The Department of Housing and Urban Development is pleased to inform you that the City of Dubuque
(2004-FSS8-IA087-53 14) has been selected to receive funding for the FY 2004 Housing Choice Voucher
Family Self Sufficiency (FSS) program to support I Homeownership renewal position(s) [$25,707] and I
regular renewal position(s) [$40,456]. The total amount of funding reserved is $66,163. An amendment to the
Consolidated Annual Contributions Contract (ACC) obligating these funds will be mailed separately by HUD's
Financial Management Center (FMC). The ACC funding increment is for one year.
By accepting this award you must comply with the requirement set forth in Section III (C)(2)(d), page
26943 of the FY 2004 Super Notice of Funding Availability (SuperNOFA) dated May 14,2004 and submit to
your local HUD Field Office a copy of your code of conduct and descri be the methods you will use to ensure
that all officers, employees, and agents of your organization are aware of your code of conduct.
