Van Milligen, City Manager SUBJECT: Four Oaks Community Partnership (cp2) Grant - Subcontract Amendment for Purchase of Services with Four Mounds Foundation Housing and Community Development Department Director David Harris recommends City Council approval of an amendment to the subcontract for services for the Four Oaks Housing Education and Rehabilitation Training (HEART) Program Community Partnership (cp2) grant. The HEART Program serves Special Education students attending Four Oaks interim or therapeutic classrooms who have elected a vocational education track rather than a full time academic classroom curriculum. HEART Program students attend school one-half day, and spend the balance of the school day together on a job site working to rehab housing for lower income occupants. Four Oaks entered into a subcontract with Four Mounds for a carpentry tradesperson to work with the students in this project. At this time, Four Mounds is requesting an amendment to the existing subcontract to extend the carpentry training services to June 30, 2005. The cp2 grant requires the written consent of the City prior to the execution of the amendment to the subcontract with Four Mounds. David Ha s, Housing and Community Development Department SUBJECT: Four Oaks Community Partnership (cp2) Grant- Subcontract Amendment for Purchase of Services with Four Mounds Foundation INTRODUCTION The purpose of this memorandum is to request the City Council's approval of an amendment to the subcontract for services for the Four Oaks Housing Education and Rehabilitation Training (HEART) Program Community Partnership (cp2) grant. A resolution providing for the approval is attached for the Mayor's signature. BACKGROUND In FY 2004, Four Oaks received a $36,000 Community Partnership (cp2) grant for the Housing Education and Rehabilitation Training (HEART) Program. The program is a collaboration between Four Oaks, Dubuque Community Schools, City of Dubuque Housing Department, Juvenile Court Services, the Department of Human Services and John Gronen Properties. A $30,000 cp2 grant has been awarded for FY 2005. The HEART Program serves special education students attending Four Oaks interim or therapeutic classrooms who have elected a vocational education track rather than a full time academic classroom curriculum. HEART Program students attend school one-half day, and spend the balance of the school day together on a job site working to rehab housing for lower income occupants. Four Oaks entered into a sub-contract with Four Mounds for a carpentry tradesperson to work with the students in this project. This tradesperson schedules the studenfs work on their current rehab project, secures materials as necessary, sets up daily work, and instructs and supervises students on the job. The Council approved this subcontract on December 15, 2004 by Resolution 505-2003. At this time, Four Mounds is requesting an amendment to the existing subcontract to extend the carpentry training services to June 30 2005 for the $30,000 FY 2005 cp2 grant amount, in addition to the balance of funds from the FY 04 grant. This would allow the continuation of the same carpentry tradesperson to assist with the HEART program for FY 2005. The cp2 grant requires the written consent of the City prior to the execution of the amendment to the subcontract with Four Mounds. Four Oaks will continue to be responsible for all other obligations of the grant. ACTION STEP The action step for the City Council is to adopt the attached resolution approving the amendment to the subcontract between Four Oaks and Four Mounds for the Community Partnership (cp2) grant. Whereas, under the provisions of Title I of the Housing and Community Development Act of 1974, as amended, the City of Dubuque has received Community Development Block Grant (CDBG) funds; and Whereas, A Community Partnership (cp2) grant was awarded to Four Oaks, Inc. in the amount of $36,000 for Fiscal Year 2004 and $30,000 for Fiscal Year 2005 by the City of Dubuque; and Whereas, the City Council approved on December 15, 2003 by Resolution No. 505- 2003 the subcontract between Four Oaks, Inc. and the Four Mounds Foundation for the direct delivery of carpentry trades training services under the FY 2004 cp2 grant; and Whereas, Four Oaks, Inc. has selected to amend the subcontract with the Four Mounds Foundation to extend the subcontract to June 30, 2005 to provide for the direct delivery of carpentry trades training services under the FY 2005 cp2 grant. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF DUBUQUE, IOWA: Section 1. That the City Council hereby authorizes Four Oaks Inc to amend the subcontract with the Four Mounds Foundation for delivery of services for the Housing Education and Rehabilitation Training Program (HEART) Community Partnership (cp2) grants as stated in the attached Amendment to Subcontract for Performance Services. Tenns of Subcontract. The tenn of the subcontract shall be a period of not exceeding twelve (12) calendar months beginning on July 1, 2004 and terminating no later than June 30,2005, subject to prior tennination as provided under Section 8 hereof This Amendment also changes Section 4., Reimbursement of Services, of the original "Subcontract for Perfonnallce of Service", entered into between Four Oaks, Inc. and Four Mounds Foundation, signed by both parties on January 7'h, 2004, to read as: 4. Reimbursement of Services. Four Mounds shall be reimbursed by Four Oaks ftom funds paid by the Grant Recipient for services perfonned hereunder and expenses incurred in the perfonnance of such services in an amount not to exceed $30,000 and the balance of the FY04 Heart Project Grant, if any, as of June 30, 2004, during the tenn hereof Requests for reimbursement shall be submitted by Four Mounds and paid to such maximum reimbursement amount in the manner and under the tenns prescribed in the Agreement, including the Standard Requirement, and conditioned upon compliance by Four Mounds with the tenns thereof and the tenns of this Subcontract.
