Personal Injury
This is a claim in which the claimant alleges that as his 6-year old son was attempting to
enter the Five Flags facility on the northeast side of the building on 5th Street, the door
slammed shut, crushing the 5th digit on his son's left hand.
This claim has been referred to Public Entity Risk Services of Iowa, the agent for the Iowa
Communities Assurance Pool.
