnel will be on site
an average of three days per week for up to six weeks.
• Continue to coordinate with the remediation contractor, North Shore
Environmental Construction, Inc. MWH will relay analytical results, review field
screening data, assist with disposal characterization, and provide general
support for scheduling and planning.
• Sampling and analysis of up to 80 samples of rock and sediment from the
trickling filters for verification of the field screening or chemical treatment. The
samples will be analyzed for total mercury. It is assumed up to 20 percent of the
samples will also be analyzed for mercury using the Toxicity Characteristic
Leaching Procedure (TCLP).
• Sampling and analysis of up to six water samples collected from the trickling filter
basins for disposal characterization. The samples will be analyzed for total
mercury with low level reporting limits (approximately one nanogram per liter; i.e.,
one part per trillion).
• Attending one meeting at the site to discuss progress and further develop
remedial plans.

It is proposed a budget not to exceed $48,700 be established for these services. Work will be
conducted on a time - and - materials basis in accordance with the October 18, 2010 Consulting
Services Agreement (CSA) and MWH's current "Fee Schedule," attached.
If you concur with the scope of services and associated budget, please sign both copies of this
proposal and return one copy as our authorization to proceed. If you have any questions please

• Associated project expenses, including telecommunications (equipment, local and long distance charges),
facsimile, computer and word processing equipment, first class postage, network charges, and in -house
reproduction and printing of plans, reports, and other documents (black & white, up to 11x17 inches, up to
5 copies) will be invoiced at $9.75 per total labor hour, in lieu of detailed invoicing of the specified items.
• A surcharge of fifty (50) percent will be added to hourly rates for expert witness testimony, participation,
preparation time, hearings, depositions, etc.
• Rates for Computer -Aided Design and Drafting equipment, when required, will be invoiced at the rate of $15.00
per hour.
• A surcharge for Contractors Pollution Liability Insurance coverage on Remediation /Construction activities will be
negotiated for each contract where required.
• The above rates are subject to change January 1 of each year.
• A service fee will be added to the unpaid balance after 30 days at the rate of eighteen (18) percent per year.
