The Iowa Department of Human Rights, Division of Community Action Agencies is
making available $120,000 to fund four (4) HAWK-I Outreach pilot projects by
Community Action Agencies within the state at $30,000 each for the fiscal year 2001.
The four (4) agencies selected will be chosen from the eighteen (18) CAA's in the state
on a competitive Request For Proposal (RFP) basis. The purpose of each pilot is to
implement strategies to increase enrollment in the state's childrens' health insurance
program.
As you know, Operation: New View Community Action Agency has been deeply
involved since day one in HAWK-I outreach activities through collaboration with our
other community partners and stakeholders. We plan to submit a strong grant
application and have our agency and service area (Dubuque, Delaware and Jackson
counties) selected as one of the four pilot areas.
With this in mind, we are seeking your help to secure this funding for our area. We
would appreciate a letter from you supporting our application for this grant. The grants
will be scored based on the strength of three factors: (1) The agency's organizational
capacity to increase HAWK-I applications and enrollment; (2) The agency's capacity to
utilize and develop innovative community partnerships and outreach strategies to
achieve enrollments; and (3) The agency's capacity to effectively measure and report
results. Hopefully in your letter you can address these factors in light of your
experience in working with us in the HAWK-I outreach efforts and giving specific
examples of our collaborative work. Your letter should be dated and addressed to the
"HAWK-I Outreach Pilot Selection Committee."
We would appreciate receiving your letter of support by Tuesday, July 11 th so it can be

Proposal for Innovative Outreach for Healthy and Weft Klds- Iowa (HAWK-I) Program
Iowa Division for Community Action Agencies
I. Objectives
Increase HAWK-I enrollment in each county by goal established by communities and DHS
Identify and report best practices
Recommend strategies to streamline program and reduce barriers to enrollment
II. Background
Core mission of CM's Is to alleviate the affects of poverty on Iowa families.
Provide services to over 100,000 low-income Iowa families each year.
Represent a service delivery infrastructlJre that indudes outreach offices in every county of the state.
CM's intelJrate and coordinate efforts at the community level that include education, health, human services, workforce
developmen~ child care, and child nutrition.
Strong record of 'MJ~ing with other stlte, federal, local, privata sector, and community partners.
Rely on an extensive network of volunteers that support the activities of the agencies.
III. Strategy
The DivIsion, in cooperation with 2-4 community action agencies (8 to 20 counties) In Iowa. will implement a pilot outreach
project. The CM's will be geographically distributed around the stale, and indude both urban and rural areas. At least one
CM area will indUde a 'higpcon~n~ati-on of non-English speaking families.

Community action agencies represent an existing outreach netwol1c Vtilt1 an ongoing relationship with the comrmmities and low-
income families they serve. HAWK.' will be integrated with a wide array of services currently being provided by lhese
agencies, as well as a wider community effort to take the program I where people live". The result will be
an effectlve community outreach modellt1at can be expal1ded statewide.
Agencies will be selected based on a competitive application that is focused around the
