The four (4) agencies selected will be chosen from the eighteen (18) CAA's in the state on a competitive Request For Proposal (RFP) basis. The purpose of each pilot is to implement strategies to increase enrollment in the state's childrens' health insurance program. As you know, Operation: New View Community Action Agency has been deeply involved since day one in HAWK-I outreach activities through collaboration with our other community partners and stakeholders. We plan to submit a strong grant application and have our agency and service area (Dubuque, Delaware and Jackson counties) selected as one of the four pilot areas. With this in mind, we are seeking your help to secure this funding for our area. We would appreciate a letter from you supporting our application for this grant. The grants will be scored based on the strength of three factors: (1) The agency's organizational capacity to increase HAWK-I applications and enrollment; (2) The agency's capacity to utilize and develop innovative community partnerships and outreach strategies to achieve enrollments; and (3) The agency's capacity to effectively measure and report results. Hopefully in your letter you can address these factors in light of your experience in working with us in the HAWK-I outreach efforts and giving specific examples of our collaborative work. Your letter should be dated and addressed to the "HAWK-I Outreach Pilot Selection Committee." We would appreciate receiving your letter of support by Tuesday, July 11 th so it can be included in the application.
