The claimant has agreed to participate in the Sanitary Sewer
Backup Assistauce Program He has signed a Release of All Claims
and has been sent a form for a stopvalve rebate in the amount of
$500 towards the purchase and installation of a backflow stopvalve.
As part of the program, the participant is also entitled to
$100 toward cleanup expenses and the purchase of a sewer backup
damage rider on a homeowner's insurance policy.
