This letter is to express the appreciation of the Visiting Nurse Association for approval of
the FY2005 Co=unity Partnership Grant for the VNA Elderly Home-based Oral Healtb
Program.
Since this will be the second year of this program, we are very pleased to be able to
continue this partnership with the City to provide oral health screening and services to
this high-risk population. This program is meeting a real need for the homebound elderly
residents of Dubuque.
By funding this program, the City is making it possible for VNA dental staff to continue
our presence in these homes. The VNA staff and these elderly clients are very gratefuL
