This is a claim in which the claimant states that a City of Dubuque Keyline bus backed
into the canopy at the new facility located at 3395 Kennedy Circle. Claimant states that in
addition to the canopy being damaged, the siding at the entrance was also dented and
cracked.
According to the report of Mark Munson, Transit Manager, a Project Concern Minibus
operator backed into the canopy at the Area Residential Care Facility on December 8,
2004.
Mr. Munson inspected the damage to the canopy and reviewed the submitted estimate.
It is the recommendation of Mark Munson to approve this claim for the amount of the
estimate of $585.00. The Legal Department concurs with this recommendation.

On December 8, 2004, Project Concern Minibus operator Bill Greenwood backed
a KeyLine Minibus into the canopy at the new ARC facility at 3395 Kennedy
Circle. I have inspected the damage and the estimate provided by ARC from
Conlon Construction and recommend payment in the amount of $585.00 for said
damages.
Please advise should you require further in this regard. Thank you.
