This is a claim in which the claimant alleges that a City water main break which occurred
on January 23, 2005 near 2367 West Street caused water damage to claimant's home
located at 2480 Mineral Street.
This claim has been referred to Public Entity Risk Services of Iowa, the agent for the Iowa
Communities Assurance Pool.
