Commission:
Attached you will find my resume that outlines my past education and employment
experiences. I have a great deal of knowledge of similar type operations, that being hotel
environments, in multiple cities to broaden my approach to this particular facility. I have
been trained in and have been operating these businesses in regards to driving revenues,
reducing expenses, loss prevention, employee and guest relations, and community
involvement. As an employee ofK.inseth Hospitality Company, we were in the RFP
Process for the management of the Five Flags Center. I also serve on the board of the
Convention and Visitors Bureau in Dubuque,
What contributions do you feel you can make to this advisory board?
Input on local events and ways to drive business into the Five Flags Center. Also, being
the General Manager of the adjacent Holiday Inn, I feel that I can bring on some ways in
which our businesses can work together on projects to bring about greater financial and
operational gains in the long ron. I have an incredible understanding of budgets and
numbers and the ways and means to achieve certain goals. I also have a great
understanding of the Dubuque market.

Holiday Inn Dubuque/Galena-Dubuque, IA
Responsibilities include the direct management of all operations of a 193-room
full service, upper mid-scale hotel in downtown Dubuque, IA. I directly manage
a staff of 12 managers, 3 supervisors, and over 80 hourly associates. The
mcilities include 4 food and beverage outlets, indoor pool, fitness center,
concierge floor, business center, game room, and over 9,000 square feet of
meeting and banquet facilities with combined annual revenues of 4,2 million
dollars. Responsible for annual budgeting, marketing plans, capital
improvement and renovation management and planning, service and quality
improvement processes, yield management, payables and receivables, and the
direction of the hotel ,ales team. In April of2002, we were ranked second to
last in the state of Iowa in Guest Service Scores and revenues had deteriorated
drastically. In April of2004, we were ranked #3 in the state ofIowa in Guest
SeTVice and will have the best year financially in 13 years since the property
opened,
Janus Hotels and Resorts
General Manager
Holiday Inn Ocean Beach Resort-Pompano Beach, FL
Responsibilities included the direct management of all operations of a 135-room
hotel and resort. I directed a team of 10 managers and 65 hourly associates.
The hotel was a full service resort. The mcilities included 3 food and beverage
outlet" boating marina, 2 outdoor pools, miniature golf, game room, gift shop,
outdoor-amusement facilities, beach rentals, and 3,500 square feet of meeting
and banquet facilities with combined annual revenues of 3.8 million dollars.
Responsible for annual budgeting preparation, marketing plans, directing sales
staff, and maintenance of service and product standards. While at the hotel, we
increased service scores 8%, decreased food and beverage co,ts by 3.3% and
2.4% respectively, and increased rev par by 5.4%. During my tenure with Janus
Hotels and Resorts, I also took on additional responsibilities, which include new
property takeovers, troubleshooting, and renovations for new acquisitions of the
company,

housekeeping department, and shuttle service of a I 35-room full service hotel. I
directly supervised a team of2 manager' and 30 hourly associates. I was
responsible for budgeting within the rooms division, directing a team for yield
management, and working in depth to constantly upgrade the levels of guest
satisfaction at the property. I worked with a team that raised guest satisfaction
scores by 10%, lowered our quality failure index from 192 to 90, and lowered
service and quality related guest relations complaint calls from Al per thousand
rooms sold to 0_00 between April OO-April OJ. While with Meyer-Jabara
Hotels, I championed several committees between our other 30 properties, and
wa, involved in traveling to train new managers within the company.

Guest Service Manager
Holiday InnlHoliday Inn Express-Sandusky, OH
Responsible for complete supervision of fiont desk, night audit, and bell staff for
2 properties. The Holiday Inn was a full service "Holidome" with 175 guest
rooms, two food and beverage outlet" two swimming pools, interactive game
room, IS-hole miniature golf, health club, and 11,000 square feet of meeting and
banquet space. The Holiday Inn Express was a 40-room limited service
property located adjacent to the full service hotel. Responsibilities included
maintaining rates and inventories, revenue management, upgrading service
levels, in'pecting gue't rooms to en,ure quality of product, and interacting
directly with guests to learn more about their expectations.

Sec. 13-15. Programs, projects and services,
conformity to legal and policy re-
strictions.
AH programs, projects and services to be devel-
oped by the commission must conform to the pol-
icies of the city and to applicable local, state and
federal laws, rules and regulations.

Sec. 13-16. Solicitation and acceptance of do-
nations and gifts.
The commission may solicit and otherwise ac-
cept donations and gifts of money, personal prop-
erty and personal services. All such monies and
personal property as received shaH become the
property of the city for the general use for the
support of the civic center and its programs un-
less such items have been designated by the do-
nors for specific purposes,

Sec. 13-17, Membership, appointment.
(a) The commission shaH be comprised of a
membership of five (5) persons, whc> shall be ap-
pointed by the Dubuque City Council for terms of
two (2) years. Two (2) of the initial appointments
shaH be for a term of one year, three (3) of such
initial appointments shaH be for a term of two (2)
years.
(b) Upon completion of their term of office, mem-
bers of the commission shaH continue to serve in
their full capacity until a successor has been duly
appointed,
(c) Membership on the commission shall be lim-
ited to persons who are residents of the city.
Id) On and after July 18, 1983, the terms of
members of the commission shaH be for three (3)
years. Terms of those members originaHy sched-
uled to expire June 29, 1984, shall be extended to
expire June 29,1985, and terms of those members
originaHy scheduled to expire June 29,1985, shaH
be extended to expire June 29, 1986. Thereafter
all terms shaH be for three (3) years.

A vacancy caused by death, resignation or oth-
erwise on the commission shall be promptly filled
by the Dubuque City Council for the unexpired
term of office.

Sec. 13-19. Attendance at meetings, fre-
quency required; record of atten-
dance.
(a) Members of the commission shaH attend at
least two-thirds (%) of aH regularly scheduled and
held meetings within any twelve-month period. If
any member does not attend such prescribed
number of meetings, it shaH constitute grounds
for the commission to recommend to the Dubuque
City Council that said member be replaced.
(b) The attendance of aH commission members
shaH be entered upon the minutes of aH meetings
by the secretary.

HOUSING REGULATIONS
to provide any funds for any of the purposes of
this Article. (Ord. 52-92, § 1, 7-20-1992)
Sec. 26-36,
Housing Commission Trust
Fund Advisory Committee:
(a) A Housing Trust Fund Advisory Commit-
tee of the Housing Commission shall be estab-
lished to recommend policy direction and over-
sight to the administration and operation of the
Housing Trust Fund by: 1) soliciting funding; 2)
developing by-laws to govern the Committee's
operations; and 3) developing policies and
guidelines for acquisition and distribution of
funds. Such solicitations, by-laws, policies and
guidelines shall be approved by the Housing
Commission and submitted to the City Council
for its approval.
(b) The Housing Trust Fund Advisory Com-
mittee shall coordinate an effective mechanism
for public participation in the operation of the
Trust Fund, recommend projects to the Housing
Commission for funding approval and engage in
activities providing for the efficient use of the
Trust Fund.
(c) The Housing Trust Fund Advisory Com-
mittee shall have five (5) members appointed by
the City Council to serve for three (3) year
terms. Three (3) of the Committee members
shall be members of the Housing Commission
and two (2) shall be members of the public. One
of the initially appointed Committee members
shall serve a term of one year and two (2) of the
initially appointed Committee members shall
serve terms of two (2) years. The chair of the
Trust Fund Advisory Committee shall be ap-
pointed by the Housing Commission.
The two (2) members of the public shall be
selected specifically on the basis of experience
in real estate development, banking, finance,
accounting, funds management or other related
areas of expertise.
(d) The Housing Trust Fund Advisory Com-
mittee shall hold at least six (6) meetings per
year. The Housing Commission may adopt rules
and regulations it deems necessary to govern

the Committee's organizational procedures and
which do not conflict with this Article or other
resolutions and/or decisions of the City Council.
(e) All administrative, personnel, accounting,
budgetary and procurement policies and prac-
tices of the City shall govern the Housing Trust
Fund Advisory Committee in all its operations

Code-
Except as hereinafter added to, deleted, modi-
fied or amended, there is hereby adopted by
reference as the mechanical code of the city that
certain mechanical code known as the "uniform
mechanical code", 2000 edition, including the
2000 uniform mechanical code appendices as
prepared and edited by the International Asso-
ciation of Plumbing and Mechanical Officials,
Walnut, California, and the provisions of such
mechanical code shall be controlling for the
installation, maintenance and use of heating,
ventilating, cooling, refrigeration systems, in-
cinerators and other miscellaneous heat produc-
ing appliances within the corporate limits of the
city and shall be known as the "Dubuque me-
chanical code". A copy of the uniform mechani-
cal code, 2000 edition, as adopted, shall be on
file in the office of the city clerk for public in-
spection. (1976 Code, § 19-1; Ord. 45-90, § I,
5-21-1990; Ord. 101-99, § I, 11-15-1999; Ord.
47-02, § I, 6-3-2002)
Sec. 24-2, Uniform Mechanical
Amendments:

The code adopted by Section 24-1 of this
Chapter is hereby amended as provided in this
Section:
Sec. 203. Section 203, "Board of Appeals" is
hereby amended by repealing such section and
replacing such section with a new section in
lieu thereof as follows:
Sec. 203. Mechanical Code Board.
(a) Board established. In order to determine
the suitability of alternative materials and
methods of installation, to provide for reason-
able interpretations of the mechanical code, to
serve as an appeal body for the decisions of the
building official, to advise the city council on all
heating, ventilating, air conditioning and refrig-
eration regulations and procedures, and to pro-
vide for examining applicants for certification of
competency and issuing certificates of compe-
tency and registration, there is hereby created
the mechanical code board.

§ 24-2
(b) Membership. The mechanical code board
shall consist of seven (7) members appointed by
the city council. A member shall have a back-
ground or experience in sheet metal contract-
ing, hot water and steam-fitting contracting,
hot water and steam-fitting work, refrigeration
contracting, refrigeration work, or mechanical
engineering.
(c) Term of office. The term of office of all
board members shall be three (3) years.
(d) Rules and regulations, The board shall
make such rules and prescribe such procedures
as may be reasonably necessary for its opera-
tion, except that such rules shall include the
following:
(1) The chairperson of the board shall be a
member of the board elected annually by
a majority of the board.
(2) The building official, or the building
official's designee, shall be secretary of
the board. The secretary shall arrange
for meetings and prepare and keep such
minutes and records and perform such
other clerical work as the board may
direct.
(3) Four (4) members shall constitute a quo-
rum. The concurring vote of four (4)
members of the board shall be necessary
to pass any motion,
(4) Upon completion of the term of office,
members of the mechanical code board
shall continue to serve in their full ca-
pacity until their successor has been duly
appointed.
(5) The mechanical code board shall meet at
least once every three (3) months upon
call of the chairperson, secretary or city
manager. Members of the mechanical
code board shall attend at least two-
thirds (2/3) of all scheduled meetings
within a twelve-month period. If any
member does not attend such prescribed
number of meetings, it shall constitute
grounds for the board to recommend to

the city council that said member be
replaced.
(6) The attendance of all members shall be
entered on the minutes by the secretary.
(7) All meetings shall be held in confor-
mance with the provisions of the Iowa
Open Meetings Law.
(8) The mechanical code board shall file with
the city council a copy of the minutes of
each meeting of the board within ten (10)
working days after such meeting.
(9) All administrative, personnel, account-
ing, budgetary, and procedural policies of
the city shall govern the mechanical code
board in all of its operations,
(10) Any person who is aggrieved by a deci-
sion of the building official or from any
requirements resulting from the enforce-
ment of the mechanical code may appeal
from such decision to the mechanical
code board, and said board shall serve as
an appeal board. In case the aggrieved
party is a board member, said member
shall be disqualified as a member of the
board acting as an appeal board until the
person aggrieved has been heard and a
decision rendered.
The appeal shall be made by the person
aggrieved, giving written notice of such
appeal to the building official at the
building official's office within seven (7)
days after becoming aggrieved. The me-
chanical code board sitting as an appeal
board shall meet within seven (7) days
after receiving such notice and shall
render a decision within five (5) days
thereafter. AI1y interested party, includ-
ing the building official, may have the
right to present such party's case to the
appeal board, whose decision shall be
final unless appealed to the district court
as provided by law.
Sec. 203. Section 203 is hereby amended

Welder:
Section 203-Approved Qualified Welder-A
person who specializes in welding of pipes and
holds a valid certificate of competency from a
recognized testing laboratory, based on the
requirements of the A.S.M.E. Boiler and Pres-
sure Vessels Code, Section IX or as approved by
the administrative authority's testing proce-
dures.

Sec. 204. Violations and Penalties.
It shall be unlawful for any person, firm or
corporation within the jurisdictional limits of
the city to erect, cause or continue any attrac-
tive, common or public nuisance declared by
any provision of this code or ordinance or any
code or ordinance herein adopted by reference;
or to erect, construct, enlarge, alter, repair,
move, improve, remove, convert, demolish,
equip, use, or maintain any mechanical systems
or equipment, or any portion or combination
thereof, or cause or permit the same to be done,
contrary to or in violation of any of the provi-
sions of this code or any code or ordinance here-
in adopted by reference; or to fail or neglect by
omission to comply with any of the provisions of
this code or ordinance or any code or ordinance
herein adopted by reference.

The doing of any act, or the omission of any
act, declared to be unlawful by this code or
ordinance, or any code or ordinance herein
adopted by reference, is declared to be a mis-
demeanor, and each said act shall be deemed a
separate offense for each and every day or por-
tion thereof during which such unlawful act is
continued or permitted and, upon conviction,
shall be punishable by a fine in a sum not ex-
ceeding one hundred dollars ($100.00) or by
imprisonment not to exceed thirty (30) days or
by both such fine and imprisonment. The penal-
ty herein provided shall be cumulative with and

(a) 'l'he Housing Commission shall be com-
prised of a membership of ten (10) persons.
(b) Current commissioners whose terms ex-
pire August 17, 1993, shall continue to serve
until such time, or until a successor is duly
appointed, Three (3) commissioners sha1l be
appointed for al1 initial term of two (2) years,
and three (3) commissioners shall be appointed
for an inUial term of three (3) years. An suhse-

(a) The Housing Commission shaU hold at
