I am attaching Change Orders numbered 003 — PS (Parking Structure) and 003 — FS (Fire
Station) for you review and signature.
Change Order 003 — FS reflects a contract addition in the amount of $ 6141.00. This addition
is for the cement block grinding and painting of the north interior wall of the old fire station
were the previous overhead door tracks were removed. In addition, this change order also
reflects a cost addition in the application of the roofing materials to the flexcore roof.
The second request is for Change Order 003 — PS. This change order has a zero direct cost
on the construction of the ramp. Due to exceptionally cold weather and snow in December,
JP Cullen is requesting as time extension to the contract of 7 (seven) days. They have
attached weather documentation from NOAA and this has been discussed at our weekly
construction meetings.
Including these change orders, the total change order amount for the Fire Station Expansion
is $ 23,652 and a total change order amount for the parking ramp of $ 39,037.00. The total
contract for the project was $ 7,712,379.00 so I believe this reflects well on the design and
construction team on the project.

On January 11, 2011, JPC, the City, Straka Johnson, Windco and Tremco met to
resolve the issue with the roof fastening system required for the project. There was a
conflict between the type of fastening system that could be used, mechanical or
adhesive, on the substrate for this project, which is concrete. It was determined at this
meeting that a 3 -way split would occur between Wiindco, Tremco and the City.
This quote is for the portion that the City has agreed to pay.
