CLAIM AGAINST THE CITY OF DUBUQUE, IOWA
This written report constitutes your claim against the City of Dubuque, Iowa. You should complete this
form in full and attach any additional information that supports your claim. The claim must be filed with
the City Clerk at City Hall, 50 West 13th St., Dubuque, IA 52001. It will then be referred to the
appropriate department for investigation and to the City Attorney's Office. Once that investigation is
completed, a report and recommendation will be submitted to the City Council. You will be provided
with a copy of that report and recommendation. The final decision on all claims is made by the City
Council. No employee of the City of Dubuque has the authority to make any representation to you as to
whether your claim will or will not be paid.

4. Date of Incident: February 10 was when I noticed the damage.
5. Time of Incident: I became aware of the damage around 12:30PM on February 10` I got a message
from the city to move my car for snow removal. Upon getting to my car to move it I noticed it had been
hit by a snowplow causing damage.
6. Location of Incident (Be specific): The incident occurred on the side of the street directly in front of
the apartment building with the address of : 260 West 17 St.
7. Describe the accident or occurrence that caused injury or damage. (Give full details upon which you
base your claim. If a City employee was involved, give the employee's name.) My car was parked on the
street and a snow plowed scraped along the left side of my car. I got an estimate from Abra and they
confirmed that the damage was done by a snow plow.
8. What were weather conditions like? The weather was good. There had been some snow in the
previous week.

from Abra.
13. What other damages do you claim, if any? I do not claim any damages other than car repairs.

15. What amount do you claim from the City of Dubuque? $1,725.46 which was the estimated costs of
repair.
16. Why do you claim the City of Dubuque is responsible? One of the cities snow plows hit my legally
parked car.
